AM-7438 is a laboratory molecule, a chemical cousin of THC, developed in 2014 by an American academic team. It was designed to act quickly, then be rapidly cleaved by the enzymes of the blood. It does not exist in cannabis, is found in no commercial product and has never been studied in humans.

Pharmacology
AM-7438 is an analogue of (-)-Δ8-tetrahydrocannabinol in which the pentyl chain is replaced by a dimethylated chain bearing a cyanopropyl ester, a function deliberately made fragile in the face of plasma esterases. It binds to both cannabinoid receptors with nanomolar affinity (a Ki of 0.5 nM at rat brain CB1, 1.4 nM at human CB2) and behaves at CB1 as a full agonist: in the cAMP assay it reproduces close to 90% of the CP-55,940 response, whereas Δ9-THC is only a partial agonist. This full efficacy at CB1, shared with the synthetic cannabinoids encountered on the parallel market, is what most clearly separates this molecule from THC and makes its effects less predictable. In the rodent, it produces the hypothermia and analgesia characteristic of CB1 activation, with a more rapid onset and a shorter duration than the neighbouring analogues of the same series, and its acid metabolite is devoid of activity at both receptors. Beyond this single piece of animal pharmacology work, human data are entirely lacking: neither pharmacokinetics, nor clinical toxicology, nor a reported case.

No biosynthetic pathway: the molecule exists in no living organism. It is obtained by synthetic chemistry, by esterifying the side chain of a Δ8-tetrahydrocannabinol-type skeleton.
Legal framework
France
AM-7438 is named in no French decree. The molecule derives from the tetrahydrocannabinol skeleton and the generic clause of annex IV of the decree of 22 February 1990 classifies as narcotics "tetrahydrocannabinols, their esters, ethers, salts as well as the salts of the aforementioned derivatives": it is under this generic wording, and not through a listing by name, that an esterified derivative of Δ8-THC is apprehended. Its status therefore rests on the clause, with the reservation that the ester function here bears on the side chain and not on the phenol. In practice, it is a laboratory compound, without marketing authorisation or lawful use in France.
European Union
No control measure at Union level targets this molecule: it has been the subject neither of an EUDA (formerly EMCDDA) risk assessment nor of a Council decision, unlike several synthetic cannabinoids of the JWH, AM or MDMB series. Nor does it appear in the schedules of the international conventions of 1961 and 1971, which cover tetrahydrocannabinol and its isomers. Member States applying a generic definition of THC derivatives may nevertheless cover it, as the French reading does.

Pharmacokinetics
Les seules données disponibles viennent du rongeur. Dans le plasma de souris, l'hydrolyse enzymatique donne une demi-vie in vitro d'environ 45 minutes. La polarité plus forte que celle des analogues voisins limite le stockage de la molécule dans les tissus gras, ce qui explique l'installation plus rapide et la durée d'action plus courte observées sur l'hypothermie du rat, effet qui s'estompe en quelques heures. Le plasma humain contient beaucoup moins de carboxylestérases que celui des rongeurs, si bien que ces vitesses ne sont pas transposables à l'humain. Aucune donnée de biodisponibilité, de distribution ou d'élimination chez l'humain n'est publiée.
Metabolism
La voie dominante est l'hydrolyse de l'ester porté par la chaîne latérale, qui libère l'acide correspondant ; les auteurs indiquent qu'elle l'emporte largement sur les voies microsomales. Ce métabolite acide n'a plus d'affinité significative pour le CB1 ni pour le CB2 : la désactivation est intégrée à la structure même de la molécule, sur le principe des médicaments dits souples comme le rémifentanil. Aucun profil de métabolites humains n'est décrit.
Toxicology and risks
Aucune étude de toxicité, aucun cas clinique et aucune intoxication ne sont rapportés pour AM-7438 : les seules expositions publiées concernent le rat et la souris. Le point de vigilance est pharmacologique. Comme les cannabinoïdes de synthèse, cette molécule active le CB1 de façon complète, profil associé, pour d'autres composés de cette classe, à des convulsions, des troubles du rythme cardiaque, des états confusionnels, des vomissements incoercibles et des atteintes rénales aiguës. Ces observations appartiennent à ces autres substances et ne constituent pas des données propres à AM-7438, dont la marge de sécurité chez l'humain reste inconnue.
