- Le composé parent MDA-19 a été mis au point en 2008 au MD Anderson Cancer Center de l'université du Texas, comme agoniste sélectif du récepteur CB2 destiné à la douleur neuropathique. L'analogue fluoré 5F-MDA-19 a été identifié pour la première fois en 2021 par Liu, Hua, Jia et Li, dans des poudres et des liquides de vapotage saisis en Chine après l'interdiction générique de classe entrée en vigueur en juillet 2021 ; la même année, le Center for Forensic Science Research and Education en signalait la saisie aux États-Unis. La nomenclature OXIZID, construite sur le noyau oxo-indoline et sur son lien azoté, a été proposée à cette occasion par Cayman Chemical et le programme NPS Discovery, afin d'éviter la confusion entre le sigle MDA et l'abréviation de la méthylènedioxyamphétamine.

5F-BZO-POXIZID is a synthetic cannabinoid made in a clandestine laboratory, with no link whatsoever to hemp. It appeared in 2021 in seized powders and vaping liquids, as a variant of a research molecule called MDA-19. Its effects in humans have never been studied and no safety data exist for this substance.

Pharmacology
5F-BZO-POXIZID belongs to the OXIZID class, the isatin acylhydrazones designed to escape the generic bans that targeted the indole and indazole skeletons of earlier synthetic cannabinoids. In a β-arrestin 2 recruitment assay, it behaves as a full agonist at the CB1 receptor, with a median effective concentration of 226 nanomolar and an efficacy of 731 % of that of CP55,940, and as a partial agonist at the CB2 receptor, markedly more potent, at 4.11 nanomolar. Its potency at CB1 is about ten times lower than that of JWH-018, but its intrinsic efficacy there is more than twice as high: the reputation for CB2 selectivity attached to the parent compound MDA-19 does not carry over, and a cannabis-type psychotropic action remains to be expected. A second team has confirmed CB1 activation by the OXIZID series in Gαi protein as in β-arrestin 2, with substitution for THC in drug discrimination in the mouse, while placing these compounds below the preceding generations for potency as for efficacy. No clinical study, no controlled trial and no published case documents this molecule in humans: human data are entirely lacking.

No biosynthetic pathway: the molecule does not exist in living systems and comes solely from organic synthesis, by assembly of an N-alkylated isatin nucleus and a benzoic hydrazide.

5F-BZO-POXIZID is named in no French text classifying narcotics. The generic clause of annex IV of the decree of 22 February 1990 covers seven families of synthetic cannabinoid skeletons, among them the indol-3-yl methanones, the indazol-3-yl methanones, the naphthoylpyrroles and the indole or indazole carboxamides; the oxo-indoline nucleus with a hydrazone linkage of the OXIZID class does not appear there, and the molecule is neither a tetrahydrocannabinol nor an ester, an ether or a salt of THC. As the law stands, it therefore falls among unscheduled substances, which corresponds exactly to the objective pursued by the manufacturers of this series. The situation may change at any moment, as the national medicines safety agency may list a substance by name. The absence of classification amounts neither to an authorisation to sell nor to a presumption of harmlessness: placing such a product intended for human consumption on the market remains unlawful.
European Union
No international United Nations convention covers this molecule, and no delegated act has extended the European definition of a drug within the meaning of Framework Decision 2004/757/JHA to the OXIZID class. These substances are monitored by the Union's early warning system, coordinated by the European Union Drugs Agency, which may trigger a risk assessment and then a control measure. National situations diverge: Hungary added several OXIZID analogues to its list of controlled new psychoactive substances as early as December 2021, whereas other Member States do not cover them. China, the starting point of the series, had introduced in July 2021 a class ban covering seven skeletons, which these compounds circumvent by construction.

Pharmacokinetics
Aucune donnée de pharmacocinétique humaine n'est publiée : ni biodisponibilité, ni demi-vie, ni concentration sanguine documentée après consommation. Les seules informations disponibles proviennent d'incubations sur microsomes et sur hépatocytes humains, qui renseignent sur les voies de transformation mais pas sur le devenir de la molécule dans l'organisme. Les modes de consommation rapportés, vapotage de liquides et inhalation de matière végétale imprégnée, exposent à une absorption rapide et à une quantité reçue très imprévisible, la répartition du produit sur son support étant hétérogène.
Metabolism
Le métabolisme a été cartographié sur microsomes et hépatocytes humains. Les transformations majeures portent sur la chaîne N-alkyle et sur le cycle benzénique : hydroxylations, N-désalkylation, oxydation en cétone, formation de dihydrodiol, glucuronoconjugaison, et surtout défluoration oxydative convertissant la chaîne 5-fluoropentyle en alcool puis en acide carboxylique. Les travaux publiés décrivent plusieurs dizaines de métabolites, jusqu'à une cinquantaine selon le modèle employé. Les isoformes du cytochrome P450 principalement impliquées sont CYP3A4, CYP3A5 et CYP2C9, ce qui rend plausibles des interactions avec les médicaments partageant ces voies, sans qu'aucune étude clinique ne l'ait vérifié.
Toxicology and risks
Aucun cas humain publié ne concerne spécifiquement le 5F-BZO-POXIZID : pas de rapport d'intoxication, pas de concentration sanguine documentée, pas de décès attribué. Les données humaines manquent, et ce vide traduit un défaut d'étude et non une innocuité. Les agonistes de synthèse des récepteurs cannabinoïdes sont associés, en tant que classe, à des agitations, des états psychotiques, des hallucinations, des convulsions, des atteintes cardiovasculaires, des détresses respiratoires, des comas et des décès. Le profil obtenu en culture cellulaire, celui d'un agoniste complet du CB1 dont l'efficacité dépasse largement celle du JWH-018, invite à ne pas lui prêter la sécurité parfois attribuée aux agonistes CB2 sélectifs dont il dérive. Chez la souris, les OXIZID se substituent au THC en discrimination, ce qui confirme une action centrale de type cannabinique. Un analogue proche, le BZO-4en-POXIZID, a été retrouvé dans les cheveux de cinq personnes fréquentant des discothèques new-yorkaises qui ignoraient y avoir été exposées, ce qui illustre le risque d'adultération à l'insu de la personne.
Detection and analysis
La molécule échappe aux tests immunologiques cannabiniques, qui ciblent les métabolites du THC, et n'apparaît dans aucun panel toxicologique de routine. Son identification repose sur la chromatographie liquide couplée à la spectrométrie de masse haute résolution, avec un standard de référence ; les voies de fragmentation des composés de type OXIZID en ionisation par électronébulisation ont été publiées en 2025. Dans l'urine, les marqueurs recommandés sont la molécule mère accompagnée des métabolites monohydroxylés sur la chaîne N-alkyle et sur le cycle benzénique. Sur produit saisi, l'identification associe chromatographie en phase gazeuse, spectrométrie de masse à temps de vol, spectroscopie infrarouge à transformée de Fourier et résonance magnétique nucléaire.
