Apparue sur le marché illicite en 2013. La substance a été formellement notifiée au Système d'alerte précoce de l'Union européenne le 3 décembre 2013. La même année, aux États-Unis, elle a été identifiée dans un produit végétal vendu sous le nom Crazy Clown, à l'origine d'un épisode groupé d'intoxications décrit par Schwartz et ses collègues en 2015. Pharmacology

ADB-PINACA is a synthetic cannabinoid of the indazole-3-carboxamide family, bearing a pentyl chain and an amide moiety derived from tert-leucine. It behaves as a full agonist at the CB1 and CB2 receptors, with EC50 values reported at 0.52 nM and 0.88 nM by Banister and colleagues in 2015, a potency far greater than that of delta-9-THC, which is itself merely a partial agonist. It is this combination of high affinity and maximal efficacy that accounts for the clinical picture observed: agitated delirium, psychosis, seizures, tachycardia, hypertension and coma, described in several case series and during a cluster outbreak in the United States in 2013. Hepatic metabolism produces some twenty metabolites, several of which retain activity at the cannabinoid receptors and probably prolong the effect. Controlled human data, by contrast, are entirely lacking: no pharmacokinetic study, no established dose-response relationship, no known safety margin. No therapeutic benefit has ever been demonstrated for this molecule, and the available literature is limited to intoxication cases, analytical work and in vitro measurements.

Origin (pharmaceutical research → illicit market)

None. The molecule has no biosynthetic pathway: it comes neither from cannabis nor from any other organism, and the human body does not make it. It is obtained by wholly synthetic means, by coupling an N-alkylated indazole-3-carboxylic acid with an amide derived from tert-leucine. No operational detail is provided here.

Le noyau indazole porte une chaîne pentyle sur son azote 1 et un pont carboxamide en position 3, relié à un motif amide dérivé de la tert-leucine, un acide aminé non protéinogène. Cette architecture ne partage aucun élément structural avec les cannabinoïdes du chanvre, qui reposent sur un squelette terpénophénolique.

Pharmacokinetics

Les données humaines quantitatives manquent : aucune étude de pharmacocinétique contrôlée n'a été conduite chez l'homme, l'imprévisibilité des effets rendant ce type d'essai éthiquement inacceptable. La voie d'exposition documentée est l'inhalation de matière végétale imprégnée, avec une installation rapide des effets rapportée dans les séries de cas. Les concentrations sanguines relevées lors des intoxications se situent dans la gamme du nanogramme par millilitre, voire en dessous, ce qui reflète la très forte puissance de la molécule. Fortement lipophile, elle subit un métabolisme hépatique intense, si bien que le composé parent est fréquemment indétectable dans les urines.

Metabolism

Carlier et ses collègues ont identifié en 2017 dix-neuf métabolites majeurs d'ADB-PINACA dans des hépatocytes humains cryoconservés. Les voies principales sont l'hydroxylation de la chaîne pentyle, l'hydroxylation suivie d'une oxydation en cétone, et la glucuronoconjugaison. Les auteurs recommandent de cibler les métabolites hydroxypentyle et cétopentyle, ainsi que les formes 5-hydroxypentyle et acide pentanoïque, comme marqueurs urinaires de consommation. Longworth et ses collègues ont montré la même année que les métabolites dépourvus de fonction acide carboxylique gardent une activité agoniste notable sur CB1 et CB2, et participent donc au profil pharmacologique global ; l'apparition d'un acide carboxylique libre réduit en revanche nettement cette activité.

Toxicology and risks

Le profil de risque est celui d'un agoniste complet de CB1, très éloigné de celui du cannabis. Schwartz et ses collègues ont décrit en 2015 un épisode groupé de huit patients exposés à un même produit contenant ADB-PINACA, avec anxiété, délire, psychose et comportements agressifs, accompagnés de tachycardie, vomissements, hypokaliémie, hyperglycémie et acidose ; une crise convulsive retardée a été observée chez un patient. Armenian et ses collègues ont rapporté en 2018 une série associant délire agité, léthargie et coma, convulsions, tachycardie, hypertension et hallucinations. La substance a été associée à des hospitalisations et à des décès. La prise en charge décrite reste symptomatique et repose sur la protection des voies aériennes et le contrôle de l'agitation et des crises. Il n'existe aucun antidote. La répartition irrégulière du produit sur le support végétal est un facteur de risque documenté pour toute cette classe : elle provoque des surdosages même chez des consommateurs expérimentés, qui avaient fumé le même sachet la veille sans incident.

Detection and analysis

L'identification repose sur la chromatographie liquide couplée à la spectrométrie de masse en tandem ou à haute résolution, après extraction liquide-liquide ou sur phase solide. Les immunoessais ne sont pas recommandés pour le criblage biologique : la diversité structurale de cette classe et les concentrations attendues, de l'ordre du nanogramme par millilitre ou moins, dépassent leurs capacités. Dans les urines, le composé parent est souvent absent, si bien que les laboratoires ciblent les métabolites de phase I, généralement après hydrolyse enzymatique des glucuronides. Distinguer ADB-PINACA de son analogue fluoré 5F-ADB-PINACA impose de suivre des ions produits spécifiques et d'optimiser la séparation chromatographique, car les deux composés génèrent des métabolites isomères de position.
