Pharmacology
JWH-210 belongs to the naphthoylindoles, the first major generation of synthetic cannabinoids found in Spice-type mixtures. It is an agonist at the CB1 and CB2 receptors, with affinities of the order of 0.46 nM at CB1 and 0.69 nM at CB2, roughly a hundred times that of THC at CB1. The nature of the activation counts as much as the potency: THC activates CB1 only partially, whereas JWH-210 activates it fully, which removes the relative margin of safety observed with cannabis. A series of 22 patients admitted to emergency departments with JWH-210 as the sole synthetic cannabinoid in serum, between 0.18 and 90 ng/mL, reports nausea, tachycardia, drowsiness, hypokalaemia, hypertension and agitation, with cases of diplopia, seizures, syncope and electrocardiogram abnormalities. An observational study in eight users who inhaled 2 to 3 mg conversely describes marked subjective effects but modest cardiovascular impact, which above all illustrates the variability of responses. No therapeutic indication is established, no controlled clinical trial exists, and human data are confined to case series and field observations.

Origin (pharmaceutical research → illicit market)
No biosynthetic route: JWH-210 is produced by no living organism and appears in no hemp chemotype. It belongs to synthetic chemistry alone, by acylation of an N-pentylated indole with a 4-ethylnaphthalene derivative, the carbonyl bond linking the two aromatic systems.
Legal framework
France
A substance classified as a narcotic in France, and cited by name. JWH-210 appears in annex IV of the decree of 22 February 1990 establishing the list of substances classified as narcotics, in the naphthoylindole family introduced by the decree of 19 May 2015: the family is defined there generically, then illustrated by a list of compounds cited by name, including JWH-210 with its full chemical designation. This is therefore not a mere attachment through a generic clause. Production, possession, transfer and use fall under the criminal narcotics regime. This molecule has no place in the CBD trade and is not present in any product sold here.
European Union
No synthetic cannabinoid of the JWH series is listed in the international conventions of 1961 and 1971, and JWH-210 has not been the subject of a control measure taken at Union level. Control is therefore national, and the molecule is classified in many member states, for example in annex II of the German narcotics act. It also falls within the European early warning system, which has monitored synthetic cannabinoids since 2010 under the aegis of the agency now called the EUDA. Outside the Union, it is classified in Schedule I in the United States and in class B in the United Kingdom.

Naphtoylindole de la série JWH, analogue 4-éthylé du JWH-018: un indole N-pentylé porte en position 3 un groupe carbonyle relié à un naphtalène substitué par un éthyle en position 4. Aucun squelette terpénophénolique, donc aucune parenté structurale avec les cannabinoïdes du chanvre, la ressemblance se limitant au site de fixation sur le récepteur.
Pharmacokinetics
Voie d'exposition habituelle: inhalation du mélange végétal imprégné. Chez huit usagers ayant inhalé 2 à 3 mg, les effets subjectifs culminent vers 20 minutes; dans la salive, la concentration maximale mesurée est d'environ 22,5 ng/mL à 20 minutes, retombe à environ 4,3 ng/mL à une heure et devient indétectable au-delà de trois heures. Chez le rat, après administration sous-cutanée, le profil sérique est biphasique, avec un premier pic à une heure et un second vers quatre heures, la molécule restant mesurable à 24 heures. La forte lipophilie favorise le stockage tissulaire, si bien que la durée des effets ressentis et la durée de présence analytique ne coïncident pas.
Metabolism
Métabolisme hépatique intense, dominé par des hydroxylations portant sur la chaîne pentyle, sur le groupe éthyle du naphtalène, ou sur les deux, suivies d'une glucuronoconjugaison. Dans le modèle porcin étudié par chromatographie liquide couplée à la spectrométrie de masse haute résolution, la molécule mère n'est pas retrouvée dans les urines, à la différence du THC: le marqueur urinaire retenu est le glucuronide du métabolite N-hydroxypentyle, dont la fenêtre de détection observée est courte, de l'ordre de quelques heures après administration.
Toxicology and risks
Le tableau décrit dans la littérature d'urgence associe nausées, tachycardie, hypertension, somnolence, agitation et hypokaliémie, avec des cas de diplopie, de convulsions, de syncope et de modifications de l'électrocardiogramme telles qu'une inversion de l'onde T ou une bradycardie. Trois facteurs se combinent en un risque de surdose difficile à anticiper: une activation complète du récepteur CB1, une puissance très supérieure à celle du THC, et une répartition inégale du produit sur le support végétal. Des décès impliquant des cannabinoïdes de synthèse de cette famille figurent dans les revues de toxicologie médico-légale, le plus souvent en présence d'autres substances. Il n'existe aucun antidote, la prise en charge hospitalière est symptomatique.
Detection and analysis
Les immunoessais cannabis usuels ne détectent pas le JWH-210, dont la structure est trop éloignée de celle du THC. L'identification repose sur la chromatographie liquide couplée à la spectrométrie de masse en tandem ou haute résolution, en ciblant la molécule mère dans le sérum et le sang total, et les métabolites hydroxylés conjugués dans les urines. Des concentrations sériques de 0,18 à 90 ng/mL ont été mesurées chez des patients admis aux urgences. En contexte post mortem, l'interprétation quantitative reste difficile, faute de données suffisantes sur la redistribution et sur la stabilité des échantillons.
