- Le squelette indazole-3-carboxamide est issu d'un brevet pharmaceutique de Buchler et collaborateurs déposé en 2009, sans rapport avec un usage récréatif. MDMB-FUBINACA lui-même est apparu sur le marché illicite en 2014 dans la Fédération de Russie, où il a été associé à une vague d'intoxications collectives. Sa pharmacologie a été caractérisée par Banister et collaborateurs en 2016, puis par Gamage et collaborateurs en 2018 ; il a servi d'agoniste de référence dans la première structure cryo-électronique du complexe récepteur CB1 et protéine G, publiée en 2019.

It acts on the same receptors as THC but far more strongly, and it has been linked to hundreds of severe poisonings and to several deaths. It is classified as a narcotic in France and goes into no legal CBD product.

Pharmacology
MDMB-FUBINACA is a synthetic cannabinoid receptor agonist (SCRA) of the indazole-3-carboxamide family, with no structural kinship to the cannabinoids of hemp. It binds the human CB1 and CB2 receptors with an affinity in the nanomolar range or below, at least ten times that of delta-9-THC, and behaves in signalling assays as a full agonist where delta-9-THC is only a partial agonist. It is this combination of potency and maximal efficacy, and not any original pharmacology, that explains the very narrow margin between the sought-after effect and poisoning. In rodents it fully substitutes for delta-9-THC in stimulus discrimination, which suggests cannabis-like subjective effects, but at doses about two hundred times lower. What the literature does not establish counts just as much: no controlled clinical study exists, human pharmacokinetic data are lacking, no activity is documented at TRPV1, GPR55, PPAR-gamma or 5-HT1A, and the World Health Organization found no formal toxicology study. No therapeutic use is recognised.

Origin (pharmaceutical research → illicit market)
No biological pathway. MDMB-FUBINACA does not exist in living organisms and has never been isolated from a plant: it is obtained entirely by synthesis, through alkylation of the N1 nitrogen of an indazole-3-carboxylate and then amide coupling with a hindered amino-acid methyl ester. The indazole-3-carboxamide skeleton comes from a series of pharmaceutical patents filed in 2009, subsequently diverted to the illicit market.
Legal framework

A substance classified as a narcotic. MDMB-FUBINACA appears by name in annex IV of the decree of 22 February 1990 setting the list of substances classified as narcotics, within the structural family of indazole 3-carboxamide derivatives, since the decree of 31 March 2017 published in the Official Journal of 6 April 2017. The situation is twofold: the molecule is covered by a generic definition specific to synthetic cannabinoids, and it is furthermore cited by its name and by its IUPAC name in the consolidated list published by the ANSM. The generic clause covering tetrahydrocannabinols, their esters and their ethers does not come into play here, since this molecule is not a THC derivative. Production, possession, transfer and use are prohibited and criminally punishable; no legal CBD product may contain it.
European Union
MDMB-FUBINACA has been monitored by the European Union Early Warning System and triggered a public health alert from the European drugs monitoring centre in 2016, without any delegated directive specifically covering it; it is controlled under national law in most Member States. The framework changed in 2026: on the recommendation of the World Health Organization expert committee that met in October 2025, the United Nations Commission on Narcotic Drugs placed MDMB-FUBINACA in Schedule II of the 1971 Convention on Psychotropic Substances at its March 2026 session, which now imposes national control on all parties. In the United States, the substance has been in Schedule I of the Controlled Substances Act since 2020.

Carboxamide d'indazole : noyau indazole portant un groupe 4-fluorobenzyle sur l'azote N1 et une fonction carboxamide en position 3, reliée à un groupe tert-leucinate de méthyle de configuration S. Aucune parenté avec le squelette terpénophénolique des phytocannabinoïdes.
Pharmacokinetics
Les voies décrites sont exclusivement inhalées : combustion de végétaux imprégnés ou vapotage d'e-liquides. Aucune étude clinique n'a été conduite et les paramètres humains in vivo manquent. Les données disponibles sont in vitro : l'énantiomère S actif est lié aux protéines plasmatiques à 99,5 pour cent, reste stable dans le plasma sur cinq heures d'incubation, et présente une demi-vie de l'ordre de dix minutes dans des hépatocytes humains en pool, contre une vingtaine de minutes pour l'énantiomère R. La clairance hépatique humaine prédite est élevée. Conséquence pratique : la molécule mère disparaît vite de la circulation et n'est que rarement retrouvée intacte dans les prélèvements.
Metabolism
Biotransformation hépatique intense. Vingt-deux métabolites ont été identifiés dans des urines humaines authentiques, provenant de patients hospitalisés et d'investigations post-mortem. La première transformation est l'hydrolyse de l'ester méthylique terminal, qui produit l'acide carboxylique correspondant, de loin le métabolite le plus abondant. S'y ajoutent des monohydroxylations, une déshydrogénation, la formation de dihydrodiols et la perte du groupe fluorobenzyle, puis une glucuroconjugaison de phase II. Une partie de ces métabolites est commune avec celle de l'ADB-FUBINACA, ce qui complique l'attribution analytique quand les deux substances circulent en même temps.
Toxicology and risks
Les études de toxicologie réglementaire font défaut : l'Organisation mondiale de la santé n'a retrouvé aucune donnée formelle. Le signal de danger vient de la clinique et de la chimie. Chez l'animal, la substance supprime l'activité locomotrice à des doses environ deux cents fois plus faibles que le delta-9-THC, provoque une hypothermie prolongée réversible par un antagoniste CB1 mais pas par un antagoniste CB2, et une bradycardie légère. Chez l'humain, les signes rapportés lors d'intoxications confirmées analytiquement comprennent agitation ou sédation, vomissements, pertes de mémoire à court terme, hypersalivation, rhinorrhée, mydriase, tachycardie et anxiété. Au moins dix-sept décès lui ont été attribués, dont quinze dans la Fédération de Russie en 2014, avec plus de six cents intoxications signalées en deux semaines. Un facteur aggravant tient au mode de consommation : fortement chauffée, la molécule se dégrade en naphtalène, en 1-naphtylamine et en toluène, et libère du cyanure, tous inhalés avec la fumée ou la vapeur.
Detection and analysis
Le composé parent est difficile à mettre en évidence : dans une série néo-zélandaise de sangs ante-mortem et post-mortem, il n'a été retrouvé que sous la forme de son métabolite acide, à des concentrations basses. Les méthodes validées reposent sur la chromatographie liquide couplée à la spectrométrie de masse en tandem ou à haute résolution, avec des limites de détection de l'ordre du nanogramme par millilitre dans le sang total. En urine, la stratégie recommandée consiste à cribler d'abord les métabolites acides, très abondants, puis à confirmer sur les métabolites hydroxylés. Aucun immunodosage rapide ne cible spécifiquement cette molécule, et les tests urinaires destinés au cannabis ne la détectent pas.
