- Décrit en 2007 par Giblin et ses collaborateurs, chez GlaxoSmithKline, dans le Journal of Medicinal Chemistry. Le programme est parti d'un ester de pyrimidine repéré par criblage ciblé comme agoniste partiel micromolaire du récepteur CB2, puis optimisé jusqu'à cette molécule, retenue comme candidat clinique pour la douleur inflammatoire. Des essais de phase I et de phase II ont été conduits par la firme entre 2006 et 2008, avant l'arrêt du composé.

In plain terms
GW842166X is a laboratory molecule, with no botanical relation to hemp. A pharmaceutical company designed it to act only on the CB2 receptor, found mainly on immune cells, and not on the brain receptor responsible for the high produced by cannabis. Tested in humans against dental pain and against knee osteoarthritis, it did no better than a placebo and its development was abandoned.

Pharmacology
GW842166X is a selective synthetic agonist at the CB2 cannabinoid receptor, arising from a series of pyrimidine-5-carboxamides optimised by GlaxoSmithKline. It activates human CB2 with a median effective concentration of the order of 60 nanomolar and an efficacy close to that of the reference agonists, whereas no activity is measured at CB1 up to beyond 30 micromolar: this several-hundred-fold selectivity explains the absence of cannabis-type psychotropic effect. In animal models of inflammatory pain, notably the complete Freund's adjuvant model in the rat, the molecule is orally active at low dose; later university work also attributes to it anti-inflammatory effects on the chondrocyte and a neuroprotective effect in murine models of Parkinson's disease. The move to humans was disappointing. In a controlled trial after wisdom-tooth extraction, the molecule did not distinguish itself from placebo, while the ibuprofen comparator was superior to it on every endpoint; a phase II trial in knee osteoarthritis, against placebo and against naproxen, was carried out and the programme was then discontinued. No therapeutic benefit is established in humans, and human data on long-term tolerability are lacking.

Origin (research tool)
No biosynthetic pathway: the molecule has no natural equivalent and comes exclusively from organic synthesis, by assembling a trifluoromethylated pyrimidine ring, a dichlorinated aniline and an amine derived from tetrahydropyran.
Legal framework
France
GW842166X is named in no French narcotics scheduling text. It is neither a tetrahydrocannabinol nor an ester, ether or salt of THC: the generic clause of annex IV of the decree of 22 February 1990 therefore does not capture it, and its status is that of an unscheduled substance. It is, however, an investigational active substance that has never obtained a marketing authorisation. Its sale to the public, its incorporation into a consumer product and any health claim concerning it are prohibited in France, outside the strict framework of research.
European Union
No international convention on the control of narcotics or psychotropic substances covers this molecule, and the literature consulted reports no notification as a new psychoactive substance, which is consistent with its lack of activity at the CB1 receptor. Its development stopped at phase II: it holds no marketing authorisation in the European Union and benefits from no novel food status.

Pyrimidine-5-carboxamide non cannabinoïde : un noyau pyrimidine porteur d'un groupe trifluorométhyle, relié d'un côté à une aniline dichlorée et de l'autre à un amide de méthyl-tétrahydropyrane. La structure ne comporte ni cycle résorcinol ni chaîne terpénique, contrairement aux cannabinoïdes du chanvre.
Pharmacokinetics
Les données publiées concernent surtout l'animal. Chez le rat, la biodisponibilité orale rapportée est d'environ 58 % et la demi-vie d'élimination d'environ 3 heures. La molécule est nettement lipophile, avec un logP voisin de 4,3, et peu soluble en milieu aqueux, ce qui a orienté sa mise en forme orale. Chez l'humain, elle a été administrée par voie orale en prise unique à deux niveaux de dose lors de l'essai en douleur dentaire, et une étude d'imagerie a été conduite pour caractériser sa distribution cérébrale ; les résultats détaillés de cette étude n'ont pas été publiés.
Metabolism
Le profil métabolique humain détaillé n'a pas été publié. Les données précliniques indiquent une absence d'inhibition significative des principales isoformes du cytochrome P450 (1A2, 2C9, 2C19, 2D6 et 3A4) jusqu'aux fortes concentrations testées, ce qui laissait présager un faible risque d'interaction médicamenteuse. Les voies de biotransformation elles-mêmes ne sont pas documentées dans la littérature accessible.
Toxicology and risks
Dans les essais cliniques de phase II conduits en douleur dentaire et en arthrose du genou, les traitements ont été décrits comme bien tolérés, sans effet de type cannabinique sur le système nerveux central, ce qui concorde avec l'absence d'activité sur CB1. L'arrêt du programme tient au manque d'efficacité et non à un signal de sécurité identifié. Aucune donnée de tolérance à long terme n'existe chez l'humain, et la molécule n'a jamais été évaluée en dehors d'essais encadrés : son profil de risque en usage non médical reste inconnu.
Detection and analysis
Aucune méthode médico-légale dédiée n'est documentée. La molécule ne circule pas comme produit de consommation et n'est pas recherchée dans les panels toxicologiques de routine. Elle n'est pas détectée par les tests immunologiques cannabiniques, qui ciblent les métabolites du THC, et ne pourrait être identifiée que par chromatographie liquide couplée à la spectrométrie de masse, avec un standard de référence.
