Otenabant is an experimental medicine developed by the Pfizer laboratory against obesity. Instead of mimicking the cannabinoids, it does the opposite: it blocks the brain's CB1 receptor, the one THC activates, which suppresses appetite. The clinical trials were halted because the product increased anxiety and dark thoughts in the people treated.

Pharmacology
Otenabant is a competitive antagonist and inverse agonist at the CB1 receptor, arising from a series of 8,9-diarylpurines developed at Pfizer. Its affinity for human CB1 is subnanomolar (0.7 nM in binding, 0.12 nM in a functional test) against 7,600 nM for CB2, that is a selectivity close to ten thousand-fold. In animals it abolishes the four central responses provoked by a cannabinoid agonist (reduced motor activity, hypothermia, analgesia, catalepsy), reduces food intake and increases energy expenditure as well as fat oxidation. In humans, three phase 3 trials conducted at 10 and 20 mg per day in more than 4,700 participants did show dose-proportional weight loss after one year, but also an increased frequency of anxiety, depressed mood, depression and suicidal ideation compared with placebo. The programme was interrupted before its term, after the withdrawal of rimonabant. The literature therefore establishes no usable therapeutic benefit, and the benefit-risk ratio of central CB1 inhibition remains unfavourable. A 2026 preprint identifies an unexpected secondary target, the human PIEZO1 channel, but this result has not yet been validated by peer review.

Origin (research tool)
No biosynthetic pathway: otenabant is an entirely synthetic molecule, arising from a series of diarylpurines developed in medicinal chemistry. No living organism, plant or animal, produces it, and it derives from no natural cannabinoid precursor.
Legal framework
France
An unscheduled substance in France. Otenabant is neither a tetrahydrocannabinol, nor an ester, ether or salt of THC: the generic clause of annex IV of the decree of 22 February 1990 therefore does not capture it, and the ANSM does not mention it by name in any list of narcotics or psychotropic substances. This absence of classification does not amount to authorisation. The molecule never obtained a marketing authorisation, its clinical development was halted, and it may lawfully appear neither in a food supplement nor in an everyday consumer product. Its use is limited to research.
European Union
No Member State has authorised otenabant as a medicine: the phase 3 programme was interrupted before any application was filed, in the regulatory context created by the suspension of Acomplia (rimonabant) by the European Medicines Agency on 13 November 2008, followed by the withdrawal of the authorisation on 16 January 2009. The substance does not appear in the international conventions on narcotics and is not monitored by the EUDA as a new psychoactive substance, for want of presence on the recreational market. The status of a research-only compound prevails throughout the Union.

Diarylpurine synthétique. Le noyau purine porte un groupe 2-chlorophényle en position 8 et un 4-chlorophényle en position 9 ; la position 6 est reliée à une pipéridine disubstituée portant un carboxamide et un groupe éthylamino. La molécule n'a aucune parenté structurale avec les cannabinoïdes du chanvre, dont elle ne partage ni le squelette terpénophénolique ni la chaîne alkyle.
Pharmacokinetics
Après une dose orale unique de 25 mg marquée au carbone 14 chez des volontaires sains, l'absorption est lente, le pic plasmatique survenant vers six heures. La récupération totale de la radioactivité dans l'urine et les selles atteint environ 60 % sur 672 heures, avec une nette prédominance de la voie fécale. Moins de 2 % de la dose est retrouvée sous forme inchangée dans les excreta, ce qui indique un métabolisme très poussé malgré une bonne absorption. Chez le rat, la souris et le chien, l'excrétion fécale domine également et reflète une élimination biliaire plutôt qu'une absorption incomplète.
Metabolism
La voie principale est une N-déséthylation catalysée pour l'essentiel par les cytochromes CYP3A4 et CYP3A5, qui produit le métabolite M1. Une étude comparative des deux isoformes place otenabant parmi les substrats dont la clairance dépend le plus fortement du CYP3A5, ce qui laisse attendre une variabilité entre individus liée au génotype. M1 est ensuite transformé par hydrolyse de la fonction amide, N-hydroxylation, hydroxylation du cycle pipéridine et conjugaison au ribose ; le dérivé N-hydroxylé évolue vers des métabolites oxime et cétone. M1 et ces deux derniers dérivés circulent à des concentrations comparables ou supérieures à celles de la molécule mère, M1 atteignant environ cinq fois l'exposition du composé parent.
Toxicology and risks
Le signal de sécurité est psychiatrique et central. Dans les essais de phase 3, l'anxiété, l'humeur dépressive, la dépression et les idées suicidaires rapportées par les participants étaient plus fréquentes sous otenabant que sous placebo, aux doses de 10 comme de 20 mg. Les effets indésirables les plus courants comprenaient diarrhée, nausées, rhinopharyngite et céphalées. Ce profil recoupe celui observé avec le rimonabant et correspond au risque de classe attribué au blocage de l'activité constitutive du CB1 cérébral. Aucune donnée de toxicité chronique chez l'humain n'est disponible, les essais prévus sur deux ans ayant été arrêtés avant leur terme.
Detection and analysis
Aucune méthode de dépistage de routine n'est décrite. La substance n'a jamais circulé sur le marché de consommation et ne figure pas dans les panels toxicologiques usuels ni dans les criblages de cannabinoïdes de synthèse. Les données publiées reposent sur des études de disposition au carbone 14 associées à la chromatographie liquide et à la spectrométrie de masse, employées pour identifier les métabolites chez l'animal et chez le volontaire sain.
