Pharmacology
JNJ-42165279 is not a cannabinoid: it is an arylpiperazine urea that inhibits FAAH (fatty acid amide hydrolase), the enzyme responsible for hydrolysing anandamide, oleoylethanolamide and palmitoylethanolamide. The urea carbonyl carbamylates the catalytic serine of the enzyme and forms a slowly reversible covalent adduct; inhibition is potent in cells, of the order of 75 nM, and selective, with no direct affinity reported for the CB1 or CB2 receptors. The effect therefore passes entirely through the accumulation of the substrate lipids, which then stimulate their own targets: CB1, CB2 and TRPV1 for anandamide, PPARα for the other two. In healthy volunteers, positron emission tomography shows brain FAAH occupancy of 96 to 98%, with plasma anandamide increased 5- to 10-fold and cerebrospinal fluid anandamide increased 45- to 77-fold. The target is thus unambiguously engaged, but clinical benefit does not follow: the phase 2 trials in social anxiety, major depression with anxious distress, autism spectrum disorder and post-traumatic stress disorder all missed their primary endpoint. No therapeutic effect is established in human beings.

Origin (research tool)
No biological pathway: the molecule comes from no living organism and has nothing to do with the biosynthesis of hemp cannabinoids. It is obtained by organic synthesis, in the class of arylpiperazine ureas, by combining a 2,2-difluoro-1,3-benzodioxole motif with a 4-chloropyridin-3-ylamine.
Legal framework
France
In France, JNJ-42165279 is named in none of the annexes to the decree of 22 February 1990 setting the list of substances classified as narcotics. Nor does the molecule fall under the generic clause of annex IV covering the tetrahydrocannabinols, their esters, ethers and salts as well as the salts of those derivatives: it is neither a tetrahydrocannabinol nor a tetrahydrocannabinol derivative, but an arylpiperazine urea with no chemical kinship to the cannabinoids. It likewise escapes the provisions covering cannabinoid receptor agonists, since it does not act on those receptors. Its status is therefore that of an unscheduled substance. That does not mean it is available: in the absence of a marketing authorisation, it is an investigational medicinal product whose administration to human beings is lawful only within research authorised by the ANSM. Its sale as a consumer product would be illegal, not under the narcotics rules, but under medicines regulation.
European Union
At European level, the molecule appears neither in the schedules of the United Nations conventions on narcotic drugs and psychotropic substances, nor among the new psychoactive substances placed under control under Framework Decision 2004/757/JHA as amended. It is not monitored as a street substance by the EUDA (formerly the EMCDDA), which is simply explained: it has never left the pharmaceutical circuit. No marketing authorisation has been granted by the EMA or by a national agency, development having stopped at phase 2. Its status is therefore that of an investigational medicinal product, governed by the European regulation on clinical trials, and not that of a consumer product.

Urée arylpipérazinique. Le squelette associe une pipérazine N-carboxamide, un groupe 4-chloropyridin-3-yle porté par l'azote de l'urée et un méthylène relié à un noyau 2,2-difluoro-1,3-benzodioxole. Cette architecture est étrangère aux terpénophénols du chanvre : il n'y a ni cycle résorcinol, ni chaîne alkyle latérale, ni motif cannabinoïde.
Pharmacokinetics
Administration par voie orale. Chez le volontaire sain, dans une étude de doses répétées croissantes conduite sur dix jours, la demi-vie plasmatique moyenne s'est située entre 8,1 et 14,1 heures, avec une accumulation modérée à l'état d'équilibre. La liaison aux protéines plasmatiques est élevée, la fraction libre étant estimée entre 6,4 et 7,5 %. Le passage vers le système nerveux central est réel mais quantitativement faible : les concentrations mesurées dans le liquide céphalorachidien représentaient un peu plus de 3 % du pic plasmatique. Cela suffit néanmoins à saturer la cible, puisque l'occupation cérébrale de la FAAH mesurée par tomographie par émission de positons avec le traceur [11C]MK-3168 atteint 96 à 98 % dès les paliers intermédiaires et reste supérieure à 80 % au creux résiduel. L'activité FAAH des leucocytes s'effondre sous les 11 % de sa valeur initiale et le reste tout au long du traitement.
Metabolism
Le devenir métabolique détaillé n'est pas publié dans la littérature accessible. Le mécanisme d'action repose lui-même sur une réaction chimique avec la cible : la molécule se comporte en substrat de la FAAH et carbamyle la sérine catalytique, formant un adduit covalent lentement réversible qui inactive l'enzyme. Les essais d'interaction menés sur microsomes hépatiques humains ne montrent qu'une inhibition faible du CYP2D6, à des concentrations très supérieures aux expositions cliniques. Chez le volontaire sain, les concentrations d'acide arachidonique du liquide céphalorachidien restent inchangées sous traitement, ce qui confirme que le blocage porte bien sur l'hydrolyse des amides d'acides gras et non sur une étape ultérieure.
Toxicology and risks
Le profil observé chez l'être humain est resté modéré. Dans les études de phase 1 chez le volontaire sain, aucun décès, aucun événement indésirable grave ou sévère et aucun arrêt de traitement pour effet indésirable n'ont été rapportés ; les manifestations les plus fréquentes étaient des céphalées, des douleurs dorsales et de la fatigue, avec quelques élévations discrètes et réversibles des transaminases. Les essais de phase 2 menés ensuite ont conclu à une tolérance acceptable, sans signal nouveau. Un point mérite d'être signalé : chez le rat Sprague-Dawley, une exposition in utero à des niveaux très supérieurs aux expositions pharmacologiques a augmenté l'incidence de cataractes nucléaires chez les jeunes, sur un fond de lésions du cristallin déjà spontanément présentes dans cette souche. Enfin, en janvier 2016, après l'accident clinique survenu à Rennes avec un autre inhibiteur de la FAAH, le BIA 10-2474, l'administration a été suspendue par précaution dans les essais de JNJ-42165279, alors qu'aucun événement indésirable grave n'y avait été observé ; les essais ont ensuite repris. Les données de sécurité au long cours restent limitées.
Detection and analysis
Aucune méthode de dépistage de routine n'existe : la molécule n'est ni un stupéfiant classé, ni une substance recherchée dans les panels toxicologiques ou antidopage courants. Dans le cadre des essais cliniques, le dosage a été réalisé par chromatographie liquide couplée à la spectrométrie de masse en tandem, dans le plasma, le liquide céphalorachidien et les urines. Un marqueur pharmacodynamique complémentaire existe : l'effondrement de l'activité FAAH des leucocytes et l'élévation marquée des amides d'acides gras circulants, en particulier l'anandamide, signent une inhibition de l'enzyme.
