- Stéroïde décrit dans les années 1930 pendant les travaux d'isolement et de synthèse des hormones stéroïdiennes, la première synthèse étant attribuée à Adolf Butenandt et ses collaborateurs en 1934. Son action sur le récepteur cannabinoïde CB1 est bien plus récente : elle a été rapportée en janvier 2014 dans la revue Science par les équipes Inserm de Pier Vincenzo Piazza et Giovanni Marsicano, au Neurocentre Magendie de Bordeaux.

French researchers showed in 2014 that the brain produces a great deal of it after THC intake, and that it then curbs part of the effects of cannabis, rather like a natural safety brake.

Pharmacology
Pregnenolone is the first steroid derived from cholesterol and the common precursor of all the body's steroids. Its interest for the endocannabinoid system comes from work published in Science in 2014 by the Inserm teams in Bordeaux: THC causes a massive rise in brain pregnenolone, of the order of thirtyfold within two hours, and this pregnenolone then binds to an allosteric site on the CB1 receptor. There it does not displace agonists and does not alter the inhibition of cyclic AMP, but it blocks the phosphorylation of the MAP kinases ERK1/2 and the drop in mitochondrial respiration triggered by the receptor. This profile, described as signalling-specific inhibition, attenuates in animals the sedation, memory impairment and cannabinoid self-administration associated with THC, without any behavioural effect of its own and without precipitating withdrawal, unlike orthosteric antagonists such as rimonabant. The limitations are real: an independent team did not reproduce this modulation with 2-AG in autaptic neurons, which suggests agonist dependence, and human data are lacking for pregnenolone itself in this indication. Its low oral bioavailability and rapid conversion into other steroids have led to the development of non-metabolisable derivatives, including AEF0117, evaluated up to phase 2a in cannabis use disorder.

Pregnenolone is the first steroid formed from cholesterol: the StAR protein carries cholesterol to the inner mitochondrial membrane, where the enzyme CYP11A1 (cytochrome P450scc) cleaves the side chain. This reaction takes place in the adrenal cortex and the gonads, as well as directly in the brain, where pregnenolone is synthesised de novo by astrocytes and several types of neuron, which earns it the status of a neurosteroid. It is then the obligatory precursor of progesterone, DHEA, the androgens, the oestrogens and the corticosteroids.
Legal framework
France
Pregnenolone is not a narcotic in France. It is not covered by the generic clause of annex IV of the decree of 22 February 1990, which concerns tetrahydrocannabinols and their esters, ethers and salts: it is a steroid, devoid of any THC-type skeleton. Nor does it fall within the scope of the ANSM Decision of 22 May 2024 relating to derivatives of the benzo[c]chromene nucleus. It is governed by hormone law rather than narcotics law: unlike in the United States, where it is sold freely as a food supplement, in France it can be obtained only on prescription, from a pharmacist who puts it up in capsules, according to the Vidal reference. No French marketing authorisation has been granted for it.
European Union
An endogenous substance, not listed under the international narcotics control conventions and absent from the lists of new psychoactive substances monitored by the EUDA. Its marketing is governed by hormone and food supplement law, whose framework remains largely national within the Union: several Member States treat hormone precursors as medicines by function. A non-metabolisable derivative of pregnenolone, AEF0117, has been evaluated in phase 1 and phase 2a clinical trials in cannabis use disorder, with no marketing authorisation to date.

Stéroïde en C21 de la série pregnane, bâti sur le squelette stérane tétracyclique, portant une fonction 3β-hydroxyle, une double liaison Δ5 et un groupe acétyle en C17. Ce n'est pas un cannabinoïde : la molécule ne possède ni noyau benzo[c]chromène ni résorcinol alkylé. Elle appartient à la famille des neurostéroïdes.
Pharmacokinetics
Molécule très lipophile qui franchit facilement la barrière hémato-encéphalique. Sa biodisponibilité orale est faible et sa demi-vie courte, ce qui a conduit les équipes de développement à écarter la prégnénolone elle-même comme candidat médicament. Les essais cliniques qui ont cherché à élever durablement les concentrations plasmatiques ont utilisé des doses orales élevées, de l'ordre de 300 à 500 milligrammes par jour répartis en deux prises. Les concentrations physiologiques matinales chez l'adulte jeune se situent entre 0,20 et 0,60 nanogramme par millilitre de sérum, avec des variations nycthémérales marquées et sans différence liée au sexe.
Metabolism
La prégnénolone est un carrefour métabolique plus qu'un produit terminal. Elle est convertie en progestérone par la 3β-hydroxystéroïde déshydrogénase (voie Δ4) ou en 17-hydroxyprégnénolone puis en DHEA (voie Δ5), et elle est sulfoconjuguée en sulfate de prégnénolone par les sulfotransférases. Dans le foie, des réductions successives conduisent au prégnanediol, éliminé principalement sous forme sulfoconjuguée dans les urines. Cette conversion rapide en stéroïdes actifs est précisément la raison pour laquelle des dérivés non métabolisables ont été mis au point pour un usage pharmaceutique.
Toxicology and risks
Aucun profil de toxicité aiguë comparable à celui des cannabinoïdes de synthèse n'est décrit : la prégnénolone est une molécule endogène et les études animales n'ont pas mis en évidence d'effet comportemental propre, ni de sevrage précipité, ni de baisse de prise alimentaire, contrairement aux antagonistes orthostériques du CB1. Les risques documentés sont d'ordre hormonal et découlent de sa conversion en androgènes et en œstrogènes : acné, chute de cheveux, pilosité accrue et modification de la voix chez la femme, ainsi que nausées, fatigue, douleurs abdominales, élévation de la pression artérielle et baisse du cholestérol HDL. Les références cliniques françaises la déconseillent pendant la grossesse et l'allaitement, chez l'enfant, en cas d'épilepsie et en cas d'antécédent personnel ou familial de cancer hormonodépendant. Le bénéfice cognitif observé chez l'animal n'a pas été confirmé chez l'humain.
Detection and analysis
Le dosage de la prégnénolone sérique ou plasmatique est un examen de biologie médicale courant, prescrit notamment pour rechercher un déficit en 3β-hydroxystéroïde déshydrogénase devant un hirsutisme ou une hyperandrogénie. Il repose historiquement sur la radio-immunologie précédée d'une extraction et d'une chromatographie, condition nécessaire à une bonne spécificité, et de plus en plus sur la chromatographie liquide couplée à la spectrométrie de masse en tandem. Le prélèvement se fait le matin avant dix heures en raison des variations nycthémérales. En contrôle antidopage, la prégnénolone ne figure pas nommément sur la liste des interdictions de l'Agence mondiale antidopage, même si certaines revues la rangent parmi les précurseurs de la testostérone.
