Molécule issue d'un programme cannabinoïde d'AstraZeneca : le code AZD-2207 apparaît dans les registres chimiques au milieu des années 2000, et une étude clinique de phase I à dose orale unique, avec et sans aliment, dans l'indication obésité, a été ouverte en septembre 2006 puis menée à son terme sans publication de résultats. La première description pharmacologique publique de la structure est bien plus tardive : elle figure sous le numéro 11d dans un article de 2017 signé par une équipe de l'université de Leiden et par des chimistes d'AstraZeneca, consacré aux relations structure, affinité et cinétique de liaison des 1,2-diarylimidazole-4-carboxamides.

AZD-2207 is a laboratory molecule with no connection whatsoever to hemp: a pharmaceutical company designed it to block the CB1 receptor, the very one that THC activates. The aim was to reduce appetite in overweight people, but the project stopped after a single trial in humans. Pharmacology

AZD-2207 belongs to the 1,2-diarylimidazole-4-carboxamides, bioisosteres of rimonabant in which the pyrazole nucleus is replaced by an imidazole, the phenyl at position 1 here bearing an ester of 3,3,3-trifluoropropane-1-sulfonic acid. At the recombinant human CB1 receptor, the molecule displaces CP55940 with a Ki of about 0.11 nM and inhibits the signalling triggered by that agonist, with an IC50 of about 1 nM in the [35S]GTPγS assay and a selectivity of about 110-fold over CB2. Its mode of action is thus the opposite of that of THC, which is a partial CB1 agonist: AZD-2207 occupies the receptor without activating it and prevents agonists from doing so. The published assays measured blockade, not inverse agonist activity, which was not tested. What is missing remains considerable: human data are lacking, on efficacy as on tolerability, the profile at TRPV1, GPR55, PPARγ or 5-HT1A has not been reported, and nothing establishes whether the molecule reaches or spares the central nervous system, even though the series was designed with a high polar surface area with the intention of limiting brain penetration. The only published human data concern its absorption: highly lipophilic and poorly soluble, it was the subject of a salt-selection study whose gains observed in vitro and in the dog were not reproduced in humans.

Pharmacokinetics

Molécule très lipophile, avec un XLogP calculé de 6,7 et une surface polaire d'environ 102 Ų, dont l'absorption est limitée par la solubilité. Un travail biopharmaceutique publié a comparé plusieurs sels de cette base faible : le sel chlorure associé à un inhibiteur de précipitation donnait des résultats favorables in vitro puis chez le chien, résultats qui n'ont pas pu être reproduits chez l'être humain, l'écart étant attribué au degré de sursaturation dans l'estomac plutôt que dans l'intestin. Une gélule d'un sel de la molécule a également servi de modèle dans une étude de transfert gastro-intestinal biopertinent portant sur l'exposition plasmatique précoce. L'essai de phase I d'AstraZeneca visait la tolérance et la pharmacocinétique de prises orales uniques, avec et sans aliment, mais ses résultats n'ont pas été publiés.

Metabolism

Aucune étude de métabolisme propre à AZD-2207 n'a été publiée : ni les enzymes impliquées, ni les métabolites, ni la demi-vie d'élimination ne sont documentés dans la littérature accessible. Les travaux disponibles sur la même série chimique portent sur d'autres composés et leurs résultats ne peuvent pas être attribués à cette molécule.

Toxicology and risks

Aucune donnée de toxicologie propre à AZD-2207 n'est publiée, et le seul essai clinique identifié, une étude de phase I à dose unique ouverte en 2006 dans l'obésité, n'a pas donné lieu à une publication de résultats. Le risque documenté est celui de la classe pharmacologique, pas celui de cette molécule : le rimonabant, seul antagoniste CB1 mis sur le marché, a été suspendu en Europe en 2008 après que le CHMP a conclu à un doublement approximatif du risque de troubles psychiatriques, et les programmes d'antagonistes CB1 à action centrale ont été interrompus dans les années suivantes. Rien ne permet d'affirmer qu'AZD-2207 partage ce risque, rien ne permet non plus de l'écarter : la question n'a pas été tranchée publiquement. En pratique, cette substance n'a jamais été évaluée pour un usage humain au-delà d'un premier essai et ne doit pas être consommée.
