Comme pour les autres composés de cette série, aucune protection de propriété intellectuelle n'a été déposée, et la molécule est passée sur le marché gris. Sa puissance a été établie par comparaison directe avec les cannabinoïdes de synthèse alors dominants, dans le travail que Verena Angerer, Volker Auwärter et leurs collaborateurs de l'institut de médecine légale de Fribourg ont consacré aux liquides de cigarette électronique.

Pharmacology
CUMYL-PINACA marks a potency threshold in the history of the synthetic cannabinoids. In the cyclic AMP biosensor assay used by the forensic medicine team in Freiburg, it reaches an EC50 of 0.06 nM at the CB1 receptor and proves the most potent of the compounds compared, ahead of AM-2201 at 0.45 nM, JWH-018 at 1.13 nM and THJ-2201 at 1.68 nM. An order of magnitude therefore separates this molecule from the agonists that dominated the market a few years earlier, while tetrahydrocannabinol is only a partial agonist of much lower potency. Its structure partly explains that step up: the carboxamide linking the indazole to the cumyl head is far more stable than the ester of the quinolinyl carboxylates, which broke down under heat as under esterases. The molecule comes from the SGT series of the New Zealand company Stargate International, where it bears the code SGT-24, and it passed onto the grey market for want of filed protection. Its human metabolism is known through a controlled experiment rare in this field: after a single oral dose of 0.6 milligrams, ten metabolites were identified in urine, the main route being hydroxylation of the pentyl chain, and the recommended marker remained detectable for more than eight days. An off-target screen published in 2022 finally places it, with AMB-FUBINACA, among the synthetic cannabinoids that inhibit the greatest number of non-cannabinoid receptors at high concentration, without any numerical values having been established for it. No clinical study exists: the active human dose, tolerance and dependence are not characterised.

CUMYL-PINACA is classified as a narcotic in France, and it is so under its own name. Annex IV of the decree of 22 February 1990 mentions it explicitly, in the form « CUMYL-PINACA ou SGT-24 ou 1-pentyl-N-(2-phenylpropan-2-yl)-1H-indazole-3-carboxamide », within the generic family of the 3-carboxamide indazole derivatives. That generic clause covers molecules bearing, on the nitrogen at position 1 of the indazole, a substituent of the alkyl, haloalkyl, halobenzyl or related type, and, on the nitrogen of the carboxamide bridge, a substituent of the cumyl, naphthyl, adamantanyl, benzyl or bicycloheptanyl type: cumyl is expressly provided for, so that the molecule would be covered even without a listing by name. Production, possession, transfer, transport and use fall under the criminal law on narcotics.
European Union
No harmonised control measure at Union level covers CUMYL-PINACA: the substance does not appear in the annex to Framework Decision 2004/757/JHA, to which other, more recent carboxamides have by contrast been added by delegated acts. Control therefore remains national, and it is effective in many Member States by means of generic definitions covering indazole carboxamides with a cumyl head; France is among them, with a listing by name. The molecule is monitored by the Union's early warning system as a new psychoactive substance.

Indazole-3-carboxamide à tête cumyle : noyau indazole portant une chaîne n-pentyle sur son azote en position 1, relié en position 3 à un groupement 2-phénylpropan-2-yle, dit cumyle, par une fonction carboxamide. La tête cumyle distingue cette sous-famille des carboxamides à chaîne valinamide ou tert-leucinamide, plus fréquents dans les séries MDMB et ADB, et lui vaut son préfixe. Le carboxamide, à la place de l'ester des carboxylates quinoléinyles, rend la molécule bien plus stable, ce qui est le trait de conception majeur de cette génération.
Pharmacokinetics
Aucune étude de pharmacocinétique humaine complète n'est publiée, mais une donnée d'exposition contrôlée existe, ce qui est rare dans cette classe : une prise orale unique de 0,6 milligramme a permis de suivre l'excrétion urinaire des métabolites, et le marqueur principal restait détectable plus de huit jours après la prise. Cette fenêtre longue tient à la stabilité du carboxamide, qui distingue cette génération des esters quinoléinyles rapidement hydrolysés. Biodisponibilité, demi-vie plasmatique et volume de distribution restent en revanche inconnus, et la voie d'exposition habituelle sur le marché est l'inhalation, non la voie orale employée dans cette expérimentation.
Metabolism
Le métabolisme de phase I a été caractérisé in vitro et in vivo par l'équipe de médecine légale de Fribourg. Après administration orale contrôlée, dix métabolites ont été identifiés dans l'urine. La voie principale est l'hydroxylation de la chaîne N-pentyle. Le métabolite monohydroxylé désigné par les auteurs comme marqueur de référence reste détectable plus de huit jours après une prise unique, ce qui en fait une cible robuste pour documenter une consommation. Cette persistance contraste avec celle des cannabinoïdes de synthèse à lien ester, dont les marqueurs disparaissent bien plus vite, et elle s'explique par l'absence de point de coupure hydrolytique dans la molécule.
Toxicology and risks
Aucune étude contrôlée chez l'être humain destinée à évaluer la sécurité n'est publiée et il n'existe pas de donnée de toxicité systématique propre à cette molécule. Le profil de risque est dominé par sa puissance : à une CE50 de 0,06 nM sur CB1, la quantité nécessaire pour saturer les récepteurs est infime, et la marge entre une prise sans conséquence et une intoxication devient difficile à maîtriser lorsque la substance est pulvérisée sur un support végétal ou diluée dans un liquide de cigarette électronique. La classe est associée chez les consommateurs à des tachycardies, des convulsions, des vomissements, une agitation sévère et des décompensations psychiatriques aiguës. Le criblage hors cible de 2022 ajoute une préoccupation propre : cette molécule figure parmi celles qui touchent le plus grand nombre de récepteurs non cannabinoïdes à concentration élevée, ce qui suggère que ses effets ne se réduisent pas au seul récepteur CB1.
Detection and analysis
L'identification repose sur la chromatographie couplée à la spectrométrie de masse, en phase gazeuse ou liquide, par comparaison à un étalon de référence certifié. Les immunoessais destinés au cannabis ne réagissent pas à cette structure, et un dépistage négatif ne vaut pas absence. Dans l'urine, la recherche porte sur le métabolite monohydroxylé de la chaîne pentyle, détectable plus de huit jours après une prise unique. La détection en spectrométrie de mobilité ionique, employée pour cribler les papiers imprégnés en milieu carcéral, a été évaluée pour les cannabinoïdes de synthèse à tête cumyle, avec des valeurs de mobilité réduite qui diffèrent parfois de celles rapportées antérieurement, ce qui impose de tenir les bibliothèques d'instruments à jour.
