Le squelette vient de la recherche académique sur la douleur : Diaz et collaborateurs publient en 2008 dans le Journal of Medicinal Chemistry une série d'acylhydrazones d'isatine N-alkylées conçue comme agonistes sélectifs du récepteur CB2, dont le chef de file est le MDA-19, étudié à l'University of Texas M. D. Anderson Cancer Center, et un brevet de la même période couvre ces modulateurs hydrazones des récepteurs cannabinoïdes ; le numéro CAS de la molécule date de cette époque. Sur le marché récréatif, la première identification documentée est américaine : le Center for Forensic Science Research and Education a reçu le 4 novembre 2021 un échantillon transmis par le laboratoire médico-légal d'Indianapolis, puis publié son rapport de caractérisation le 18 novembre 2021. Le nom BZO-CHMOXIZID vient de la nomenclature systématique proposée en 2021 par des chimistes de Cayman Chemical et du CFSRE pour cette sous-classe, où OXIZID désigne le cœur oxindole et son lien hydrazone.

An entirely synthetic molecule, absent from cannabis as from any living organism: it can be found neither in a hemp flower nor in a cannabidiol product. Its appearance on the market in 2021 is linked to the Chinese regulation that came into force in July 2021, which banned at a stroke seven chemical skeletons of synthetic cannabinoid agonists, essentially indole- and indazole-based, without covering the oxindole core of the OXIZIDs; manufacturers switched to this family, which had remained outside the scope. The sample characterised in the United States in late 2021 was a plant-like material, consistent with the usual mode of distribution of these substances: the powder is dissolved and then sprayed onto herbs, tobacco or paper, and the finished product is sold as a smoking mixture, sometimes in a presentation evoking cannabis or CBD. None of this has anything to do with hemp or with cannabidiol products.

Pharmacology

BZO-CHMOXIZID belongs to the OXIZID subclass of synthetic cannabinoid receptor agonists, built not on an indole or an indazole but on a 2-oxoindoline core bearing a hydrazone linkage and a benzamide head, with the cyclohexylmethyl tail attached to the oxindole nitrogen. In vitro, the molecule behaves as a full agonist at the CB1 receptor and a partial agonist at the CB2 receptor. In β-arrestin 2 recruitment, its potency is 84.6 nM at CB1 and 2.21 nM at CB2, making it the most potent of the five OXIZIDs compared in that work; a second laboratory measures a Ki affinity of around 0.07 µM and a potency of around 0.06 µM at CB1, with an efficacy of 95.6 % in the Gαi pathway and of 127 % in β-arrestin 2 translocation, referenced to AMB-FUBINACA. This full CB1 agonism is precisely what separates these molecules from THC, which is only a partial agonist, and what makes their effects less predictable. Potency nevertheless remains moderate compared with earlier generations of synthetic agonists, and the authors of the most complete pharmacological evaluation suggest that this middling CB1 activity could correspond to a lower toxicity than that of the preceding series, a hypothesis unverified in the field. In mice, the substance fully reproduces the discriminative signal of THC and proves the most potent of the series on that criterion. No target outside CB1 and CB2 has been studied, no clinical trial exists, and human data are entirely lacking, on effects as on toxicity, tolerance and dependence.

Pharmacokinetics

Aucune donnée de pharmacocinétique humaine n'a été publiée : ni concentration sanguine documentée, ni délai d'action, ni demi-vie, ni volume de distribution. Les seules informations disponibles viennent d'incubations sur microsomes hépatiques et hépatocytes humains, qui montrent une transformation rapide de la molécule mère au niveau de la chaîne portée par l'azote de l'oxindole. La voie d'exposition réelle étant l'inhalation d'un support végétal imprégné de façon inégale, la quantité effectivement absorbée reste imprévisible d'une bouffée à l'autre comme d'un produit à l'autre.

Metabolism

Le métabolisme a été caractérisé in vitro, sur microsomes hépatiques et hépatocytes humains, par deux équipes indépendantes. Les réactions dominantes sont la mono-hydroxylation de la queue cyclohexylméthyle, la N-désalkylation qui libère l'oxindole, la formation de cétone et l'hydrolyse de la liaison amide ; la défluoration oxydative décrite pour les OXIZID à queue fluorée ne s'applique pas ici, cette molécule ne portant pas de fluor. Les enzymes principales identifiées pour la série sont le CYP3A4, le CYP3A5 et le CYP2C9. Pour le BZO-CHMOXIZID, les auteurs retiennent comme marqueurs urinaires la molécule mère elle-même et ses métabolites mono-hydroxylés sur le cyclohexylméthyle. Aucun échantillon urinaire humain authentique contenant cette molécule n'avait été analysé dans ces travaux : les marqueurs proposés restent une prédiction issue de l'expérimentation in vitro.

Toxicology and risks

Aucun cas clinique ni décès attribué au BZO-CHMOXIZID n'a été retrouvé dans la littérature publiée : c'est un vide documentaire, pas un signal de sécurité. Ce qui est établi tient au mécanisme et à la classe. Les auteurs de l'évaluation pharmacologique de 2024 rappellent que les agonistes synthétiques des récepteurs cannabinoïdes sont associés à des effets indésirables graves et à des décès plus souvent que le THC, et le rapport de caractérisation du CFSRE porte le même avertissement pour cette molécule. L'agonisme complet de CB1, combiné à une répartition irrégulière de la poudre sur son support végétal, est le cœur du risque : convulsions, délire et agitation, tachyarythmie, vomissements incoercibles, dépression de la conscience et atteinte rénale aiguë sont documentés pour d'autres molécules de cette classe, mais pas pour celle-ci. Une publication de 2024 rapporte cinq expositions non intentionnelles à un analogue OXIZID, le BZO-4en-POXIZID, chez des personnes fréquentant des clubs à New York, la substance y étant présente comme adultérant d'autres drogues ; ces observations concernent l'analogue et non le BZO-CHMOXIZID. Les mêmes auteurs de 2024 avancent par ailleurs que l'activité CB1 modérée de la série OXIZID pourrait s'accompagner d'une toxicité inférieure à celle des générations antérieures, hypothèse qui n'a été ni confirmée ni infirmée chez l'être humain. Comme pour toute la classe, l'association avec l'alcool, les opioïdes ou les sédatifs majore le risque.

Detection and analysis

La substance échappe aux tests immunologiques destinés au cannabis, qui ne la reconnaissent pas. Son identification repose sur la chromatographie couplée à la spectrométrie de masse : le CFSRE l'a caractérisée en 2021 par chromatographie en phase gazeuse couplée à la spectrométrie de masse et par chromatographie liquide couplée à un spectromètre de masse à temps de vol, en comparaison avec un matériau de référence acquis auprès d'un fournisseur analytique. En biologie, la stratégie recommandée cible dans l'urine la molécule mère et ses métabolites mono-hydroxylés sur la queue cyclohexylméthyle, marqueurs déduits d'expériences in vitro faute d'échantillons humains positifs disponibles. Les analogues OXIZID ne différant que par leur queue N-alkyle, la séparation chromatographique et la lecture des fragments sont indispensables : un résultat acquis pour le BZO-HEXOXIZID ou le BZO-POXIZID ne vaut pas pour le BZO-CHMOXIZID.
