- La famille des benzoylindoles remonte aux travaux sur les aminoalkylindoles des années 1980, dont est issue la pravadoline (WIN 48,098). Ce régio-isomère du RCS-4 apparaît ensuite sur le marché des produits psychoactifs de synthèse : il figure dès 2012 dans le panel de vingt-deux cannabinoïdes de synthèse dosés dans les cheveux par Hutter et ses collègues, et sa pharmacologie sur les récepteurs cannabinoïdes est décrite en 2015 par Banister et ses collègues, dans leur étude des régio-isomères et des homologues du RCS-4.

The RCS-4 ortho isomer is a synthetic cannabinoid manufactured in a laboratory. It does not come from hemp and has nothing in common with CBD, even though it is sometimes found sprayed onto herbal mixtures sold under other names. Its possession and consumption are prohibited in France, and its effects on human beings have never been studied.

Pharmacology
The RCS-4 ortho isomer belongs to the benzoylindoles, the family of synthetic cannabinoid receptor agonists that came out of research on aminoalkylindoles. It is the regioisomer of RCS-4: the methoxy group occupies position 2 of the benzoyl ring instead of position 4. The data curated by ChEMBL for this exact molecule place its agonist activity at an EC50 of 54 nM at the human CB1 receptor and of 6.9 nM at the human CB2 receptor, measured on AtT-20 cells co-expressing GIRK channels, that is nanomolar potency at both receptors. The decisive difference from THC lies not in potency but in efficacy: the synthetic agonists of this class fully activate the CB1 receptor, whereas THC is only a partial agonist at it, which removes the ceiling of effect characteristic of cannabis and explains the severity of the intoxications reported with these products. For this particular regioisomer, the sources consulted document potency without detailing maximal efficacy. No clinical study, no human pharmacokinetic data and no case series concern it: human data are lacking, and the work published on RCS-4 itself cannot be transposed to it.

No biosynthetic pathway: the molecule exists in no living organism. It is obtained by a purely synthetic route, through acylation of an N-alkylated indole by means of a benzoic acid derivative bearing a methoxy group at the ortho position.
Legal framework
France
Prohibited in France. The molecule is not cited by name in the list of narcotics, but it is captured there by annex IV of the amended decree of 22 February 1990, under two generic clauses. On the one hand, the entry that enumerates the classified cannabinoids also covers their isomers, and RCS-4, that is 1-pentyl-3-(4-methoxybenzoyl) indole, is named there: this compound is its positional isomer. On the other hand, the same entry classifies any molecule of the indol-3-yl methanone family bearing, on the nitrogen of the indole nucleus, a substituent of the alkyl type and, on the carbon of the methanone bridge, a group of the phenyl type, whether or not substituted, a description that corresponds exactly to this structure. This is therefore a classification by generic clause and not a listing by name.
European Union
At European level, this molecule has given rise to no risk assessment and to no Union control measure: it is not among the substances added on that basis to the European definition of a drug. It is, however, monitored by the early warning system and recorded by the UNODC Early Warning Advisory among the synthetic cannabinoid receptor agonists. Nor does it appear in the schedules of the 1971 Convention on Psychotropic Substances. Control therefore falls to national laws, several member states targeting it through generic definitions comparable to the one adopted in France.

Pharmacokinetics
Aucune donnée de pharmacocinétique, humaine ou animale, n'a été publiée pour ce régio-isomère. Les études de distribution et d'élimination disponibles portent sur le RCS-4, l'isomère para, et ne peuvent pas lui être transposées.
Metabolism
Le métabolisme de ce régio-isomère n'a pas été caractérisé. Des profils métaboliques détaillés existent pour le RCS-4, l'isomère para, obtenus sur hépatocytes humains et sur urine, mais ces résultats appartiennent à cette autre molécule et ne décrivent pas celle-ci.
Toxicology and risks
Aucun cas clinique, aucune intoxication et aucun décès ne sont rapportés dans la littérature pour ce composé précis, et les données humaines manquent. Le risque se déduit de sa classe : les agonistes synthétiques des récepteurs cannabinoïdes activent pleinement le récepteur CB1, sans le plafond d'effet du THC, et les séries publiées sur d'autres membres de cette classe documentent convulsions, delirium, tachyarythmies, vomissements incoercibles, atteintes rénales aiguës et décès. Ces observations concernent d'autres molécules, pas celle-ci. Le danger propre à ces produits tient aussi à leur présentation : le principe actif est déposé de façon inégale sur des végétaux et vendu sous des étiquettes trompeuses, parfois comme produit au CBD, si bien que la personne qui les consomme ignore ce qu'elle absorbe.
Detection and analysis
Le composé est intégré aux méthodes forensiques usuelles. Hutter et ses collègues l'ont inclus, sous le nom de RCS-4 ortho isomère, dans une méthode de chromatographie liquide couplée à la spectrométrie de masse en tandem dosant vingt-deux cannabinoïdes de synthèse dans les cheveux, avec une limite de quantification de 0,5 picogramme par milligramme pour la plupart des composés. Akutsu et ses collègues l'ont ensuite intégré à une bibliothèque de soixante-deux cannabinoïdes de synthèse analysés par chromatographie en phase gazeuse couplée à la spectrométrie de masse en ionisation par photons, technique qui fournit l'ion moléculaire et aide à séparer les isomères. La difficulté analytique centrale est précisément là : le RCS-4 et ses isomères de position partagent la même masse et des fragmentations très proches, de sorte que seule une séparation chromatographique adaptée permet de les distinguer.
