Le premier essai chez l'humain, une étude de doses orales multiples croissantes, a débuté en 2009, suivi d'un programme de phase 1 poursuivi jusqu'en 2012 (études de pénétration dans le liquide céphalorachidien, bilan de masse, interactions médicamenteuses) et de deux essais de phase 2 conduits entre 2011 et 2014 par Astellas Pharma Europe. Les résultats n'ont été publiés qu'entre 2017 et 2021, après l'arrêt du développement.

In plain terms
ASP-3652 is a laboratory molecule developed by the pharmaceutical group Astellas. It does not exist in hemp: rather than imitating cannabis, it slows down the enzyme that destroys the cannabinoids the body makes itself. Tested against bladder and pelvic pain, it did no better than a placebo and was never put on sale.

Pharmacology
ASP-3652 is a reversible inhibitor of FAAH, the enzyme that hydrolyses anandamide and the neighbouring N acylethanolamines. It does not itself bind to CB1 or CB2: by slowing degradation, it raises the circulating concentrations of anandamide, oleoylethanolamide and palmitoylethanolamide, which then act on the cannabinoid receptors, on TRPV1 and on the PPAR receptors. The molecule was designed to remain peripheral, and the human data bear this out: after repeated doses, exposure in cerebrospinal fluid reached about 0.2 % of plasma exposure, with no central adverse effect even at supra therapeutic exposures. Phase 1 trials did show dose dependent inhibition of FAAH and a rise in plasma endocannabinoids, but the two phase 2 trials failed: in 239 men with chronic prostatitis or chronic pelvic pain syndrome, the study was stopped for futility at the interim analysis, and in 287 women with interstitial cystitis, the highest dose evaluated did not beat placebo on average daily pain. Development stopped there. No therapeutic benefit is established, and the entire class of FAAH inhibitors has disappointed in pain clinical trials.

Origin (research tool)
No natural biosynthetic pathway: this molecule falls exclusively within chemical synthesis and is not a plant metabolite. Its action, by contrast, bears on endocannabinoid metabolism, since it blocks the hydrolysis of anandamide, which is made on demand by cell membranes and then normally destroyed by FAAH.
Legal framework
France
ASP-3652 is not a cannabinoid and appears on no French list of narcotics or psychotropics. It is not named by ANSM, nor is it caught by the generic clause of annex IV of the decree of 22 February 1990, which covers tetrahydrocannabinols, their esters, ethers, salts and the salts of these derivatives: the structure does not derive from THC. The applicable status is therefore that of an unscheduled substance. That does not mean it is legally available: for want of a marketing authorisation, it remains a product strictly intended for research, and its sale or use as a medicine has no legal framework in France.
European Union
No marketing authorisation has been granted by the European Medicines Agency, development having stopped at the phase 2 stage. The substance appears neither in the international drug control conventions nor among the new psychoactive substances monitored by the EUDA, formerly the EMCDDA. It remains an abandoned drug candidate, with no commercial status in the European Union.

Petite molécule synthétique de type benzamide N (méthylsulfonyl), portant un substituant isopropylsulfanyle et une chaîne 2 hydroxy 2 phényléthylamino. Elle n'appartient à aucune famille cannabinoïde : ce n'est ni un terpénophénol végétal, ni un dérivé d'acide gras, mais un inhibiteur enzymatique classique de la chimie médicinale.
Pharmacokinetics
Administration par voie orale. Le pic plasmatique survient environ une heure après une prise unique, l'état d'équilibre est atteint en trois jours environ lors de prises répétées, et l'accumulation reste modeste. La prise avec un repas abaisse nettement la concentration maximale. L'exposition croît un peu plus que proportionnellement à la dose, sans effet marqué de l'âge ni du sexe, une exposition légèrement supérieure ayant été observée chez les femmes âgées. Le point le plus notable est la très faible pénétration centrale : les concentrations mesurées dans le liquide céphalorachidien ne représentaient qu'une fraction infime des concentrations plasmatiques, ce qui distingue cette molécule des inhibiteurs de FAAH à action centrale.
Metabolism
Le devenir de la molécule a été caractérisé par une étude de bilan de masse au carbone 14, qui a suivi la radioactivité éliminée dans les urines et les fèces après une prise orale unique, en distinguant la molécule mère de la radioactivité totale. Le programme d'interactions médicamenteuses a associé ASP-3652 au gemfibrozil puis au répaglinide, sondes habituellement employées pour explorer la voie du CYP2C8, ce qui oriente vers une implication de cette isoforme. Le détail des métabolites identifiés n'a pas fait l'objet d'une publication accessible.
Toxicology and risks
Sur l'ensemble du programme clinique, la tolérance a été jugée bonne : l'incidence des effets indésirables était comparable à celle du placebo, y compris lors d'expositions supra thérapeutiques, et aucun effet indésirable d'ordre central n'a été relevé, ce qui est cohérent avec la faible pénétration cérébrale. Ce constat mérite d'être replacé dans le contexte de la classe : un autre inhibiteur de FAAH, le BIA 10-2474, a provoqué en 2016 lors d'un essai de phase 1 mené en France des atteintes neurologiques graves et un décès, toxicité que les autorités sanitaires ont attribuée à des propriétés propres à cette molécule et non à l'inhibition de la FAAH en soi. ASP-3652 n'a jamais montré ce type de signal, mais son évaluation s'est arrêtée à la phase 2 : aucune donnée de sécurité au long cours n'existe, et aucune exposition en dehors d'un essai encadré n'est justifiable.
Detection and analysis
Aucune méthode de dépistage courante ne cible cette molécule. Elle n'est pas recherchée par les tests urinaires cannabinoïdes, qui visent les métabolites du THC, et ne partage avec eux aucun métabolite. Les dosages publiés reposent sur des méthodes bioanalytiques par chromatographie liquide couplée à la spectrométrie de masse, mises en oeuvre dans le cadre des essais cliniques, complétées par la mesure de l'activité FAAH et des taux plasmatiques d'anandamide, d'oléoyléthanolamide et de palmitoyléthanolamide comme marqueurs pharmacodynamiques.
