La série des 3,4-diarylpyrazolines est issue d'un criblage à moyen débit, puis optimisée pour corriger une absorption orale médiocre. La publication princeps, parue dans le Journal of Medicinal Chemistry en 2004, décrit le candidat sous le numéro de composé 80, devenu SLV319. Bristol-Myers Squibb a ensuite repris le développement sous le code BMS-646256, et la dénomination commune ibipinabant a été attribuée en 2007.

In plain terms

Ibipinabant is a laboratory molecule with no connection to hemp: it was made to block the CB1 receptor, the very one that THC activates. Two pharmaceutical companies developed it in the 2000s as an appetite suppressant against obesity and type 2 diabetes. The programme was halted, because of muscle damage observed in animals and because this whole family of medicines was set aside after the psychiatric effects of rimonabant.

Pharmacology

At the human CB1 receptor, ibipinabant binds with an affinity of about eight nanomolar and roughly a thousandfold more weakly to the CB2 receptor; the literature describes it sometimes as an antagonist, sometimes as an inverse agonist, depending on the functional assays used. In rodents, it reduces food intake and improves glycaemic control, including independently of weight loss in a Zucker rat model of diabetes, where it preserves pancreatic islet mass. Later work has nonetheless shown that part of these metabolic effects proceeds through a partial opening of SUR1/Kir6.2 potassium channels, hence through a pathway foreign to the CB1 receptor. A phase IIb trial in obese and diabetic patients had been registered by Solvay, then withdrawn before any enrolment: published human data remain almost non-existent, and no clinical efficacy has been established. The programme came to a halt on a double obstacle: the muscle damage observed in a preclinical study in dogs, attributed to inhibition of mitochondrial ADP/ATP transport, and the general abandonment of the class after the withdrawal of rimonabant. The molecule survives today only as a pharmacological tool, with a recent antibacterial repositioning lead still at the preclinical stage.

No biological route exists for this molecule: ibipinabant is a product of organic synthesis belonging to the diarylpyrazoline class, with no plant precursor and no enzymatic intermediate. Hemp does not make it, and it appears in no natural cannabinoid profile.

3,4-diarylpyrazoline, c'est-à-dire un noyau 4,5-dihydro-1H-pyrazole portant deux groupements aryle et une fonction sulfonylcarboxamidine. L'énantiomère actif possède la configuration 4S, établie par diffraction des rayons X. La molécule ne dérive pas du squelette terpénophénolique du cannabis ; elle se rapproche du pyrazole rimonabant par son mode de liaison au récepteur, non par sa filiation naturelle.

Pharmacokinetics

Actif par voie orale chez l'animal. L'ibipinabant est issu de l'optimisation d'une tête de série mal absorbée : le passage d'une fonction amidine primaire à un dérivé N-méthylé a été le changement décisif pour obtenir une biodisponibilité orale. Les travaux princeps signalent un rapport de concentration cerveau sur plasma élevé, donc une pénétration marquée dans le système nerveux central, à l'opposé des antagonistes CB1 périphériques conçus plus tard. Sa forte lipophilie et sa faible solubilité aqueuse ont imposé une mise en forme galénique particulière, une dispersion solide amorphe séchée par atomisation, pour la fabrication des comprimés destinés aux essais. Les paramètres pharmacocinétiques humains détaillés n'ont pas été publiés.

Metabolism

Le métabolisme humain de l'ibipinabant n'a fait l'objet d'aucune publication détaillée : ni les isoformes du cytochrome P450 impliquées, ni les métabolites majeurs, ni les voies d'élimination ne sont documentés dans la littérature accessible. Des métabolites circulants ont bien été observés lors des études bioanalytiques chez l'animal, où ils ont été écartés par filtrage de défaut de masse, mais ils n'y sont pas caractérisés individuellement.

Toxicology and risks

Le signal de sécurité déterminant est une toxicité musculaire apparue lors d'une étude préclinique de six semaines chez le chien. Une analyse métabolomique par chromatographie liquide couplée à la spectrométrie de masse haute résolution y a identifié une élévation nette de plusieurs acylcarnitines plasmatiques, signature d'une perturbation de la bêta-oxydation des acides gras, proposée comme marqueur précoce de cette atteinte. Le mécanisme a ensuite été rattaché non pas au récepteur CB1 mais à une cible hors cible : l'inhibition de la translocase des nucléotides adényliques, donc de l'échange ADP/ATP mitochondrial, avec production accrue d'espèces réactives de l'oxygène et baisse de la capacité de production d'ATP dans des myoblastes en culture. Une modification structurale mineure suffit à supprimer cette inhibition, ce qui confirme qu'elle est dissociable de l'activité sur CB1. Par ailleurs, la classe des antagonistes CB1 a été associée chez l'être humain à des troubles dépressifs et anxieux, motif du retrait du rimonabant ; l'ibipinabant, fortement pénétrant dans le cerveau, partageait ce risque de classe. Pour la molécule elle-même, les données humaines manquent.

Detection and analysis

Aucune méthode forensique de routine n'est établie : la substance n'est pas un produit de consommation et ne figure pas dans les criblages toxicologiques usuels ni dans les systèmes d'alerte européens sur les nouvelles substances psychoactives. Les dosages publiés relèvent de la bioanalyse pharmaceutique, par chromatographie liquide couplée à la spectrométrie de masse sur plasma animal. Une recherche ciblée par spectrométrie de masse haute résolution reste la voie envisageable si le composé doit être identifié dans un échantillon.
