- Décrite pour la première fois en 2009 par Buchler et ses collaborateurs, dans une demande de brevet déposée pour Pfizer à l'issue d'un programme de recherche sur la douleur ; seul l'énantiomère (S) figurait dans ce brevet. La molécule est ensuite apparue sur le marché des nouvelles substances psychoactives : en 2015, le Centre commun de recherche de la Commission européenne et les laboratoires des douanes l'ont caractérisée dans une poudre saisie par la douane belge sur un colis venu de Chine et déclaré comme « pigments blancs », en mélange avec le 5F-AMB. Elle a également été retrouvée dans des produits et du matériel de consommation lors de l'épisode d'intoxications survenu à Anchorage, en Alaska, entre 2015 et 2016.

APP-CHMINACA, also known as PX-3, is a synthetic cannabinoid made in the laboratory. It does not occur in hemp and has nothing to do with CBD: it is found deposited on herbs for smoking or in vaping liquids, almost always sold under another name. It acts on the same brain receptors as THC, but far more forcefully, and human data on it are almost entirely lacking.

Pharmacology
APP-CHMINACA is a synthetic cannabinoid receptor agonist of the indazole-3-carboxamide family, described in 2009 in a Pfizer patent application before being identified on the new psychoactive substances market in 2015. The published binding data ascribe to it an affinity of 9.81 nM at CB1 and 4.39 nM at CB2; a functional [35S]GTPγS assay gives the (S) enantiomer a potency of 251 nM at CB1 and 8.09 nM at CB2. The molecule is therefore less potent at CB1 than most of its analogues, but it shares their full-agonist efficacy, where Δ9-THC remains a partial agonist, and the (R) enantiomer is very much less active. Two metabolism studies, one on human liver microsomes, the other combining in vitro and in vivo models, describe rapid transformation dominated by hydrolysis of the terminal amide and hydroxylation of the cyclohexyl ring, and propose the corresponding analytical markers. By contrast, no clinical pharmacology study exists, no series of poisonings is attributed to it in isolation, and human data are lacking: what is known comes from seizures, forensic work and an episode of poisonings in Alaska where it was among the molecules found.

Origin (pharmaceutical research → illicit market)
No biological pathway: the molecule is produced by no plant and no living organism. It arises from organic synthesis, by amide coupling between an indazole-3-carboxylic acid bearing a cyclohexylmethyl group on nitrogen 1 and an amide derived from phenylalanine. It has no kinship with the cannabigerol pathway that produces the cannabinoids of hemp.
Legal framework
France
A narcotic in France, but without being named. The decree of 31 March 2017, which amends annex IV of the decree of 22 February 1990, listed twelve generic families of synthetic cannabinoids, together with their isomers, stereoisomers, esters, ethers and salts; among these families are the indazole 3-carboxamide derivatives, the substances subsequently cited being given as examples. APP-CHMINACA falls under this family clause: indazole core, carboxamide at position 3, cycloalkylmethyl substituent on the nitrogen and an amine head of the 1-amino-1-oxo type bearing a phenyl group, a motif shared by 5F-APP-PICA and 5F-APP-PINACA, two substances expressly named in the same text. It is therefore not the mention of "tetrahydrocannabinols, their esters, ethers, salts" in annex IV that applies, since the molecule is not a THC derivative. Use, possession, transfer and trafficking are prohibited and criminally punishable.
European Union
National control, with no European measure of its own. APP-CHMINACA has been the subject neither of a joint report by the European drugs monitoring centre and Europol, nor of a European risk assessment, and it does not appear in the annex to Framework Decision 2004/757/JHA listing the substances subject to control measures throughout the Union. It remains monitored by the Early Warning System, its first analytical characterisation in Europe having been published in 2016 by the Commission's Joint Research Centre together with the customs laboratories. Control therefore rests with each Member State, several of which cover it through national generic definitions, as France has done since 2017. At international level, the molecule appears neither in the Single Convention of 1961 nor in the 1971 Convention on Psychotropic Substances, and no assessment by the WHO Expert Committee on Drug Dependence has been published on it.

Agoniste synthétique des récepteurs cannabinoïdes de type indazole-3-carboxamide : noyau indazole, queue cyclohexylméthyle portée par l'azote 1, lien carboxamide en position 3 et tête amide dérivée de la phénylalanine, d'où le préfixe APP. La molécule est le membre indazolique à queue cyclohexylméthyle de la série dite PX, aux côtés du 5F-APP-PICA et du 5F-APP-PINACA. Aucune parenté structurale avec les cannabinoïdes terpénophénoliques du chanvre.
Pharmacokinetics
La consommation se fait par inhalation d'un support végétal imprégné ou d'un liquide à vapoter, et rien ne permet de savoir ce que contient réellement un produit, la répartition sur le support étant très inégale. Chez la souris, après administration parentérale, la molécule mère et ses principaux métabolites atteignent leur maximum sanguin en une trentaine de minutes puis diminuent nettement à trois heures ; dans l'urine, les quatre métabolites majoritaires culminent entre une heure et demie et deux heures et demie et restent détectables sur les vingt-quatre heures explorées. In vitro, la clairance microsomale rapportée dans la littérature est élevée, avec une demi-vie de l'ordre de cinq minutes sur microsomes hépatiques, ce qui annonce une disparition rapide de la molécule mère. Aucune étude de pharmacocinétique humaine n'a été publiée.
Metabolism
Métabolisme hépatique rapide et étendu. Sur microsomes hépatiques humains, douze métabolites répartis en sept classes ont été caractérisés, deux voies dominant le tableau : l'hydrolyse de la fonction amide terminale, qui libère le produit de désamidation, et l'hydroxylation du cycle cyclohexyle porté par l'azote de l'indazole. Les auteurs proposent comme cibles analytiques le produit d'hydrolyse simple, son homologue monohydroxylé et un dérivé dihydroxylé du cycle cyclohexyle. Un travail ultérieur, combinant modèles in vitro et in vivo, confirme ces deux voies, ne relève pas de différence entre microsomes humains et murins, retient le produit d'hydrolyse comme meilleur marqueur sanguin et ses dérivés hydroxylés comme marqueurs urinaires les plus abondants et les plus durables. Comme pour les autres cannabinoïdes de synthèse, une part de l'excrétion se fait sous forme de conjugués glucuronidés, ce qui impose une hydrolyse enzymatique avant analyse.
Toxicology and risks
Le risque tient à l'agonisme complet du récepteur CB1, propriété que le Δ9-THC ne possède pas et qui explique la brutalité des tableaux cliniques observés avec cette famille : agitation, confusion, convulsions, tachycardie, vomissements, atteinte rénale aiguë et décès sont documentés pour plusieurs indazole-3-carboxamides, mais ces observations appartiennent à ces analogues et non à l'APP-CHMINACA. Pour cette molécule précise la documentation est mince, et il faut le dire clairement : aucune série d'intoxications ni aucun décès ne lui sont attribués isolément dans la littérature. Elle figure en revanche parmi les onze cannabinoïdes de synthèse identifiés dans les échantillons analysés lors de l'épisode d'Anchorage, en Alaska, entre juillet 2015 et mars 2016, épisode qui a donné lieu à 1 351 transports en ambulance, avec 6,6 % de patients intubés et 10,2 % d'admissions en réanimation ; la part revenant à chaque molécule n'y est pas départageable. Aucune marge de sécurité n'est établie, aucune donnée humaine contrôlée n'existe, et la teneur des produits vendus varie fortement d'un lot à l'autre.
Detection and analysis
Les tests urinaires du cannabis ne détectent pas cette molécule, car ils ciblent le métabolite carboxylé du THC : une recherche spécifique doit être demandée. L'identification repose sur la chromatographie liquide couplée à la spectrométrie de masse en tandem ou à haute résolution ; la molécule mère disparaissant vite, l'analyse urinaire doit viser les métabolites, en particulier les dérivés hydroxylés du produit d'hydrolyse de l'amide. La caractérisation initiale de la poudre saisie a associé résonance magnétique nucléaire et spectrométrie de masse, et la substance figure dans les méthodes de criblage par chromatographie en phase gazeuse mises au point pour les cannabinoïdes de synthèse. Enfin, les deux énantiomères n'ayant pas la même activité, une séparation sur colonne chirale de type polysaccharide est nécessaire lorsque la configuration doit être établie.
