In plain terms
AM404 is a laboratory molecule, absent from cannabis as from any other plant. It has the particularity of also being formed in the brain after paracetamol is taken, from a breakdown product of that medicine. Researchers use it to slow the elimination of anandamide, a cannabinoid made by the body, but it is neither a supplement nor a product intended to be consumed.

Pharmacology
AM404 was designed as a selective inhibitor of the membrane transport of anandamide: by slowing the reuptake of this endocannabinoid by neurons and astrocytes, it prolongs its synaptic lifetime and indirectly amplifies CB1 signalling, without itself behaving as an outright cannabinoid agonist. Its pharmacology subsequently proved broader, since the molecule potently activates the vanilloid receptor TRPV1, a target that explains a good part of its antinociceptive and anticonvulsant effects in animals, and weakly inhibits purified cyclo-oxygenases. Its notoriety rests above all on its second identity: in 2005, the conjugation of p-aminophenol with arachidonic acid by FAAH was demonstrated in the brain and the spinal cord, making AM404 an active metabolite of paracetamol and a candidate explanation for the central action of this analgesic. Human data are lacking: AM404 has been detected in the cerebrospinal fluid of patients who had received paracetamol, but the molecule has never been administered as such to human beings, and there is no clinical trial, no posology and no established safety profile. A signal of abuse is worth noting, the compound having proved to be self-administered intravenously by the squirrel monkey, with reinstatement of drug seeking, both effects being attenuated by a CB1 antagonist.

Origin (research tool)
No plant pathway: AM404 does not come from cannabis. Its only documented natural formation is metabolic and takes place in mammals. After paracetamol is ingested, hepatic deacetylation releases p-aminophenol, which reaches the central nervous system and is there conjugated with arachidonic acid by fatty acid amide hydrolase (FAAH). This conjugation disappears in mice lacking FAAH, which designates that enzyme as responsible for the reaction. Formation occurs mainly at the supraspinal level, in the brain and the spinal cord, and has also been observed in peripheral sensory neurons.
Legal framework
France
AM404 is named in no annex of the decree of 22 February 1990 setting the list of substances classified as narcotics. Nor is it reached by the generic clause of annex IV, which covers « tetrahydrocannabinols, their esters, ethers, salts as well as the salts of the aforementioned derivatives »: AM404 is neither a tetrahydrocannabinol nor a derivative thereof, but a fatty acid amide without a terpenophenolic ring. Neither does it fall under the generic classifications covering synthetic cannabinoid agonists, whose structure and mode of action it does not share. The applicable status is therefore that of an unclassified substance, in practice reserved for research. AM404 holds no marketing authorisation in France and has no place in a consumer product; the paracetamol from which it derives remains, for its part, a regulated medicine.
European Union
No European Union instrument classifies AM404. The molecule does not appear in the schedules of the 1961 and 1971 United Nations conventions, and no Council implementing decision taken under Regulation (EU) 2017/2101 on new psychoactive substances covers it. It benefits from no marketing authorisation as a medicine, nor from any recognised status as a food or cosmetic ingredient. In practice, AM404 circulates solely as a laboratory reagent and the Member States treat it as an unclassified chemical substance.

N-acylphénolamine : amide formé entre l'acide arachidonique et le 4-aminophénol. La molécule reprend la chaîne grasse polyinsaturée en C20 de l'anandamide, mais le groupement éthanolamine y est remplacé par un noyau phénolique para-hydroxylé, ce qui la range parmi les analogues synthétiques des N-acyléthanolamines plutôt que parmi les cannabinoïdes terpénophénoliques du cannabis.
Pharmacokinetics
Les paramètres pharmacocinétiques d'AM404 chez l'être humain ne sont pas établis, la molécule n'ayant jamais été administrée directement. Ce qui est documenté concerne sa formation endogène. Chez le rat recevant du paracétamol à des doses équivalentes aux doses thérapeutiques humaines, AM404 apparaît dans le cerveau à des concentrations de l'ordre de quelques centaines de picogrammes par gramme de tissu, soit environ cent mille fois moins que le paracétamol mesuré dans le même compartiment. Chez l'être humain, AM404 a été retrouvé dans le liquide céphalo-rachidien après une dose unique d'un gramme de paracétamol, chez dix-sept des vingt-six patients étudiés. La molécule est très lipophile, mais c'est son précurseur p-aminophénol qui franchit la barrière hémato-encéphalique, la conjugaison ayant lieu ensuite dans le tissu nerveux. Aucune demi-vie, aucune clairance et aucune biodisponibilité ne sont publiées.
Metabolism
AM404 est lui-même un métabolite, formé par la FAAH à partir du p-aminophénol issu de la désacétylation hépatique du paracétamol. Son propre devenir métabolique reste peu décrit. La FAAH, qui catalyse sa formation, hydrolyse aussi les amides d'acides gras : la coupure de la liaison amide, qui restituerait l'acide arachidonique et le 4-aminophénol, constitue la voie de dégradation attendue, sans qu'elle ait été caractérisée en détail. Aucun métabolite d'AM404 n'a été identifié chez l'être humain et aucune étude d'interaction avec les cytochromes P450 n'est disponible.
Toxicology and risks
Le profil de sécurité d'AM404 chez l'être humain est inconnu : la molécule n'a jamais fait l'objet d'un essai clinique et il n'existe ni dose sans effet, ni dose toxique documentée. Les signaux disponibles viennent de l'animal. Le plus notable concerne le potentiel d'abus : chez le singe écureuil, AM404 administré par voie intraveineuse a maintenu un comportement d'auto-administration et a réamorcé la recherche de drogue précédemment associée aux cannabinoïdes ou à la cocaïne, ces deux effets étant réduits par un antagoniste des récepteurs CB1 ; les auteurs en concluent que les composés augmentant le tonus endocannabinoïde par blocage du transport membranaire peuvent présenter un potentiel d'abus. L'activation marquée de TRPV1 laisse par ailleurs attendre des effets sensoriels et thermorégulateurs communs aux agonistes vanilloïdes. Des travaux de toxicologie de la reproduction explorent une implication du système endocannabinoïde, via AM404, dans les effets du paracétamol sur le développement gonadique chez le rongeur, sans conclusion transposable à l'être humain. En pratique, AM404 est un réactif de laboratoire et ne doit pas être ingéré.
Detection and analysis
AM404 se dose par chromatographie liquide couplée à la spectrométrie de masse en tandem, technique employée pour le retrouver dans le plasma, le liquide céphalo-rachidien et le tissu cérébral après administration de paracétamol. Les concentrations attendues se situant au niveau du picogramme par gramme de tissu, la méthode exige un étalon interne marqué et une préparation d'échantillon soignée. AM404 ne figure dans aucun panel de dépistage toxicologique de routine, urinaire ou salivaire, et n'est recherché ni par les laboratoires antidopage ni par la police scientifique, faute de statut de substance contrôlée. Sa présence dans un prélèvement humain traduit plus vraisemblablement une prise de paracétamol qu'une exposition à la molécule elle-même.
