In plain terms

Surinabant is an entirely artificial molecule, developed in the laboratory by the Sanofi group. It has nothing to do with hemp: instead of mimicking cannabis, it blocks the receptor to which THC normally binds. Studied as an aid to stopping smoking, it did not give convincing results and its development was abandoned in 2008.

Pharmacology

Surinabant (SR147778) is a selective antagonist at the cannabinoid CB1 receptor, of the diarylpyrazole family, structurally very close to rimonabant. Its affinity for CB1 is nanomolar and its selectivity over CB2 exceeds two orders of magnitude; initial screening revealed no notable activity at more than a hundred other targets. In animals, given orally and with a long duration of action, it antagonises the hypothermia and analgesia induced by cannabinoid agonists and reduces intake of ethanol, sucrose and food. In healthy volunteers, a cross-over study showed that it strongly inhibits the effects of inhaled THC, on heart rate as on central measures (body sway, alertness, feeling of intoxication), at medium and high doses, with no detectable effect of its own. What the literature has not established, by contrast, is a clinical benefit: the controlled smoking-cessation trial showed no improvement in the abstinence rate compared with placebo, only a modest reduction in weight gain after stopping smoking. Development was halted in 2008, after the suspension of rimonabant for psychiatric adverse effects, which carried away the whole class. Human data are therefore limited to a few phase I and phase II trials.

No natural biosynthetic pathway. Surinabant is a product of organic synthesis in the diarylpyrazole class, obtained by construction of the pyrazole nucleus followed by formation of the N-piperidinyl carboxamide. It has never been isolated from a plant, a fungus or an animal tissue.

Pharmacokinetics

Par voie orale chez le volontaire sain, le pic plasmatique survient en une heure et demie environ, et l'exposition croît de façon moins que proportionnelle à la dose. La demi-vie terminale est très longue, de l'ordre d'une semaine en administration répétée, ce qui implique une accumulation marquée. L'élimination se fait principalement par voie fécale.

Metabolism

Le métabolisme repose principalement sur le cytochrome CYP3A4, ce qui expose la molécule aux interactions classiques avec les inhibiteurs et les inducteurs de cette isoenzyme. L'excrétion est majoritairement fécale. Les métabolites n'ont pas fait l'objet d'une caractérisation publiée détaillée.

Toxicology and risks

Dans l'essai contrôlé de sevrage tabagique, les événements indésirables plus fréquents sous surinabant que sous placebo ont été des céphalées, des nausées, une insomnie, une anxiété, des rhinopharyngites, des diarrhées et une transpiration excessive. L'étude d'interaction avec le THC n'a rapporté aucun événement grave, les effets observés restant légers à modérés et transitoires. Le point critique est un risque de classe plutôt qu'un signal propre à cette molécule : avec le rimonabant, les antagonistes et agonistes inverses du CB1 ont montré une augmentation des troubles de l'humeur, de l'anxiété et des idées suicidaires, motif de la suspension européenne d'octobre 2008 et de l'arrêt du développement du surinabant, de l'otenabant, de l'ibipinabant et du taranabant. Les données humaines de sécurité au long cours manquent pour le surinabant, et aucune étude publiée de toxicité chronique ne lui est consacrée.

Detection and analysis

Aucune méthode de dépistage de routine n'est documentée. Le surinabant ne figure pas dans les panels toxicologiques usuels, ni parmi les substances suivies par le système d'alerte précoce européen sur les nouvelles substances psychoactives. Les concentrations plasmatiques rapportées dans la littérature proviennent uniquement des dosages réalisés dans le cadre des essais cliniques du promoteur.
