Décrit en 2008 par Diaz, Xu, Astruc-Diaz, Pan, Brown et Naguib, au University of Texas M. D. Anderson Cancer Center, au sein d'une série de dérivés N-alkyl isatine acylhydrazone conçus comme agonistes sélectifs du récepteur CB2 contre la douleur neuropathique, puis caractérisé pharmacologiquement en 2010 sous le code MDA19 et couvert par une demande de brevet sur les modulateurs hydrazone des récepteurs cannabinoïdes. La molécule réapparaît comme produit de rue en septembre 2021 : un matériel végétal saisi par le laboratoire d'Indianapolis-Marion County est transmis au CFSRE, qui publie son rapport d'identification le 19 octobre 2021 et propose la même année la nomenclature systématique OXIZID.

Arising from an academic analgesic research programme, it was diverted towards the synthetic cannabinoid market after the Chinese generic ban of July 2021, which targeted the classical indole and indazole scaffolds: the isatin acylhydrazone core of the OXIZIDs escaped that definition. It circulates as impregnated plant material, resin, powder or vaping liquid, very often in mixtures. A Brazilian series of 53 seizures between September 2021 and February 2022 contained it, some of them combined with other CB1 agonists, with cocaine or with caffeine. In France, it has been found in two samples of « la chimique », a mixture of tobacco and synthetic cannabinoids consumed in Mayotte.

In plain terms

BZO-HEXOXIZID, also called MDA-19, has nothing to do with hemp: it is a laboratory molecule, developed in 2008 by American researchers working on nerve pain. It reappeared in 2021 in smoking mixtures sold on the street, replacing synthetic cannabinoids that had become prohibited. Pharmacology

BZO-HEXOXIZID belongs to the OXIZID subclass of synthetic cannabinoid receptor agonists, built on an isatin acylhydrazone scaffold foreign to hemp chemistry. Its initial characterisation, in 2008 and then in 2010, describes it as a preferential ligand of the CB2 receptor (Ki of 43.3 nM at the human CB2 against 162.4 nM at the human CB1), able to reduce allodynia in rat models of neuropathic pain without altering locomotor activity. This profile, hardly conducive to recreational use, made its appearance on the illicit market difficult to explain, and the forensic literature has explicitly treated it as a puzzle. Work carried out after 2021 qualifies the initial reading: in β-arrestin recruitment and Gi protein activation assays, the molecule does indeed activate CB1, albeit with a potency and an efficacy lower than those of earlier generations of synthetic cannabinoids, and with an activity profile close to that of THC in certain models. The consistency of these results remains imperfect across species and across assays, and human data are lacking: neither a clinical series nor published pharmacokinetics. What is documented amounts to rapid hepatic metabolism by the cytochromes CYP3A4, CYP3A5 and CYP2C9, developmental neurotoxicity observed in zebrafish, and circulation almost always in mixtures, which makes any attribution of effect uncertain.

No biosynthetic pathway: the molecule does not exist in living organisms and does not derive from the cannabigerol pathway. It is obtained by chemical condensation between an N-alkylated isatin and a benzohydrazide, a chemistry unrelated to that of plant cannabinoids.

Acylhydrazone d'isatine N-alkylée (benzohydrazide d'oxindol-3-ylidène), chef de file de la sous-classe OXIZID des agonistes synthétiques des récepteurs cannabinoïdes. Le squelette ne présente aucune parenté avec le squelette terpénophénolique des cannabinoïdes du chanvre.

Pharmacokinetics

Substance très lipophile, comme l'ensemble des agonistes synthétiques des récepteurs cannabinoïdes, et fortement liée aux protéines plasmatiques, deux propriétés qui ralentissent la clairance de cette famille. Les voies d'usage rapportées sont l'inhalation et l'ingestion. Les incubations sur microsomes et hépatocytes humains la décrivent comme métaboliquement instable, ce qui plaide pour une disparition rapide de la molécule mère au profit de ses métabolites. Aucune étude de cinétique chez l'humain n'a été publiée : ni concentrations plasmatiques, ni demi-vie, ni biodisponibilité.

Metabolism

Le métabolisme est hépatique et repose principalement sur les cytochromes CYP3A4, CYP3A5 et CYP2C9. Les profils obtenus sur microsomes et hépatocytes humains montrent une hydroxylation de la chaîne N-hexyle et du cycle phényle, une N-désalkylation, la formation de cétones et de dihydrodiols, puis une glucuroconjugaison. Plusieurs dizaines de métabolites ont été décrits pour chaque composé de la série. Les auteurs retiennent la molécule mère et les métabolites monohydroxylés, sur la chaîne alkyle comme sur le phényle, en tant que marqueurs urinaires les plus utiles, et signalent qu'une hydrolyse enzymatique des glucuronides améliore la détection.

Toxicology and risks

Les données humaines manquent : aucune série clinique d'intoxication attribuée spécifiquement à cette molécule n'a été publiée, et sa présence quasi constante en mélange empêche d'isoler sa responsabilité. Chez l'animal, une étude sur larves de poisson-zèbre rapporte une neurotoxicité développementale, avec réduction de la nage et de l'activité, atteinte du développement des motoneurones, élévation des espèces réactives de l'oxygène et perturbation du métabolisme énergétique. En pharmacologie comportementale chez le rongeur, la molécule ne substitue pas complètement au stimulus discriminatif du THC. L'ANSM associe à la famille des cannabinoïdes de synthèse des effets aigus graves : vomissements, perte de connaissance, coma, convulsions, paranoïa, anxiété, hypertension artérielle, tachycardie, douleurs thoraciques, ainsi que des cas d'insuffisance rénale, avec un risque de dépendance. La composition réelle des produits vendus ne correspond souvent pas à l'étiquetage et les concentrations sont inconnues de l'usager.

Detection and analysis

Identification en routine par chromatographie en phase gazeuse ou liquide couplée à la spectrométrie de masse. Le rapport de chimie du CFSRE décrit en GC-MS des fragments caractéristiques à m/z 244, 105 et 77, et une masse exacte de 350,1863 pour l'ion [M+H]+. Sur le plan biologique, les tests immunochimiques urinaires classiques ne détectent pas ces substances : l'ANSM demande une recherche ciblée par chromatographie liquide couplée à la spectrométrie de masse en tandem dans le sang ou les urines, complétée si possible par l'analyse du produit consommé via le dispositif SINTES de l'OFDT. Des expositions ont également été documentées par analyse segmentaire de cheveux pour un composé voisin de la série.
