- La molécule n'a pas d'antériorité pharmacologique propre : elle appartient à la série des indazole-3-carboxamides couplés à un acide aminé, décrite dans un brevet Pfizer publié en 2009, série restée absente du marché récréatif jusqu'en 2021. Les agonistes de synthèse porteurs d'un brome en position 5 du noyau indazole sont apparus après l'entrée en vigueur, en juillet 2021, du classement générique chinois des cannabinoïdes de synthèse, le brome servant à sortir du champ des définitions alors en vigueur. L'ADB-5'Br-PINACA a été signalé au système d'alerte précoce de l'Union européenne en 2022, et une monographie analytique lui a été consacrée par le Center for Forensic Science Research and Education en 2023. Son métabolisme a été décrit par Rautio et collaborateurs en 2024, et sa première caractérisation pharmacologique publiée revient à Gréen, Norman, McKenzie et collaborateurs, en 2025.

In plain terms
ADB-5'Br-PINACA is a synthetic cannabinoid made in a laboratory: it exists neither in hemp, nor in cannabis, nor in cannabidiol products. It is encountered above all deposited on smoking herbs, in vaping liquids or on impregnated paper, sold under another label, so that the person buying it has no idea what they are consuming or in what quantity. It acts on the same receptor as THC, but it activates it fully instead of partially, which makes it markedly more dangerous.

Pharmacology
ADB-5'Br-PINACA belongs to the generation of synthetic cannabinoid receptor agonists of the indazole-3-carboxamide class that appeared after the Chinese generic scheduling of July 2021: it is ADB-PINACA to which a bromine atom has been added at position 5 of the indazole ring, a modification intended to escape existing legal definitions. In vitro, the molecule behaves as a full agonist of the CB1 receptor, with an EC50 of 19.8 nM and a maximal efficacy of 108% relative to JWH-018, values measured in a calcium-release assay on CHO-K1 cells expressing the human receptor. This is the point that matters: THC is only a partial CB1 agonist and its effect plateaus, whereas full activation does not plateau, which is what makes this class dangerous. Bromine does not weaken potency, which remains indistinguishable from that of JWH-018 and close to that of ADB-BUTINACA. Beyond this measurement the literature is thin: the CB2 receptor has not been tested, no data exist on TRPV1, GPR55, PPARγ or 5-HT1A, and metabolism has been described only in vitro, in human hepatocytes, where ten metabolites were identified and the terminal hydroxylated derivative retained as a screening marker. No study in human beings has been published: human data are lacking, on effects as on toxicity, tolerance and dependence.

No biosynthetic pathway: the molecule is produced by no living organism and arises from organic synthesis, by acylation of a brominated indazole substituted on its nitrogen followed by coupling to the tert-leucine amide. Manufacture is described here only by chemical class.
Legal framework
France
Classified as a narcotic in France, but not under its own name. It falls under the generic definition in annex IV of the decree of 22 February 1990, devoted to indazole-3-carboxamide derivatives: the text covers those bearing on the nitrogen at position 1 an alkyl or haloalkyl substituent, and on the nitrogen of the carboxamide bridge a 1-amino-1-oxo-butan-2-yl group, whether or not that group is substituted at position 3 by one or two alkyl, cycloalkyl or phenyl substituents. The definition further specifies that it applies "whether or not the indazole ring is otherwise substituted": the bromine at position 5 therefore does not take the molecule outside the scope of the text, its pentyl tail satisfies the first branch and its tert-leucine amide head the third. The list names the non-brominated ADB-PINACA as well as 5F-ADB-PINACA, but no brominated compound of this series appears in it by name: the attachment is therefore generic and not nominative. Production, possession, transfer, importation and use fall under the narcotics regime. The substance has no food, cosmetic or therapeutic status and enters into no legally sold cannabidiol product.
European Union
No control measure specific to the Union level: the molecule does not appear among the substances listed in the annex to Framework Decision 2004/757/JHA, and no risk assessment report by the European drugs agency has been devoted to it. It is, however, monitored by the Union's early warning system, to which it was notified in 2022, within the series of brominated indazoles that appeared after the Chinese generic scheduling of July 2021; compounds of this same series have also been detected outside the Union, in the United States, Scotland, New Zealand, South Korea and Kuwait. Several Member States reach it through their own generic definitions, as France does: control today is national before it is European. Internationally, the molecule appears in the schedules of neither the 1961 Single Convention nor the 1971 Convention, and no assessment by the WHO Expert Committee on Drug Dependence has been published on it.

Agoniste synthétique des récepteurs cannabinoïdes de la famille des indazole-3-carboxamides : noyau indazole portant un atome de brome en position 5 du cycle benzénique, queue n-pentyle fixée sur l'azote en position 1, lien carboxamide et tête amide de tert-leucine, soit le groupement 1-amino-3,3-diméthyl-1-oxobutan-2-yle. C'est l'analogue 5-bromé de l'ADB-PINACA, dont il ne diffère que par cet atome de brome. Il ne doit pas être confondu avec l'ADB-5'Br-BUTINACA, qui porte une queue butyle, ni avec le MDMB-5'Br-PINACA, dont la tête est un ester méthylique : ce sont trois molécules distinctes, aux données propres.
Pharmacokinetics
Aucune donnée de pharmacocinétique humaine n'a été publiée : ni concentration sanguine, ni demi-vie, ni biodisponibilité, ni durée d'action. Les seuls éléments disponibles proviennent d'incubations sur hépatocytes et microsomes hépatiques humains, qui montrent une transformation étendue de la molécule en quelques heures. Le mode de consommation habituel de cette série, l'inhalation d'un support imprégné, conduit à un passage sanguin puis cérébral rapide, mais cela n'a pas été mesuré pour cette molécule. La quantité effectivement reçue reste imprévisible, la poudre se répartissant de façon irrégulière sur un support végétal.
Metabolism
Le métabolisme a été caractérisé sur hépatocytes humains, avec confirmation des métabolites majeurs par des références synthétisées et analyse en chromatographie liquide couplée à la spectrométrie de masse à temps de vol. Dix métabolites ont été identifiés. Les biotransformations relevées associent la formation d'un dihydrodiol, l'hydrolyse de la fonction amide terminale, l'hydroxylation, la déshydrogénation, la formation d'un carbonyle et la glucuronidation, ainsi que leurs combinaisons. Le métabolite hydroxylé en bout de chaîne pentyle est retenu comme marqueur recommandé pour le dépistage. Aucune perte de l'atome de brome ne figure parmi les biotransformations rapportées, et les enzymes responsables n'ont pas été identifiées individuellement.
Toxicology and risks
Aucun cas d'intoxication ni de décès attribué spécifiquement à cette molécule n'a été publié, et il n'existe aucune étude toxicologique réglementaire : les données humaines manquent entièrement. Ce silence de la littérature ne vaut pas innocuité, il traduit la nouveauté du composé. Le risque se déduit du mécanisme mesuré : l'activation complète de CB1, là où le THC ne produit qu'une activation partielle, supprime le plafond d'effet qui limite celui du cannabis. Les équipes qui ont étudié cette molécule rappellent que les agonistes de synthèse sont bien plus puissants que le THC et produisent des effets indésirables plus proches de ceux des psychostimulants que de ceux du cannabis, psychose, agressivité, dépendance, insuffisance rénale aiguë et décès inclus, et qu'ils sont régulièrement liés à des intoxications sévères et à des morts par surdose dans le monde. Ces éléments valent pour la classe, non pour cette seule molécule. Deux facteurs aggravent la situation des composés bromés récents : ils circulent sous une étiquette trompeuse, et la teneur varie fortement au sein d'un même produit. L'association avec l'alcool, les opioïdes ou les sédatifs majore le risque vital.
Detection and analysis
La molécule échappe aux tests immunologiques du cannabis, qui recherchent les métabolites du THC. Son identification repose sur la chromatographie couplée à la spectrométrie de masse : les saisies sont caractérisées en phase gazeuse et en phase liquide haute résolution de type temps de vol, la monographie analytique de 2023 donnant un ion [M+H]+ à 423,1390. Le brome facilite le repérage, cet atome produisant en spectrométrie de masse un profil isotopique caractéristique à deux pics d'intensité voisine séparés de deux unités de masse. En biologie, le dépistage cible le métabolite hydroxylé terminal plutôt que la molécule mère, et la glucuronidation observée in vitro justifie une hydrolyse enzymatique préalable des conjugués. Aucun intervalle de concentration de référence n'est établi dans les prélèvements humains.
