In plain terms

Pharmacology

Elcubragistat is a hexafluoroisopropyl carbamate that covalently and irreversibly inhibits monoacylglycerol lipase (MAGL), the enzyme responsible for degrading 2-arachidonoylglycerol (2-AG). It does not act directly on the cannabinoid receptors: no affinity of its own for CB1 or CB2 is reported, and its effects pass entirely through the accumulation of 2-AG, the endogenous agonist of CB1. Activity-based protein profiling shows a marked selectivity within the serine hydrolases, with an IC50 of 14 nM at human MGLL against 2.7 µM at ABHD6 and more than 10 µM at FAAH, so that anandamide remains intact. In rodents, cerebral inhibition of the enzyme occurs at doses of the order of a milligram per kilogram and is accompanied by a rise in 2-AG as well as a reduction of pain behaviours. In humans, a phase 1b study had suggested a reduction of tics in Gilles de la Tourette syndrome, but the twelve-week controlled trial conducted subsequently showed no benefit on tics compared with placebo. The other programmes, in multiple sclerosis spasticity, fibromyalgia, focal epilepsy and post-traumatic stress, were discontinued in phase 1 for recruitment or corporate strategy reasons. The molecule therefore remains a pharmacological tool for the endocannabinoid system, with no established clinical efficacy.

Pharmacokinetics

Administration par voie orale, en gélules puis en comprimés au cours du développement. Un essai de bilan de masse au carbone 14 chez des volontaires masculins a suivi la radioactivité dans les urines et les selles jusqu'à trois semaines et demie après une prise unique, et a caractérisé la cinétique plasmatique du produit parent comme celle de son métabolite Lu AG06988. Une étude dédiée avec l'itraconazole a évalué l'effet d'une inhibition du CYP3A4 sur l'exposition, ce qui signale une contribution de cette voie à l'élimination. L'inhibition de la MAGL étant covalente, l'effet biologique se prolonge au-delà de la présence du produit dans le sang, le temps que l'enzyme se renouvelle. Chez le rat, la hausse du 2-AG extracellulaire dans l'amygdale culmine environ deux à quatre heures après la prise. Les paramètres humains détaillés restent peu publiés.

Metabolism

Le devenir métabolique humain a été exploré par un essai de bilan de masse avec traceur radioactif, qui a mesuré séparément le produit parent et le métabolite Lu AG06988 dans le plasma, ainsi que la répartition de l'excrétion entre urines et selles. Une étude d'interaction avec l'itraconazole, inhibiteur puissant du CYP3A4, a servi à quantifier la part de cette isoenzyme dans l'élimination. Le potentiel d'interaction de la molécule sur les cytochromes a été testé chez le volontaire sain à l'aide du midazolam, du métoprolol et du bupropion, sondes respectives du CYP3A4, du CYP2D6 et du CYP2B6.

Toxicology and risks

Le profil de tolérance rapporté dans les essais reste limité mais sans alerte majeure : dans l'étude de phase 1b sur le syndrome de Gilles de la Tourette, les effets indésirables décrits étaient légers, avec céphalées, somnolence et fatigue ; l'essai contrôlé de douze semaines a été jugé globalement sûr, sans événement indésirable grave. Les arrêts de programme dans l'épilepsie focale, la fibromyalgie, la spasticité et chez le volontaire sain ont été motivés par des difficultés de recrutement ou par des décisions stratégiques, et non par un signal de sécurité déclaré. Deux observations précliniques appellent malgré tout à la prudence : chez la souris modèle du syndrome de Dravet, un traitement subchronique a augmenté la fréquence des crises spontanées tout en réduisant leur sévérité ; dans un modèle murin d'atteinte hépatique alcoolique, la survie diminuait à mesure que la dose augmentait. Les données humaines de sécurité à long terme manquent, et la molécule n'a jamais été évaluée pour un usage hors essai clinique.

Detection and analysis

Aucune méthode de dépistage de routine ne cible cette molécule : elle n'est pas une substance d'usage récréatif et ne figure pas dans les panels toxicologiques courants, urinaires ou salivaires. Dans les essais cliniques, les concentrations plasmatiques du produit et de son métabolite sont mesurées par des méthodes bioanalytiques dédiées, et l'étude du devenir humain a reposé sur un marquage au carbone 14. Aucune publication de médecine légale ne lui est consacrée.
