- La piste vient de l'équipe de Pier Vincenzo Piazza, à Bordeaux (université de Bordeaux, Inserm), qui a montré en 2014 dans Science que la prégnénolone, dont la synthèse cérébrale augmente après activation du récepteur CB1 par le THC, agit en retour comme un inhibiteur signal-spécifique de ce même récepteur. AEF0117 est un dérivé de la prégnénolone optimisé par la société bordelaise Aelis Farma pour conserver cette action sans les effets hormonaux du stéroïde d'origine, protégé par un brevet américain délivré en 2022 et décrit en détail pour la première fois dans une publication de 2023 signée Margaret Haney, Monique Vallée et leurs collègues.

Pharmacology
AEF0117 is not a cannabinoid but a steroid, derived from pregnenolone, designed as a signalling-specific inhibitor of the CB1 receptor (CB1-SSi). It binds at an allosteric site distinct from the orthosteric site and does not displace the binding of the reference agonist [3H]CP55,940. The idea is to remove part of CB1 signalling, the phosphorylation of MAP kinases and the fall in cellular respiration induced by THC, with nanomolar-range potency in a cellular model, while preserving the cyclic AMP pathway. This selectivity explains the profile observed: in animals as in humans, the molecule has no detectable effect of its own on behaviour, does not suppress appetite, does not increase anxiety and does not precipitate a withdrawal syndrome, which distinguishes it from rimonabant, a full CB1 antagonist withdrawn from the market for its psychiatric effects. The phase 1 and then phase 2a trials published in 2023 report a reduction of about 19 to 38% in the positive subjective effects of cannabis and a fall in self-administration in the laboratory, with tolerability judged good. A phase 2b trial in 333 treatment-seeking patients was completed in 2024: its results are neither posted on the registry nor published in a peer-reviewed journal, so that therapeutic efficacy remains unestablished. No marketing authorisation exists and long-term human data are lacking.

Origin (research tool)
No known biosynthetic pathway, plant or human. The compound is a matter of semi-synthesis from pregnenolone: the hydroxyl function at position 3β is converted into a benzyl ether by a 4-methoxybenzyl group. The molecule is not transformed in the body into steroids downstream of pregnenolone.
Legal framework
France
AEF0117 is named in none of the French schedules of narcotics and psychotropic substances. It is not a tetrahydrocannabinol, nor an ester, an ether or a salt of THC: the generic clause of annex IV of the decree of 22 February 1990, which classifies tetrahydrocannabinols, their esters, ethers and salts as well as the salts of those derivatives, therefore does not cover it. Its actual status is that of an investigational medicinal product: no marketing authorisation, no possible sale, availability limited to authorised clinical trials. Its presence in a consumer product, a food supplement or a CBD flower, would fall under medicines regulation and not under narcotics law.
European Union
The situation is the same in the European Union: the molecule appears on no drug control list, it has not been the subject of an EUDA (formerly EMCDDA) risk assessment and does not appear among the substances placed under control by the United Nations conventions. It falls under medicines law and the European clinical trials regulation. Its development is led by a French sponsor, Aelis Farma, with trials conducted in Europe and the United States.

Stéroïde de la série prégnane, plus précisément un dérivé 3-O-benzylé de la prégnénolone : la fonction hydroxyle en position 3β porte un groupe 4-méthoxybenzyle, la cétone en position 20 et la double liaison 5,6 étant conservées. Aucun cycle terpénophénolique n'est présent, donc aucune parenté structurale avec les cannabinoïdes du chanvre.
Pharmacokinetics
Administré par voie orale, le composé est très lipophile et formulé dans un corps gras. Le pic plasmatique survient environ trois heures après la prise et l'exposition croît proportionnellement à la quantité administrée dans les intervalles étudiés. Le trait le plus marquant est une demi-vie terminale très longue chez l'humain, de 152 à 258 heures dans l'étude à doses répétées, associée à une distribution dans le tissu adipeux et à une élimination lente : l'état d'équilibre demande plusieurs semaines et les concentrations restent mesurables durant des mois après l'arrêt. Chez l'animal, le passage vers le cerveau est bon, avec un rapport d'exposition cerveau sur plasma supérieur à 4.
Metabolism
Le composé n'est pas notablement métabolisé par les grandes voies du métabolisme des médicaments, cytochromes P450 et enzymes de phase II, et ne modifie pas leur activité, ce qui limite a priori le risque d'interaction médicamenteuse. Aucun métabolite représentant plus de 1 % du composé parent n'a été identifié. Point important pour un dérivé stéroïdien, la molécule n'est convertie ni chez l'animal ni chez l'humain en stéroïdes situés en aval de la prégnénolone, ce qui la sépare de l'activité hormonale de cette dernière. Une étude de bilan de masse avec composé radiomarqué au carbone 14 a été menée chez le volontaire sain, ses résultats ne sont pas publiés.
Toxicology and risks
Les essais de phase 1, dose unique croissante puis doses répétées chez le volontaire sain, et l'essai de phase 2a chez des consommateurs de cannabis n'ont rapporté aucun événement indésirable grave lié au traitement, ni anomalie cliniquement notable des constantes, de l'électrocardiogramme ou du bilan biologique. Les événements observés ont été peu nombreux et sans relation avec la quantité administrée : un cas de prurit avec éruption cutanée d'intensité modérée ayant conduit à l'arrêt, une élévation asymptomatique des CPK et de la myoglobine non reproduite, et chez un participant des hallucinations auditives légères en début de traitement. Aucun syndrome de sevrage n'a été précipité chez des consommateurs réguliers, et ni l'appétit ni le poids n'ont été modifiés, ce qui contraste avec le rimonabant. La demi-vie très longue impose la prudence : elle entraîne une accumulation sur plusieurs semaines et le recul sur une exposition prolongée reste limité.
Detection and analysis
Aucune méthode médico-légale dédiée n'est décrite dans la littérature publique. Le composé n'est pas recherché par les dépistages toxicologiques usuels, urinaires ou salivaires, qui ciblent les métabolites du THC, et il n'apparaît pas dans les systèmes d'alerte sur les nouvelles substances psychoactives. Son dosage plasmatique n'a été réalisé que dans le cadre des essais cliniques, avec des méthodes bioanalytiques propres au promoteur.
