In plain terms
PF-04457845 is a laboratory molecule, with no connection to hemp. Instead of mimicking cannabis, it blocks the enzyme that destroys anandamide, a substance the body makes itself, which leaves more of it in circulation. It has been tested against pain, cannabis withdrawal and post-traumatic stress, but it has never been authorised as a medicine and is found in no commercial product.

Pharmacology
PF-04457845 is an irreversible and highly selective inhibitor of FAAH, the enzyme that degrades anandamide. It carbamylates the catalytic serine of the enzyme, with an IC50 of 7.2 nM at the human protein and a remarkable selectivity within the serine hydrolases. It does not bind to CB1 or CB2: its action is indirect, through accumulation of anandamide and the related ethanolamides. In human beings, this biochemical effect is firmly established, blood and brain FAAH activity being blocked by more than 90 per cent and circulating fatty acid amides increased several-fold. The clinical translation, by contrast, is disappointing: in knee osteoarthritis the molecule produced no analgesic benefit whereas naproxen used as a comparator did, which revealed a clear gap between the animal models and the patient. A phase 2a trial did, on the other hand, observe a reduction in cannabis withdrawal symptoms and in consumption among dependent men, an isolated result that requires confirmation. Recent reviews conclude that there is a modest benefit in cannabis use disorder and an absence of efficacy in pain, and development was halted in 2023.

No known biosynthetic pathway: no living organism produces this molecule. It is obtained by organic synthesis, through the construction of a urea between a piperidine substituted with an aryl ether motif and an aminopyridazine.

France
PF-04457845 is named in no annex of the decree of 22 February 1990 setting the list of substances classified as narcotics or psychotropics. Nor is it caught by the generic clause of annex IV covering tetrahydrocannabinols, their esters, ethers, salts and the salts of the aforementioned derivatives: it is neither a cannabinoid nor a THC derivative, but a synthetic heteroaromatic urea. Its status is therefore not that of a narcotic. It is, however, an investigational medicine without a marketing authorisation: in France, its use falls solely within research involving the human person, with authorisation from the ANSM and a favourable opinion from a personal protection committee. Selling it to the public or presenting it as a food supplement would be unlawful under the public health code.
European Union
No European text classifies PF-04457845: the molecule appears neither in the Council decisions on new psychoactive substances, nor in the schedules of the international conventions of 1961 and 1971 monitored by the INCB. The EUDA, formerly the EMCDDA, does not list it among the emerging psychoactive substances of the Early Warning System. On the pharmaceutical side, it has never received an authorisation from the European Medicines Agency and retains only the status of an investigational medicine, governed by clinical trials regulation.

Urée hétéroaromatique de synthèse : une pipéridine substituée par un groupe benzylidène portant un éther de pyridine trifluorométhylée, reliée par une fonction urée à une pyridazin-3-ylamine. Agent carbamylant de sérine, sans parenté structurale avec les cannabinoïdes végétaux ni avec les endocannabinoïdes lipidiques.
Pharmacokinetics
Chez le volontaire sain, l'absorption orale est rapide, le pic plasmatique survenant en moins de deux heures, sans effet notable de la prise alimentaire. L'état d'équilibre est atteint en une semaine environ lors d'une administration quotidienne. Moins de 0,1 pour cent de la dose est retrouvé inchangé dans les urines, ce qui indique une élimination essentiellement métabolique. L'inhibition étant irréversible, la durée d'action ne dépend pas de la demi-vie plasmatique mais de la resynthèse de l'enzyme : l'activité FAAH est supprimée à plus de 97 pour cent dès les faibles doses et ne revient à sa valeur initiale qu'en une quinzaine de jours après l'arrêt. Une étude de tomographie par émission de positons a confirmé un blocage de la FAAH cérébrale supérieur à 90 pour cent.
Metabolism
Le métabolisme fait intervenir principalement le cytochrome CYP3A4, d'après les données rassemblées par PubChem, et l'excrétion urinaire de la molécule mère est négligeable. Le paramètre déterminant reste toutefois la carbamylation irréversible de la FAAH : l'enzyme modifiée doit être remplacée par une néosynthèse protéique pour que l'activité revienne, ce qui découple l'effet biologique de la cinétique plasmatique.
Toxicology and risks
Aux doses évaluées, la molécule a été bien tolérée chez le volontaire sain comme chez le patient, sans altération mesurée des fonctions cognitives et sans signal de sécurité majeur ; dans l'essai sur le sevrage cannabique, la fréquence des effets indésirables était comparable à celle du placebo et aucun événement grave n'a été rapporté. Le contexte de sécurité de la classe reste marqué par l'accident de l'essai de phase 1 de Rennes en 2016, conduit avec le BIA 10-2474, un autre inhibiteur de la FAAH, qui a causé un décès et des atteintes neurologiques sévères. Les analyses ultérieures rattachent cet accident à la faible sélectivité du BIA 10-2474 et de ses métabolites, qui touchent de nombreuses hydrolases ainsi que des aldéhyde déshydrogénases, profil que PF-04457845 ne partage pas. Cela ne vaut pas garantie d'innocuité : les données humaines portent sur quelques centaines de participants seulement et sur des expositions courtes, et il n'existe aucune donnée de sécurité au long cours.
Detection and analysis
Aucune recherche de routine de cette molécule n'existe en toxicologie médico-légale ni dans les panels antidopage. Les travaux publiés reposent sur des dosages plasmatiques par chromatographie liquide couplée à la spectrométrie de masse, sur la mesure de l'activité FAAH des leucocytes et sur le dosage des amides d'acides gras circulants, qui servent de marqueurs indirects de l'exposition et de l'observance. En imagerie, l'occupation de la FAAH cérébrale a été quantifiée par tomographie par émission de positons à l'aide d'un radioligand dédié.
