In plain terms
AM-1220 is not a hemp cannabinoid: it is a laboratory molecule, created in the early 1990s by pharmaceutical chemists, which binds to the same receptors as THC but far more strongly. Since 2010 it has been found sprayed onto dried plant material sold as "legal herb", often under another label. It is not a consumer product: no clinical study has ever evaluated its effects or its safety in human beings.

Pharmacology
AM-1220 belongs to the aminoalkylindoles, in the naphthoylindole subfamily: an indole core bearing a naphthalene-1-carbonyl group at position 3 and a (1-methylpiperidin-2-yl)methyl group on the nitrogen. Marusich et al. (Neuropharmacology 2018) measure an affinity of Ki = 2.40 nM at the human CB1 receptor, that is, roughly seventeen times that of Δ9-THC (40.7 nM) in the same work, with an EC50 of 44.4 nM in [35S]GTPγS stimulation; in mice, AM-1220 fully substitutes for THC in drug discrimination, with a potency roughly nineteen times greater than that of THC. The decisive point is that the agonists of this class act as full agonists at CB1, where THC is only a partial agonist: the effect loses its ceiling, which explains the severity of the intoxications described for the family. The CB2 profile is atypical: subnanomolar affinity (Ki = 0.26 nM) with no measurable [35S]GTPγS stimulation via CB2, hence a functional selectivity for CB1. Andersen et al. (Eur J Pharmacol 2021) add a target outside the cannabinoid system: at a maximal concentration for CB1, AM-1220 is one of only three synthetic agonists tested that significantly activate the TRPV1 channel. Beyond these in vitro measurements and the animal model, human data are lacking: no controlled study, no pharmacokinetics in volunteers, and the only published forensic case associates AM-1220 with two other synthetic agonists, which rules out attributing any causality to it in its own right.

No biosynthetic pathway: AM-1220 exists neither in hemp nor in any living organism. It is obtained by wholly synthetic means, within the aminoalkylindole chemical class. Described here by chemical class only.
Legal framework
France
A narcotic in France, but by generic clause and not by name. AM-1220 appears in no text published in the Journal officiel (Légifrance search: no result for "AM-1220"). It is captured by annex IV of the decree of 22 February 1990, in the wording arising from the decree of 31 March 2017, which classifies as a narcotic any molecule of the "indol-3-yl methanone" family bearing on the indole ring nitrogen a substituent of the "1-(N-methylpiperidin-2-yl) methyl" type and, on the carbon of the methanone bridge, a naphthyl-type group, the ring being otherwise substituted or not. AM-1220 matches this definition point by point, and analogues bearing the same nitrogen substituent are indeed cited there by name (AM-2233, AB-005, AM-1248). An equivalent family clause, covering the naphthoylindoles, already featured in the earlier wording of the same annex. Possession, supply and use therefore fall under articles 222-34 to 222-43 of the criminal code.
European Union
No control measure at European Union level: no Union legal act mentions this substance (EUR-Lex search), and it is not among the new psychoactive substances added to the definition of "drug" in Framework Decision 2004/757/JHA. It is likewise absent from the green list of the International Narcotics Control Board, hence outside the schedules of the 1971 Convention, whereas AM-2201 and JWH-018 (AM-678) are listed there. Control therefore rests entirely on national laws, often by means of generic chemical-family clauses as in France; several States outside the European Union also control it under national law.

Aminoalkylindole, sous-famille des naphtoylindoles : noyau indole substitué en position 3 par un groupe naphtalène-1-carbonyle et sur l'azote par un groupe (1-méthylpipéridin-2-yl)méthyle. Le carbone 2 du cycle pipéridine est un centre stéréogène ; la molécule circule sous forme racémique et la littérature d'origine décrit une forte énantiosélectivité en faveur de l'énantiomère (R).
Pharmacokinetics
Aucune pharmacocinétique humaine publiée. In vitro, la demi-vie de l'AM-1220 sur microsomes hépatiques humains est estimée à 3,7 minutes, ce qui la classe parmi les composés à clairance hépatique élevée (Watanabe et al., Forensic Toxicol 2018) : la molécule mère disparaît vite de la circulation, ce qui complique sa mise en évidence directe. Dans le seul cas d'autopsie publié, les concentrations plasmatiques diffèrent entre sang cardiaque et sang fémoral, ce qui suggère une redistribution post mortem (Zaitsu et al., Int J Legal Med 2015, corrigé en 2016).
Metabolism
Onze métabolites décrits in vitro : neuf sur microsomes hépatiques humains, sept sur le champignon Cunninghamella elegans, cinq communs aux deux systèmes. Les transformations observées sont la déméthylation, la formation de dihydrodiol, la combinaison des deux, l'hydroxylation et la dihydroxylation. Les trois métabolites les plus abondants dans les deux modèles sont le dérivé desméthylé, le dihydrodiol et un dérivé hydroxylé, proposés comme marqueurs urinaires de consommation (Watanabe et al., Forensic Toxicol 2018). En contexte médico-légal, la N-désalkylation apparaît comme une voie commune mais mineure des naphtoylindoles chez l'humain (Zaitsu et al., 2015).
Toxicology and risks
Aucune étude clinique, aucun essai contrôlé, aucune donnée de sécurité chez l'humain : les données humaines manquent. L'AM-1220 a été retrouvé dans le plasma et l'urine post mortem d'un cas d'autopsie, mais en présence de MAM-2201 et d'AM-2232, ce qui interdit de lui imputer le décès (Zaitsu et al., Int J Legal Med 2015). Le risque documenté est celui de la classe des agonistes cannabinoïdes de synthèse, agonistes complets de CB1 alors que le THC n'en est qu'un agoniste partiel : agitation, confusion et délire, convulsions, tachycardie et troubles du rythme, vomissements incoercibles, atteinte rénale aiguë et décès sont rapportés pour cette famille. Deux facteurs aggravants sont propres à l'AM-1220 : sa puissance in vivo, environ dix-neuf fois celle du THC en discrimination de stimulus chez la souris, et l'activation du canal TRPV1, qui ajoute une cible hors système cannabinoïde. Un troisième tient au mode de distribution : la pulvérisation sur matière végétale répartit la molécule de façon très inégale, si bien que deux prélèvements d'un même sachet n'en contiennent pas la même quantité.
Detection and analysis
Première identification formelle en 2012, dans un produit vendu comme réactif de laboratoire et dans plusieurs mélanges végétaux, par chromatographie couplée à la spectrométrie de masse et résonance magnétique nucléaire (Kneisel et al., Forensic Toxicol 2012) ; la même année, l'AM-1220 figure parmi cinq agonistes de synthèse retrouvés dans des produits illégaux au Japon (Uchiyama et al., Forensic Toxicol 2012). Piège analytique majeur : un isomère à cycle azépane de sept sommets, le (N-méthylazépan-3-yl)-3-(1-naphtoyl)indole, a circulé sous la même étiquette et ne se distingue pas de l'AM-1220 par la seule masse (Kneisel et al. 2012 ; Nakajima et al., Forensic Toxicol 2013). La molécule a ensuite été quantifiée dans des mélanges de type « spice » en Allemagne à l'automne 2012 (Langer et al., Drug Test Anal 2014), recherchée dans les cheveux (Salomone et al., Drug Test Anal 2014) et dosée dans le plasma et l'urine post mortem par LC/Q-TOFMS (Zaitsu et al., 2015). Les immunoessais cannabis de routine ne la détectent pas : le dépistage repose sur la chromatographie couplée à la spectrométrie de masse, en ciblant les métabolites desméthylé, dihydrodiol et hydroxylé.
