5F-AMB is a molecule made in a laboratory, with no connection whatsoever to the hemp plant. It has been sprayed onto plant material and then sold in smoking mixtures presented as cannabis. It acts on the same receptors as THC, but far more strongly, and several deaths have been associated with it.

Pharmacology
5F-AMB, also called 5F-AMB-PINACA or 5F-MMB-PINACA, is a full agonist of the CB1 and CB2 receptors, where THC is only a partial agonist. The values gathered by the WHO expert committee give an affinity of 8.29 nM at human CB1 and 7.93 nM at CB2, with functional EC50 values of 1.3 nM and 0.272 nM, that is to say a potency far above that of THC. In rodents, the molecule fully substitutes for THC in stimulus discrimination, causes hypothermia, bradycardia and locomotor suppression, effects reversed by a CB1 antagonist, and shifts the balance between excitation and inhibition of medial prefrontal cortex neurons towards inhibition. The major metabolite, the carboxylic acid arising from hydrolysis of the ester, remains active at both receptors and may prolong the effect. Human data, by contrast, are lacking: the active dose is not known, no dependence or self-administration study has been conducted, and preclinical toxicological assessment has never been carried out. No therapeutic or industrial use is recognised.

Origin (pharmaceutical research → illicit market)
No biological pathway: 5F-AMB is entirely synthetic and does not occur in hemp. Its structure combines an indazole-3-carboxamide core bearing a 5-fluoropentyl chain on nitrogen 1, linked to a methyl ester of L-valine.
Legal framework
France
A narcotic listed by name in France, and not caught by the generic clause on tetrahydrocannabinols. 5F-AMB appears under its own name in annex III of the decree of 22 February 1990, entered by the decree of 31 March 2017. Possession, supply and use are prohibited.
European Union
Under international control: the United Nations Commission on Narcotic Drugs placed it in Schedule II of the 1971 Convention by its decision 63/6 (63rd session, March 2020), which entered into force on 3 November 2020. The classification therefore applies in all Member States of the European Union.

Cannabinoïde de synthèse indazole-3-carboxamide de la famille PINACA : queue 5-fluoropentyle, groupement méthyl-L-valinate. Un seul centre chiral ; l'énantiomère S est la forme active retrouvée dans les produits saisis.
Pharmacokinetics
La voie d'administration est l'inhalation, à partir de matière végétale imprégnée ou de liquide à vapoter ; la dose produisant un effet chez l'humain n'est pas établie. La stabilité métabolique est faible, avec une demi-vie d'environ une minute sur microsomes hépatiques humains, et le métabolisme est rapide et extensif. La distribution n'a pas été étudiée de façon systématique : dans un cas d'autopsie, la molécule inchangée n'était quantifiable que dans le tissu adipeux.
Metabolism
Dix-sept métabolites ont été identifiés sur hépatocytes humains. La voie majoritaire est l'hydrolyse de l'ester méthylique en acide carboxylique, complétée par une défluoration oxydative, des hydroxylations et une glucuroconjugaison. L'acide carboxylique conserve une affinité pour CB1 et CB2 (Ki de 267 et 197 nM) et une efficacité élevée, ce qui peut allonger la durée d'action. Ce métabolite n'est pas un marqueur spécifique, car il est aussi produit par le 5F-AB-PINACA ; un métabolite glucuroconjugué de phase II est proposé comme biomarqueur.
Toxicology and risks
Aucune évaluation toxicologique préclinique n'a été menée ; une étude signale des convulsions chez 2 rats sur 5 après une injection intrapéritonéale de 2,5 mg/kg. Trois décès avec confirmation analytique ont été rapportés, dont un chez un homme de 34 ans où le 5F-AMB était la seule substance retrouvée, les deux autres associant plusieurs substances. Un cas d'intoxication confirmée décrit yeux rouges, troubles cognitifs, ralentissement moteur, élocution altérée, anxiété et incoordination. Au Japon, le 5F-AMB a été retenu comme facteur causal dans 21 accidents de la route entre 2012 et 2014, avec amnésie antérograde fréquente et rigidité musculaire empêchant le freinage. Comme pour les autres cannabinoïdes de synthèse, la répartition inégale du produit sur la matière végétale crée des zones très concentrées et expose au surdosage.
Detection and analysis
Identification par chromatographie gazeuse ou liquide couplée à la spectrométrie de masse, spectrométrie de masse haute résolution, résonance magnétique nucléaire et spectroscopie Raman. Les immunoessais cannabis usuels ne reconnaissent pas cette molécule, ce qui impose une recherche ciblée. En urine, le marqueur retenu est un métabolite glucuroconjugué ; dans un cas d'autopsie, le tissu adipeux s'est révélé la matrice la plus utile.
