La molécule est connue de longue date comme produit d'hydrogénation du cannabidiol, et une préparation à partir de précurseurs non cannabiques a été décrite par Crombie et ses collaborateurs en 1988. Ben-Shabat et ses collaborateurs en caractérisent les épimères et la liaison au récepteur CB1 en 2006. L'intérêt contemporain vient de l'équipe de Mark Mascal, à l'université de Californie à Davis, associée à l'université de Reading, qui publie en 2019 dans Scientific Reports une évaluation antiépileptique chez le rat et propose le composé comme alternative au cannabidiol, sans conversion possible en THC.

It is obtained by catalytic hydrogenation of cannabidiol extracted from the plant, which makes it a semi-synthetic compound, or by total synthesis from olivetol and alpha-phellandrene. Products marketed under this name pose a problem of identity: the UK Advisory Council on the Misuse of Drugs stresses that the designation remains ambiguous, since hydrogenation may affect the ring or the isopropenyl group, and that the batches sold most likely consist of mixtures.

H2-CBD is a modified version of cannabidiol: chemists add two hydrogen atoms to the CBD molecule, which removes any possibility of conversion into THC. It does not exist naturally in hemp and is prepared in the laboratory, from CBD or from raw materials unrelated to the plant. Pharmacology

H2-CBD differs from cannabidiol by the saturation of the exocyclic double bond of the isopropenyl group, a modification that is discreet in appearance but which closes off any route of conversion towards THC, in the laboratory as well as in the stomach. Pharmacologically, the molecule behaves as a very weak ligand of the CB1 receptor: measurable binding has been reported for hydrogenated derivatives of cannabidiol, without demonstrated receptor activation. Activity at CB2, TRPV1, GPR55, PPAR gamma or 5-HT1A has not been characterised for this compound. What is established amounts to a few preclinical studies: a reduction in the number and severity of pentylenetetrazole-induced seizures in the rat, of a magnitude comparable to that of cannabidiol, cytochrome P450 inhibition of the same order, and similar antioxidant and antibacterial activities in vitro. What is not established represents everything else: no clinical trial, no safety data in humans, and an antiepileptic mechanism that remains unexplained since it manifestly does not act through CB1.

No known biosynthetic pathway: the molecule is not produced by the hemp plant. It results from the catalytic hydrogenation of cannabidiol, or from a total synthesis starting from olivetol and alpha-phellandrene, two precursors unrelated to cannabis.

Pharmacokinetics

Les données humaines manquent : aucune étude de pharmacocinétique clinique n'a été publiée. Les travaux précliniques reposent sur l'administration intrapéritonéale chez le rat, avec un effet anticonvulsivant qui croît avec la dose. La biodisponibilité orale, la demi-vie, la distribution tissulaire et le passage de la barrière hématoencéphalique chez l'homme ne sont pas documentés.

Metabolism

Le métabolisme du H2-CBD n'est pas caractérisé chez l'homme et aucun métabolite urinaire n'a été décrit à ce jour. L'étude de 2019 rapporte une inhibition du cytochrome P450 comparable à celle du cannabidiol, ce qui laisse attendre un potentiel d'interaction du même ordre avec les traitements pris en charge par ces enzymes. Point structurel important : la molécule ne peut pas se convertir en THC, ni par voie chimique ni dans l'environnement acide de l'estomac.

Toxicology and risks

Aucune donnée de toxicologie humaine n'est disponible. Chez le rat, l'administration intrapéritonéale à visée anticonvulsivante n'a pas donné lieu à des effets indésirables rapportés dans l'étude de 2019, et un travail in vitro de 2023 décrit une toxicité faible sur fibroblastes cutanés humains à des concentrations très supérieures aux concentrations actives sur bactéries. Ces éléments ne valent pas démonstration d'innocuité. L'ANSM a motivé le classement de la famille semi-synthétique par des signalements de consommation aux effets graves reçus par les centres d'évaluation et d'information sur la pharmacodépendance. S'y ajoute un risque de composition : ce qui est vendu sous le nom H2-CBD peut être un mélange d'isomères d'hydrogénation, voire contenir d'autres cannabinoïdes de synthèse.

Detection and analysis

Le H2-CBD échappe aux tests immunologiques rapides : les dérivés hydrogénés du cannabidiol ne provoquent pas de réaction croisée sur les criblages usuels du cannabis, comme cela a été constaté pour l'analogue tétrahydrogéné dans des dossiers de conduite sous influence. Une identification fiable réclame la chromatographie liquide couplée à la spectrométrie de masse, la difficulté analytique portant sur la séparation des isomères de position et des diastéréoisomères issus de l'hydrogénation. Aucun dépistage de routine ne cible ce composé en France.
