- Le chef de file de la série, le MDA-19 ou BZO-HEXOXIZID, a été décrit en 2008 par Diaz et ses collègues, dans une série d'acylhydrazones d'isatine N-alkylées étudiées comme agonistes sélectifs du récepteur CB2 pour la douleur neuropathique, travaux rattachés au M.D. Anderson Cancer Center. Le BZO-POXIZID, son analogue N-pentyle, est apparu sur le marché récréatif en 2021 : signalé aux États-Unis par le CFSRE en octobre 2021, puis identifié en Chine dans des saisies de poudres et de liquides à vapoter, après l'entrée en vigueur au 1er juillet 2021 de l'interdiction générique chinoise visant sept squelettes de cannabinoïdes de synthèse. Sa pharmacologie a été caractérisée en 2022 par le laboratoire de toxicologie de l'Université de Gand.

BZO-POXIZID is a synthetic cannabinoid made in a laboratory, with no link whatsoever to hemp. It was designed to imitate the effects of cannabis while escaping the lists of prohibited substances, and circulates as a powder or sprayed onto herbs and into vaping liquids. It is not a wellbeing product, it belongs to none of our ranges, and its effects in human beings have never been studied.

Pharmacology
BZO-POXIZID belongs to the OXIZID class, isatin acylhydrazones whose oxindole core was chosen to fall outside the scope of the generic prohibitions covering indoles and indazoles. Its lead compound, MDA-19, had been designed in 2008 as a selective agonist of the CB2 receptor, and therefore in theory barely psychoactive; shortening the side chain to a pentyl group alters that profile. In a beta-arrestin 2 recruitment assay, BZO-POXIZID behaves as a full agonist at CB1, with an EC50 of 244 nM and a maximal efficacy far above that of the CP55,940 reference, and as a partial agonist at CB2, with an EC50 of 12.2 nM; it is the least CB2-selective compound of the series tested, which argues for genuine psychoactive activity. A second team, working on coupling to Gαi proteins and on pharmacological discrimination in mice, nonetheless places the OXIZIDs behind previous generations of synthetic cannabinoids, in potency as in efficacy. Beyond this in vitro observation, everything is missing: no clinical study, no human pharmacokinetic data, no poisoning case published specifically for this molecule. Metabolism, by contrast, is well described in vitro, dominated by hydroxylation of the N-pentyl chain and of the phenyl ring as well as by N-dealkylation, under the action of CYP3A4, CYP3A5 and CYP2C9.

No known biosynthetic pathway: BZO-POXIZID comes from no living organism and has never been detected in a plant. It is obtained by a purely chemical route, described here by its reaction class alone, the condensation of an N-alkylated isatin with a benzohydrazide, which forms the acylhydrazone characteristic of the OXIZID class.
Legal framework
France
BZO-POXIZID is not listed by name on the French list of narcotics: no reference to this molecule or to the OXIZID class appears in the texts published on Légifrance. Nor is it captured by the generic clauses of annex IV of the decree of 22 February 1990. That annex covers, on the one hand, tetrahydrocannabinols, their esters, ethers and salts as well as the salts of the aforementioned derivatives, which BZO-POXIZID is not, and, on the other hand, a closed list of synthetic cannabinoid scaffolds introduced by the decree of 31 March 2017: indol-3-yl methanones, indazol-3-yl methanones, naphthoylpyrroles, naphthylmethylindoles, cyclohexylphenols, indole and indazole carboxamides and carboxylates, pyrrolo[3,2-c]pyridine carboxamides, thiazolylindoles. An oxindole core linked to a benzohydrazide belongs to none of these families, and that is precisely what the manufacturers were seeking. The substance is therefore not classified to date, but the status remains in flux: the ANSM revises this list regularly, as its decisions of May and June 2024 on synthetic and semi-synthetic cannabinoids showed. The absence of classification amounts neither to an authorisation to sell nor to a presumption of harmlessness.
European Union
No control measure at European Union level covers BZO-POXIZID. Synthetic cannabinoids fall under national legislation, which is highly heterogeneous: Hungary added BZO-POXIZID, BZO-CHMOXIZID and BZO-4en-POXIZID to its list of controlled new psychoactive substances in December 2021, while other member states do not mention them. The class is monitored by the early warning system of the EUDA, formerly the EMCDDA, to which the lead compound MDA-19 had been notified from Spain as early as October 2016. Outside the Union, China introduced a generic ban on seven scaffolds with effect from 1 July 2021, which the OXIZID class was designed precisely to circumvent; no listing in the schedules of the international conventions has been adopted for this molecule.

Cannabinoïde de synthèse de la classe OXIZID, chimiquement une acylhydrazone d'isatine : un noyau oxindole, c'est-à-dire une 2-oxo-indoline portant une chaîne N-pentyle, relié par un pont hydrazone à un groupe benzamide. Le nom OXIZID condense d'ailleurs le noyau oxindoline et le lien hydrazide. Ce squelette ne comporte ni indole-3-carboxamide, ni indazole-3-carboxamide, ni indol-3-yl méthanone, ce qui le sépare nettement des générations précédentes de cannabinoïdes de synthèse.
Pharmacokinetics
Les données humaines manquent : aucune étude de pharmacocinétique n'a été publiée chez l'être humain, et ni l'absorption, ni la distribution, ni la demi-vie ne sont quantifiées. Les seuls éléments disponibles proviennent de modèles in vitro sur microsomes et hépatocytes humains, qui montrent une instabilité métabolique marquée, portée par les cytochromes P450. Sur le marché, l'exposition se fait par inhalation de vapeur ou par combustion de matières végétales imprégnées, deux voies pour lesquelles la quantité réellement délivrée reste impossible à estimer, la teneur des produits variant fortement d'un lot à l'autre.
Metabolism
Le métabolisme du BZO-POXIZID a été cartographié in vitro sur microsomes et hépatocytes humains : plusieurs dizaines de métabolites ont été détectés pour chaque OXIZID étudié. Les voies principales sont l'hydroxylation de la chaîne N-alkyle et du cycle phényle, la N-désalkylation, l'oxydation en cétone puis en carboxylate, et l'hydrolyse de la fonction amide ; l'analogue fluoré subit en outre une défluoration oxydative. Les cytochromes CYP3A4, CYP3A5 et CYP2C9 portent l'essentiel de la transformation. Une glucuronoconjugaison secondaire intervient, si bien qu'une hydrolyse enzymatique des glucuronides est recommandée avant l'analyse urinaire.
Toxicology and risks
Les données humaines manquent. Aucun cas clinique d'intoxication attribué spécifiquement au BZO-POXIZID n'a été publié, et aucun seuil de toxicité n'est établi. Le risque se déduit de la classe : les agonistes de synthèse du récepteur CB1 sont associés dans la littérature à de l'agitation, des épisodes psychotiques, des hallucinations, des convulsions, des effets cardiovasculaires, des détresses respiratoires, des comas et des décès. Le point critique est ici l'efficacité, car le BZO-POXIZID est un agoniste complet du CB1 alors que le THC n'en est qu'un agoniste partiel, ce qui supprime le plafond d'effet propre au cannabis. Chez le poisson-zèbre, le chef de file MDA-19 a réduit la longueur corporelle et l'activité locomotrice, altéré le développement des motoneurones spinaux et augmenté les espèces réactives de l'oxygène. Enfin, cinq expositions involontaires à l'analogue BZO-4en-POXIZID ont été mises en évidence par analyse capillaire chez des personnes fréquentant des clubs new-yorkais, ce qui montre que ces molécules circulent aussi comme adultérants, à l'insu de celles et ceux qui consomment.
Detection and analysis
Le BZO-POXIZID échappe aux tests cannabis courants, qui ciblent le THC et son métabolite carboxylé. Son identification repose sur la chromatographie liquide couplée à la spectrométrie de masse haute résolution ou en tandem, complétée pour les saisies par la chromatographie gazeuse couplée à la spectrométrie de masse, l'infrarouge à transformée de Fourier et la résonance magnétique nucléaire. En urine, les marqueurs les plus fiables sont la molécule mère accompagnée de deux métabolites mono-hydroxylés, l'un sur la chaîne N-alkyle, l'autre sur le cycle phényle. Les cheveux permettent de documenter une exposition répétée sur plusieurs mois. En spectrométrie de masse, les OXIZID se signalent par une fragmentation atypique, sans ions caractéristiques du noyau ni du groupe terminal, ce qui complique l'identification par bibliothèque.
