Osteoarthritis day to day: a routine with a cosmetic CBD balm

For living with osteoarthritis, the recommendations of the Haute Autorité de Santé and Inserm put adapted physical activity, weight control and physiotherapy first. A cosmetic CBD balm can be worked into a daily massage ritual, as a matter of comfort, with no therapeutic claim.

Osteoarthritis in brief
Osteoarthritis is the progressive deterioration of joint cartilage, affecting the knee, hip, hands or spine. According to Assurance Maladie, it shows up as mechanical pain that worsens with use and eases with rest, short morning stiffness of under 30 minutes, a gradual loss of range of movement and, at times, inflammatory flare-ups with swelling. Risk factors include age, previous injury, excess weight and occupations that place heavy demands on the joints.
The pillars recommended by the HAS
The Haute Autorité de Santé recommends, for osteoarthritis of the knee and hip in particular, patient education, regular adapted physical activity such as walking or swimming, weight loss where needed (a 5% reduction already lessens the load on the joints), physiotherapy support and painkillers on medical advice. Surgery remains a last resort. None of these steps rests on a cosmetic, but a CBD balm can sit alongside them within a self-care ritual. The French framework for CBD cosmetics
Cannabidiol has been listed in the European Commission's CosIng database since February 2021. CBD balms and creams fall under European cosmetics regulation 1223/2009, and the ruling of the Conseil d'État of 29 December 2022 confirmed that THC in the finished product must remain below 0.30%. No pain-relieving, anti-inflammatory or analgesic action may be attributed to these cosmetics: the permitted vocabulary remains that of joint comfort and the massage ritual.
A concrete daily routine
In the morning, gently mobilising the affected joint for ten minutes, with targeted stretches, prepares for the day. During the day, a daily walk of at least 30 minutes and avoiding prolonged static positions limit stiffness. In the evening, a lukewarm shower followed by a massage with a CBD balm, in slow circles for three to five minutes over the affected joint, rounds off the ritual before gentle stretching. During a flare-up, applying cold remains the standard step. At night, seven to nine hours of sleep and a suitable pillow — or a cushion between the knees in cases of knee or hip osteoarthritis — support recovery.
What goes into a CBD joint balm
A typical formulation combines hemp-derived cannabidiol, plant butters such as shea or cocoa for texture, plant oils such as jojoba or sweet almond, menthol or camphor for the cooling sensation, together with waxes and excipients that comply with the cosmetics framework. Before buying, check the CBD content in milligrams, the certificate of analysis published by the brand, the full INCI list and compliance with the THC threshold.
Precautions
CBD is not a medicine. In the event of an osteoarthritis flare-up, intense pain or a locked joint, consult your GP or rheumatologist. Use is not advised for pregnant or breastfeeding women or for minors. A 24-hour skin tolerance test is recommended for sensitive skin, and medical advice remains necessary if you are on treatment. Frequently asked questions
Can a CBD balm replace my painkillers?
No. A cosmetic never takes the place of a prescribed medical treatment. If your doctor has prescribed a painkiller, do not stop it without advice; the balm sits alongside it as part of a massage ritual.
How often should the balm be applied?
Once or twice a day suits a wellbeing ritual. During a flare-up, your doctor remains the first person to turn to.
Can a CBD balm cause side effects?
The reactions reported mainly concern skin sensitivity to menthol or to excipients. A 24-hour tolerance test on a small area is recommended before the first wide application.
Does physical activity make osteoarthritis worse?
On the contrary, in the great majority of cases. Inserm and the HAS stress that adapted activity slows the clinical progression of osteoarthritis and preserves joint function; it is inactivity that makes the situation worse.
