Pharmacology

CUMYL-PEGACLONE is a full agonist at the cannabinoid receptors, with affinities of the same order for hCB1 (Ki of 1.37 nM) and hCB2 (Ki of 2.09 nM) and functional activation of CB1 obtained at nanomolar concentrations, or even lower depending on the assay used. It recruits mini-Gαi and beta-arrestin 2 very efficiently, with an efficacy greater than that of the reference agonist JWH-018: it is this maximal activation, where THC behaves only as a partial agonist, that separates the two molecules in terms of risk. The rest of the file is thin. No clinical study, no controlled assessment of abuse potential in humans, an almost unexplored pharmacokinetics, and observations drawn almost solely from forensic medicine: a few poisonings, convulsions in two adolescents exposed without their knowledge, and series of deaths in which the substance was found, most often alongside other products. Human data are lacking to go further.

No known biosynthetic pathway: this molecule exists in no living organism and comes exclusively from clandestine chemical synthesis on a gamma-carbolinone nucleus, derived from a series of compounds covered by a pharmaceutical patent from the early 2000s.

Agoniste synthétique des récepteurs cannabinoïdes appartenant à la classe des gamma-carbolin-1-ones, construite sur un noyau pyrido[4,3-b]indol-1-one portant une chaîne N-pentyle et un groupement cumyle. Cette architecture correspond à la version cyclisée du cumyl-PICA, la fonction amide étant ici enfermée dans un cycle lactame.

Pharmacokinetics

Les données humaines d'absorption et de distribution manquent. La voie d'usage documentée est l'inhalation, par combustion de végétaux imprégnés ou par vapotage. Les concentrations sériques relevées en contexte médico-légal post mortem s'échelonnent de 0,38 à 34,9 nM. La substance est fortement métabolisée : le composé inchangé n'est pas retrouvé dans les urines authentiques, ce qui oblige à cibler les métabolites. Un témoignage d'usager décrit une installation rapide et une durée d'environ une heure, mais aucune étude pharmacocinétique contrôlée n'a été conduite.

Metabolism

L'analyse d'urines authentiques a permis de décrire 22 métabolites de phase I, dont 16 confirmés ensuite sur microsomes hépatiques humains. Les transformations observées associent mono et dihydroxylations, déshydrogénation, N-désalkylation, dégradation de la chaîne pentyle jusqu'à un métabolite acide propionique et formation d'une fonction carbonyle sur cette même chaîne. Deux métabolites monohydroxylés dominent et servent de marqueurs médico-légaux de consommation. Un produit de dégradation thermique, la N-pentyl-gamma-carbolinone, se forme au chauffage et peut être retrouvé dans les prélèvements biologiques.

Toxicology and risks

Les données humaines contrôlées manquent : aucune étude toxicologique préclinique n'avait été conduite avant la diffusion de la molécule, et son potentiel d'abus comme de dépendance n'a jamais été évalué chez l'humain. Elle a été retrouvée dans le sang fémoral post mortem de six personnes en Europe, mais d'autres substances étaient présentes dans cinq de ces six cas, ce qui interdit toute attribution formelle. En Australie, une série de cinq décès survenus en dix-huit mois dans le Territoire du Nord rapporte des concentrations sanguines de 0,73 à 3,0 µg/L, sur un fond de maladie cardiovasculaire et de consommation associée d'alcool. Deux adolescents ont convulsé après avoir fumé, à leur insu, un produit qui en contenait. Chez la souris, la molécule produit les effets cannabinoïdes classiques avec la puissance la plus élevée parmi les trois agonistes comparés, et l'administration d'un antagoniste CB1 déclenche des signes de sevrage. Ce profil d'agoniste complet à très forte puissance expose à des effets bien plus intenses que ceux du THC, sans aucune marge de sécurité établie.

Detection and analysis

Le composé se prête à une identification par chromatographie en phase gazeuse ou liquide couplée à la spectrométrie de masse, ainsi que par résonance magnétique nucléaire sur les produits saisis ; son maximum d'absorption ultraviolet se situe à 252 nm. En pratique médico-légale, le criblage urinaire vise les métabolites monohydroxylés plutôt que la molécule mère, absente des urines. Des méthodes ont également été publiées pour les cheveux et pour les liquides de cigarette électronique. Les teneurs des mélanges végétaux saisis varient fortement, de 8,6 à 146 mg par gramme sur une série de vingt produits, avec des écarts mesurables au sein d'un même sachet.
