Le squelette hexahydrocannabinol hydroxylé en position 8 est décrit dans la littérature du métabolisme des cannabinoïdes depuis 1990. Harvey et Brown en publient en 1991 une caractérisation comparative sur préparations microsomales de plusieurs espèces de mammifères. La synthèse stéréosélective des quatre stéréoisomères et leur confirmation structurale par RMN n'ont été menées à bien qu'en 2026 par Ieuji, Nakamura et Takahashi, fournissant enfin des étalons de référence pour les laboratoires.

Pharmacology

8-hydroxyhexahydrocannabinol is an oxidised metabolite of HHC, hydroxylated at position 8 of the saturated ring. Harvey and Brown showed as early as 1991 that this pathway exists in several mammals, with high concentrations of 8-alpha-OH-HHC in the mouse and of 8-beta-OH-HHC in the hamster, whereas hydroxylation at C11 predominates in most species. In humans, recent work on hepatocytes and authentic urine samples places 11-OH-HHC and 5'-OH-HHC among the most abundant metabolites, the C8 pathway remaining more discreet. The literature describes this molecule as an active metabolite, but no quantified affinity for CB1 or CB2 has been published concerning it: the values cited belong to the parent HHC. Four stereoisomers coexist and their complete stereoselective synthesis dates only from 2026, so that the isomer-by-isomer pharmacology remains very incomplete.

Route of preparation (chemical process)

No known plant pathway. The molecule is formed by hydroxylation at position 8 of HHC under the action of hepatic cytochromes P450 (CYP3A4, CYP2C9, CYP2C19), in competition with hydroxylation at C11, which remains the main pathway in humans. 8-hydroxy-HHC is then oxidised into 8-oxo derivatives, conjugated and eliminated. By chemical means, the four stereoisomers have been prepared from Delta-8-THC by hydroboration followed by oxidation, or by epoxidation followed by reduction.

Pharmacokinetics

Aucune étude de pharmacocinétique humaine ne porte sur le 8-hydroxy-HHC administré comme tel. Les éléments disponibles proviennent des études sur le HHC: formation hépatique du métabolite après prise orale ou inhalée, puis conjugaison. Dans les urines non hydrolysées, les métabolites du HHC circulent très majoritairement sous forme glucuronoconjuguée, ce qui impose une hydrolyse avant analyse. Les paramètres usuels, demi-vie, biodisponibilité et clairance, ne sont pas établis pour cette molécule.

Metabolism

Le 8-hydroxy-HHC est un produit du métabolisme du HHC et non un précurseur. Harvey et Brown ont observé de fortes concentrations de 8-alpha-OH-HHC chez la souris et de 8-bêta-OH-HHC chez le hamster, alors que l'hydroxylation en C11 prédomine chez la plupart des espèces étudiées. Le squelette hexahydro ayant perdu la double liaison qui rendait les positions C11 et C8 allyliques, les auteurs concluent que c'est l'orientation de la molécule sur le site actif du cytochrome P450, plutôt que la réactivité de la liaison carbone hydrogène, qui détermine la position hydroxylée. Chez l'humain, les travaux sur hépatocytes et urines authentiques placent le 11-OH-HHC et le 5'-OH-HHC parmi les métabolites majoritaires du HHC. En aval, le 8-hydroxy-HHC est oxydé en dérivés 8-oxo, conjugués puis excrétés.

Toxicology and risks

Aucune étude de toxicité ne porte spécifiquement sur le 8-hydroxy-HHC et les données humaines manquent. Le risque documenté est celui de la famille du HHC, à laquelle cette molécule appartient et dont elle partage le circuit de vente: l'ANSM et les centres d'addictovigilance rapportent vomissements, perte de connaissance, coma, convulsions, paranoïa, anxiété, attaques de panique, hypertension artérielle, tachycardie, douleurs thoraciques et insuffisance rénale, avec des effets souvent plus intenses que ceux du cannabis. Ces signalements concernent le HHC et ses dérivés commercialisés, et non des expositions au 8-hydroxy-HHC isolé. L'absence de données propres ne vaut pas absence de risque.

Detection and analysis

Les premières identifications reposaient sur la chromatographie en phase gazeuse couplée à la spectrométrie de masse. Les laboratoires de toxicologie recourent aujourd'hui à la chromatographie liquide couplée à la spectrométrie de masse en tandem ou à haute résolution, après hydrolyse des glucuronides. La difficulté propre à cette molécule est stéréochimique: quatre stéréoisomères coexistent, et trois avaient été observés dans des urines sans que tous puissent être confirmés faute d'étalons, situation levée en 2026 par la mise à disposition des quatre standards de référence.
