Sleeping with neck pain: pillow, posture, evening routine

Neck pain affects around two French people in three at least once in their lifetime, according to Inserm. To sleep better despite the pain, three proven levers: adapt your pillow to your dominant sleeping position, look after your daytime posture, and set up an evening routine with stretches or a soothing cosmetic.

Our head weighs five kilograms on average, and our neck supports it all day long. Smartphones, poorly adjusted workstations, sedentary habits: neck pain has become one of the most common musculoskeletal disorders. According to Inserm's collective expert appraisal on spinal pain, one person in five has had an episode of neck pain lasting more than 30 days over the past year. At night, the pain disrupts falling asleep and fragments sleep — which in turn worsens how pain is perceived.

Understanding neck pain

Neck pain refers to any pain located in the cervical spine (the first seven vertebrae). The causes are varied:

  • Common (non-specific) neck pain: the most frequent form, linked to muscle tension, poor posture or prolonged activity with the head bent forward.
  • Acute neck pain (wry neck): a sudden muscle contracture, often on waking after sleeping in an awkward position.
  • Neck pain linked to cervical osteoarthritis: progressive wear of the discs and joints, common after the age of 40.
  • Post-traumatic neck pain: following whiplash, a fall or a sports injury.
  • Neck pain with radiating symptoms (cervicobrachial neuralgia): pain running down the arm, a sign of nerve compression that requires medical assessment.

Assurance Maladie, the French national health insurance service, points out that the vast majority of neck pain resolves spontaneously within a few days to a few weeks. The warning signs that call for a consultation: intense pain that does not respond to painkillers, fever, neurological symptoms (weakness, persistent pins and needles), unexplained weight loss, or a recent fall or injury.

The right pillow for your dominant position

Your choice of pillow determines the alignment between head, neck and spine. Three practical cases:

If you sleep on your back, go for a thin to medium pillow that keeps the neck in line with the spine without pushing the chin towards the chest. Memory foam pillows with a cervical notch offer suitable support.

If you sleep on your side, choose a thicker pillow, one able to fill the gap between ear and shoulder without lifting the head excessively. The simple rule: the thickness should match the width of your shoulder, measured from the neck to the tip of the shoulder blade.

If you sleep on your front, ideally use no pillow at all, or a very flat one. This position twists the neck and increases pressure on the cervical spine. If you have chronic neck pain, try gradually shifting onto your side, sliding a pillow along your body as a support.

Beyond the pillow, also check your bedding: a sagging or overly soft mattress throws the whole spine out of alignment. French medical services recommend changing your mattress every 10 to 12 years, or sooner if you notice visible dips or discomfort on waking.

Five gentle stretches to prepare for the night

Five to ten minutes of stretching before bed relaxes the neck muscles and makes it easier to fall asleep. Do them slowly, without jerking, stopping at the first sensation of excessive tension:

  1. Side bend: sitting with a straight back, gently tilt your right ear towards your right shoulder. Place your right hand on the left side of your head without pulling. Five deep breaths. Repeat on the other side.
  2. Slow rotation: turn your head to the right until your chin passes your shoulder, without forcing. Hold for 5 seconds. Return to the centre. Repeat on the left.
  3. Posterior neck stretch: chin towards the chest, hands crossed behind the head, light pressure. Five deep breaths.
  4. Shoulder shrugs: raise your shoulders towards your ears, hold for 5 seconds, then release suddenly. Repeat 5 times.
  5. Trapezius self-massage: using the opposite hand's fingers, gently knead the muscles between the neck and the shoulder. 30 seconds on each side.

In the case of acute neck pain with sharp discomfort, do not stretch: instead apply heat (a hot water bottle or heat patch) for 15 to 20 minutes to release the contracture.

Look after your daytime posture to win the night

The night simply reveals the tension built up during the day. Three postural adjustments have a measurable impact on neck pain:

Workstation. Screen at eye level (the top of the screen level with your gaze), roughly an arm's length away, back against the chair, feet flat on the floor. If you work on a laptop, add a stand and an external keyboard — that is the most effective way to avoid "smartphone neck".

Phone. Raise the screen to eye level rather than lowering your head. For long calls, use a hands-free kit: holding the phone between shoulder and ear is one of the most frequent causes of wry neck.

Active breaks. Every 45 to 60 minutes at work, stand up, walk a few steps, roll your shoulders. Occupational health studies show that a break of 1 to 2 minutes every hour significantly reduces musculoskeletal disorders of the neck.

When might a CBD cosmetic be worth considering?

CBD balms and creams applied locally fall into the cosmetics category, the most favourable legal framework in France. Since February 2021, CBD has been listed in the European Commission's CosIng database, and in 2024 the Commission authorised the use of hemp leaves in cosmetic formulations.

A CBD balm does not treat neck pain, and no brand may legally present it as an anti-inflammatory or a painkiller — that would be a therapeutic claim, reclassifying the product as a medicine. What is permitted: "muscle comfort", "relaxing massage", "soothing cosmetic". What is prohibited: "relieves pain", "anti-inflammatory", "treats osteoarthritis".

The value of a CBD cosmetic for the neck lies above all in the massage gesture that goes with it: rubbing stimulates local circulation and releases contractures, massage heightens the sense of being cared for, and applying it before bed fits naturally into a soothing routine.

Whichever product you choose: check for a certificate of analysis (CBD content, absence of prohibited cannabinoids HHC, H4CBD, THCP, THC content ≤ 0.30%), in line with the position of MILDECA and the Conseil d'État decision of 29 December 2022.

Building an evening wind-down that supports sleep

Persistent neck pain disrupts sleep, and degraded sleep amplifies how pain is perceived — a vicious circle that the 2025 INSV survey documents well: people with chronic pain report on average two night-time awakenings, against one for the general population. To break the cycle, several adjustments beyond the choice of pillow and the stretches:

  • A cool bedroom (17-19 °C): heat amplifies muscular discomfort.
  • Screens off 30 to 60 minutes before bed: keeping your head bent over a smartphone right before sleeping tightens the neck.
  • A wind-down period: reading (a paper book or an e-ink reader, held at eye level), meditation, slow breathing.
  • No alcohol in the 3 hours before bed: alcohol degrades sleep quality and increases snoring, which can make neck pain worse in the morning.

When to see a doctor

Common neck pain settles within a few days to a few weeks. See your GP or an emergency department without delay if you have any of the following signs:

  • Intense pain that does not respond to over-the-counter painkillers.
  • Fever, general malaise, unexplained weight loss.
  • Neurological symptoms (weakness in an arm, persistent pins and needles, balance problems).
  • Neck pain following a recent injury (fall, accident).
  • Neck pain that worsens at night or consistently wakes you.

For chronic neck pain with no warning signs, your doctor may refer you to a physiotherapist or an osteopath depending on the case. The HAS points out that drug treatments should remain a last resort and be used for the shortest possible time, for both pain and sleep.

Paracetamol and ibuprofen are available without a prescription but are not harmless. Ibuprofen is contraindicated in cases of previous ulcer, kidney or heart failure, and is not advised for self-medication beyond 3 days. Ask your pharmacist for advice and stick to the stated doses.

Can yoga or meditation help?

Several clinical studies show a benefit of adapted yoga and mindfulness meditation for chronic neck pain, notably through reduced muscular stress and improved posture. Favour adapted classes (gentle yoga, therapeutic yoga) and tell the teacher about your neck pain.
