AM281 is a laboratory molecule, entirely made by synthesis: it does not exist in hemp. It was developed to block the CB1 receptor, the one THC activates, and above all to serve as a radioactive tracer able to make those receptors visible in the brain during imaging examinations. Pharmacology

AM281 belongs to the family of diaryl pyrazole-3-carboxamides, like rimonabant, of which it is an iodinated analogue. It binds with an affinity of the order of 12 nM to the CB1 receptors of rat brain membranes, against about 4200 nM to the CB2 receptors of the spleen, that is a CB1 selectivity of about 350-fold. Functionally, it blocks the response to cannabinoid agonists and behaves as an inverse agonist: it lowers basal G protein activity below its resting level, and in rodents a single dose increases spontaneous locomotor activity, an observation that has served as an argument in favour of a permanent endocannabinoid tone. Antagonism of the GPR55 receptor is also attributed to it in several data sets, but the results vary between assays and remain discordant. Labelled with iodine-123 or iodine-131, the molecule has served as a radioligand for single-photon emission tomography, in the baboon and then in a few patients, with a weak specific signal that has limited its use and left the field to later tracers. Outside these tracer doses, human data are wholly lacking: no evaluation of efficacy or of safety has been carried out.

No known biological pathway: AM281 comes from no living organism. It is obtained by synthesis within the class of diaryl pyrazole-3-carboxamides, by structural analogy with rimonabant, an aromatic iodine being introduced to allow radiolabelling.

Pharmacokinetics

Les seules données humaines proviennent d'une étude d'imagerie conduite à doses traceuses. Après injection intraveineuse de la forme marquée à l'iode 123, la décroissance plasmatique est biphasique et environ 60 % de la radioactivité correspond encore à la molécule intacte au bout de trois heures. La captation cérébrale est mesurable mais le rapport de liaison spécifique reste faible, de 0,19 à 0,31 dans les noyaux lentiformes, ce qui traduit une cinétique lente et une lipophilie élevée. Chez le rongeur, l'occupation des récepteurs CB1 après administration intraveineuse suit la dose, une valeur de 0,45 mg/kg déplaçant la moitié du traceur. Aucune donnée orale, de biodisponibilité ou de demi-vie d'élimination formelle n'est publiée.

Metabolism

Très peu caractérisé. L'analyse du plasma humain par chromatographie sur couche mince montre une dégradation progressive, avec environ 60 % de composé inchangé à trois heures, sans identification des métabolites formés. Aucune étude d'enzymologie hépatique, de contribution des cytochromes P450 ni de voie d'excrétion n'est disponible.

Toxicology and risks

Il n'existe aucune donnée de sécurité chez l'humain en dehors des doses traceuses d'imagerie, où l'exposition radiologique a été jugée faible, de l'ordre de 0,011 mSv/MBq pour la forme marquée à l'iode 123. Le principal signal de risque est celui de la classe : les agonistes inverses de CB1 abaissent l'activité de base du système endocannabinoïde, et le rimonabant, chef de file commercialisé puis retiré du marché en 2008, a provoqué anxiété, humeur dépressive et idées suicidaires lors d'usages prolongés. Chez le rongeur, les agonistes inverses de cette famille produisent des comportements évocateurs de nausée et des effets anxiogènes que les antagonistes neutres ne reproduisent pas. AM281 n'a fait l'objet d'aucune étude publiée de toxicité à doses répétées.

Detection and analysis

Aucune méthode médico-légale validée n'est publiée pour AM281, qui n'est pas suivie comme nouveau produit de synthèse par les systèmes d'alerte européens. En recherche, les formes radiomarquées à l'iode 123 ou 131 se mesurent par comptage gamma et par tomographie d'émission monophotonique ; l'identification de la molécule non marquée relèverait d'une chromatographie liquide couplée à la spectrométrie de masse, la présence d'un iode aromatique et de deux atomes de chlore donnant un profil isotopique reconnaissable.
