Le Cumyl-PEGACLONE a été détecté pour la première fois en Allemagne en 2016 et son analogue fluoré en 2017, dans les deux cas alors qu'aucune législation nationale ne les contrôlait. Le composé fluoré porte le code SGT-269 dans la série de la société néo-zélandaise Stargate International. La classe a été caractérisée pharmacologiquement en 2020 par Liesl Janssens, Annelies Cannaert et Christophe Stove, au laboratoire de toxicologie de l'université de Gand, dans une étude qui comparait la fluoration et deux isomères de structure afin de comprendre ce qui, dans cette architecture, produit l'activité.

Pharmacology

5F-CUMYL-PEGACLONE belongs to an architecture that is rare among synthetic cannabinoids. Where ordinary carboxamides link an indole or an indazole to their head group through an open amide bridge, the pegaclones enclose that same amide within a fused ring, forming a gamma-carbolinone core. Cumyl-PEGACLONE was the first compound of this architecture encountered in forensic medicine, detected in Germany in 2016, and its fluorinated analogue followed in 2017; neither was controlled by national legislation at the time, and both have been associated with several deaths. The study conducted at the University of Ghent and published in 2020 measured their activity at the CB1 receptor in two independent assays, one following beta-arrestin 2 recruitment, the other that of a modified G protein. Both compounds produce strong activation, with sub-nanomolar effective concentrations and a relative efficacy exceeding that of JWH-018 by a factor of three in the first assay and by almost double in the second. Two results are counter-intuitive and deserve to be retained. First, no significant biased agonism was observed. Second, fluorination at position 5 is not associated with an intrinsically higher activation potential, which contradicts the received idea that a fluorinated analogue would automatically be more potent. Two structural isomers tested in parallel see their activity collapse instead, which the authors attribute to steric hindrance in the binding pocket. On the cardiac side, a study on isolated perfused rat hearts did not demonstrate any clear statistical difference from controls, but two exposed hearts showed ST-segment depression suggestive of ischaemia, with QT-interval prolongation and QRS-complex distortion also observed at the individual level.

It arises from organic synthesis, through construction of the pyridinone ring on a previously alkylated indole, the lactam nitrogen bearing the cumyl group. Dérivé de gamma-carbolinone, structure rare parmi les cannabinoïdes de synthèse : le squelette est un pyrido[4,3-b]indol-1-one, c'est-à-dire un indole fusionné à un cycle pyridinone. La différence avec les carboxamides ordinaires est topologique plutôt que chimique : la fonction amide existe bien, mais son carbonyle et son azote sont intégrés au cycle au lieu de former un pont ouvert entre l'indole et la tête cumyle. L'azote indolique porte une chaîne 5-fluoropentyle, l'azote de l'amide porte le groupement cumyle. La molécule est l'analogue fluoré du Cumyl-PEGACLONE, premier cannabinoïde de synthèse à cœur gamma-carbolinone rencontré en médecine légale.

Pharmacokinetics

Aucune étude de pharmacocinétique humaine n'est publiée : biodisponibilité, demi-vie plasmatique et volume de distribution restent inconnus. La voie d'exposition documentée est l'inhalation, à partir d'un support végétal ou d'un papier imprégné, avec un délai d'action de quelques minutes. Le cœur gamma-carbolinone ne comporte aucun point de coupure hydrolytique comparable à l'ester des carboxylates quinoléinyles, ce qui rend la molécule mère plus stable et prolonge sa présence dans les prélèvements. Les concentrations retenues pour l'étude cardiaque sur cœurs de rat isolés, de 50 et 100 nanogrammes par millilitre, ont été choisies pour couvrir l'ordre de grandeur des concentrations sanguines rapportées dans les cas humains.

Metabolism

Le métabolisme humain de cette molécule n'a pas fait l'objet d'une étude dédiée sur hépatocytes parmi les sources consultées ici. Les voies attendues pour un composé portant une chaîne 5-fluoropentyle sont la défluoration oxydative suivie de carboxylation, ainsi que des hydroxylations du cœur tricyclique et du groupement cumyle, suivies de glucuroconjugaison. Ce schéma reste une extrapolation de classe. Une difficulté prévisible tient à la convergence avec le composé non fluoré : les métabolites issus de la défluoration portent le squelette du Cumyl-PEGACLONE, dont le statut juridique diffère, ce qui rend l'attribution d'une consommation dépendante d'un métabolite ayant conservé le fluor.

Toxicology and risks

Aucune étude contrôlée chez l'être humain n'est publiée, mais des cas humains sont documentés : le composé et son analogue non fluoré ont été associés à plusieurs décès en Allemagne, à une époque où aucun des deux n'était contrôlé. Une étude expérimentale a cherché à préciser le versant cardiaque en exposant des cœurs de rat isolés perfusés à 50 et 100 nanogrammes par millilitre. L'analyse statistique n'a pas montré de différence nette entre les groupes traités et les témoins, mais deux cœurs exposés ont présenté des sous-décalages du segment ST évoquant une ischémie myocardique, et des allongements de l'intervalle QT ainsi que des déformations du complexe QRS ont été relevés à l'échelle individuelle. Les auteurs concluent que le risque d'arythmie ou d'infarctus est vraisemblablement plus élevé en cas de consommation simultanée de plusieurs substances ou de maladie cardiovasculaire préexistante, et citent des cas où la consommation concomitante d'alcool augmentait ce risque. Le profil général reste celui de la classe : agoniste complet de CB1 sans plafond d'effet, associé à des tachycardies, des convulsions, des vomissements, une agitation sévère et des décompensations psychiatriques aiguës.

Detection and analysis

L'identification repose sur la chromatographie couplée à la spectrométrie de masse, en phase gazeuse ou liquide, par comparaison à un étalon de référence certifié. Les immunoessais destinés au cannabis ne réagissent pas à cette structure, et un dépistage négatif ne vaut pas absence. La détection en spectrométrie de mobilité ionique, employée pour cribler les papiers imprégnés en milieu carcéral, a été évaluée à grande échelle sur des étalons de référence incluant cette molécule : les auteurs y relèvent que les valeurs de mobilité réduite obtenues diffèrent parfois de celles rapportées antérieurement, notamment pour les composés cumyle et quinoléinyle, sans que cela affecte sensiblement l'usage opérationnel, à condition de tenir les bibliothèques des instruments à jour. Une attribution certaine impose de distinguer la molécule de son analogue non fluoré, dont le statut juridique diffère.
