- Décrite en 2015 par Guy Griebel et ses collaborateurs de Sanofi, dans un article de Scientific Reports consacré au blocage sélectif de l'hydrolyse du 2-arachidonoylglycérol. La structure cristallographique du complexe formé avec la monoacylglycérol lipase humaine avait été déposée en 2014 dans la Protein Data Bank sous le code 4UUQ. Une version marquée au carbone 11 a ensuite été publiée en 2016 par une équipe du Massachusetts General Hospital, puis utilisée pour cartographier la MAGL cérébrale chez le rat et chez le primate non humain.

Pharmacology
SAR127303 is a carbamate described in 2015 by the Sanofi teams as a covalent inhibitor of monoacylglycerol lipase (MAGL), the enzyme responsible for the degradation of 2-arachidonoylglycerol. It carbamoylates the catalytic serine of the active site, a mechanism confirmed by the crystal structure of the complex with human MAGL (code 4UUQ) and by mass spectrometry showing modification of the peptide bearing that serine. Its potency lies in the nanomolar range at the mouse enzyme as at the human enzyme. By blocking the hydrolysis of 2-AG, it strongly multiplies hippocampal levels of that mediator in the mouse, without altering anandamide or the free fatty acids measured, and without binding directly to CB1 or CB2. In rodents, this mechanism produces antinociceptive effects in models of inflammatory and visceral pain, and slows epileptogenesis in a kindling model, but it also impairs learning performance (episodic, working and spatial memory), decreases long-term potentiation at CA1 synapses and reduces hippocampal acetylcholine release, these effects being lifted by a CB1 antagonist. The authors themselves conclude that the window between analgesic benefit and memory deficit is narrow, which limits the therapeutic interest of this class. The compound has moreover served as the scaffold for a carbon-11-labelled tracer for positron emission tomography, used to map brain MAGL in the rat and the non-human primate. Human data are lacking: no published clinical study, no safety profile in humans.

No biosynthetic pathway: it is a synthetic chemical compound, built around a piperidine carbamate combined with a chlorinated sulfonamide motif and a hexafluoropropan-2-yl ester. No plant, animal or human enzyme produces it, and it appears in no natural metabolic pathway.

SAR127303 is named in no French decree and appears on no ANSM list. It is not a tetrahydrocannabinol, nor an ester, nor an ether, nor a salt of THC: it therefore does not fall within the generic clause of annex IV of the decree of 22 February 1990 either. Its status in France is that of a substance not classified as a narcotic, that is, of a research reagent: it holds no marketing authorisation, no food or cosmetic status, and has no place in a product intended for consumption.

Pharmacokinetics
Chez la souris, l'administration par voie orale donne des concentrations plasmatiques et cérébrales qui augmentent de façon presque proportionnelle à la dose, avec un pic vers quatre heures, ce qui atteste le passage de la barrière hémato encéphalique. L'inhibition de la MAGL cérébrale reste marquée pendant environ six heures, l'élévation du 2-AG hippocampique se maintient plusieurs heures, puis tout revient à la ligne de base à vingt quatre heures, moment où le composé n'est plus détectable dans le cerveau. Aucune donnée de pharmacocinétique humaine n'est publiée.
Metabolism
L'inhibition repose sur la carbamoylation de la sérine catalytique de la MAGL, avec départ de l'hexafluoropropan-2-ol : l'enzyme reste modifiée de façon durable, et la récupération de l'activité dépend de la resynthèse enzymatique. Les voies de biotransformation hépatique de la molécule elle même ne sont pas décrites dans la littérature accessible, et aucune étude de métabolisme humain n'est disponible.
Toxicology and risks
Aucune étude de toxicologie réglementaire n'a été publiée, et il n'existe aucune donnée de sécurité chez l'être humain. Chez le rongeur, le signal le plus constant est cognitif : déficits de mémoire épisodique, de travail et spatiale apparaissant à des niveaux d'exposition qui recouvrent ceux produisant l'effet antalgique, associés à une baisse de la libération hippocampique d'acétylcholine et à une diminution de la potentialisation à long terme. Un profil de type anxiogène a été rapporté au labyrinthe en croix surélevée. En revanche, la tétrade cannabinoïde classique (catalepsie, hypothermie, hypolocomotion, analgésie thermique) n'a pas été observée, et l'effet antalgique ne s'est pas émoussé après administrations répétées, contrairement à d'autres inhibiteurs de la MAGL. Les auteurs eux mêmes présentent le déficit mnésique comme la limite principale de la classe.
Detection and analysis
Aucune méthode de dépistage de routine ne cible cette molécule : elle n'apparaît ni dans les panels toxicologiques usuels, ni dans les bases européennes de saisies, ni dans les rapports de l'EUDA. En recherche, l'engagement de cible se mesure par l'activité MAGL résiduelle sur tissu et par spectrométrie de masse, qui montre la disparition du peptide non modifié portant la sérine catalytique. La version marquée au carbone 11 sert de radiotraceur en tomographie par émission de positons pour visualiser la MAGL cérébrale chez l'animal.
