- La molécule est née d'un programme pharmaceutique légitime. Les laboratoires Abbott ont exploré à partir de 2006 une série de cétones d'indole portant un groupe cycloalkyle, dans le but d'obtenir des agonistes sélectifs du récepteur CB2 capables de traiter la douleur sans les effets centraux liés à CB1. Le travail de Jennifer Frost et de ses collègues, publié en 2010 dans le Journal of Medicinal Chemistry, décrit une série de soixante-dix ligands indoliques dont la molécule fait partie, avec une sélectivité pour CB2 estimée à un facteur quatre-vingt-trois. Son détournement vers le marché des mélanges à fumer suit de peu la publication : elle apparaît dans les produits saisis en Europe et en Amérique du Nord à partir de 2012, comme substitut aux naphtoylindoles de la série JWH alors interdits. Le comité d'experts de l'Organisation mondiale de la santé l'a examinée une première fois en 2014, en recommandant une simple surveillance faute de données, puis à nouveau en novembre 2017.

UR-144 is a synthetic cannabinoid designed by a pharmaceutical laboratory, then diverted into the smoking mixtures sold as cannabis substitutes. It occurs neither in hemp nor in cannabidiol products. Its distinctive feature is a dangerous one: the heat of a cigarette partly converts it into another molecule, markedly more active on the brain than the parent compound. It is classified as a narcotic in France and is under international control.

Pharmacology
UR-144 belongs to the second generation of synthetic cannabinoid receptor agonists, that of the indole ketones bearing a tetramethylcyclopropane unit, which succeeded the naphthoylindoles of the JWH series after their prohibition. Born of an Abbott Laboratories programme aimed at CB2-selective agonists devoid of central effects, the molecule proved far less selective than announced: the preference for CB2 falls from an eighty-three-fold factor to a six-fold factor depending on assay conditions, and affinity for CB1, of the order of 29 nM, equals or exceeds that of tetrahydrocannabinol. In animals it fully substitutes for tetrahydrocannabinol in discrimination tests, which places it among the central agonists. The most important point for the user has to do with heat: the tetramethylcyclopropane unit opens at combustion temperature and yields a degradation product whose agonist activity at the human CB1 receptor is about four times that of the parent molecule. What is smoked is therefore not what was sprayed, and the dose received becomes impossible to estimate. The effects reported in humans come from case series rather than controlled studies: a review of thirty-nine Polish files from 2012 to 2015, twenty-six of them with the substance as the sole detected compound, chiefly notes slurred speech and dilated pupils, together with incoordination, unsteady gait and difficulty standing. Acute renal injury has been described in association with other synthetic agonists. No controlled study has been published in humans: human data are lacking, on doses as on tolerance and dependence.

No known biosynthetic pathway: the molecule is not formed in any living organism. It arises from organic synthesis, by acylation of the indole nitrogen followed by formation of the ketone bridge linking the indole nucleus to a tetramethylcyclopropane unit. Manufacture is described here only by its chemical class.
Legal framework
France
Classified as a narcotic in France, and designated by name. It appears in annex IV of the decree of 22 February 1990 under the wording "UR-144 or (1-pentylindol-3-yl)-(2,2,3,3-tetramethylcyclopropyl) methanone", an entry introduced by the decree of 14 October 2019. It also falls within the generic family of indol-3-yl methanones bearing an alkyl chain on the indole nitrogen and a cyclopropyl group on the carbon of the methanone bridge, a structural definition that would cover it even without a nominal entry. It is therefore not the generic clause covering tetrahydrocannabinols that applies here, since the molecule does not belong to that chemical class. Production, possession, transfer, supply and use are criminally punishable.
European Union
The molecule is today under international control: it appears in Schedule II of the 1971 Convention on Psychotropic Substances, under the identification code PU 001, in the Green List published by the International Narcotics Control Board. This listing followed the critical review conducted by the World Health Organization Expert Committee in November 2017, which concluded that the available data made the substance comparable to the synthetic cannabinoids already listed in that Schedule, after a first review in 2014 that had recommended surveillance only. No implementing decision of the Council of the European Union targets it specifically, control being a matter for national legislation: at the time of the 2017 review it was already prohibited in Germany since 2012, in the United Kingdom since 2013, as well as in Denmark, Hungary, Portugal, Slovakia, Slovenia, Turkey and Russia, listed in Schedule I of United States federal law, and controlled in China, Japan and the Republic of Korea.

Agoniste synthétique des récepteurs cannabinoïdes de la famille des cétones d'indole à unité tétraméthylcyclopropane, souvent classée avec les aminoalkylindoles. Un noyau indole porte en position 3 un pont cétone relié non pas à un naphtalène, comme dans la série JWH, mais à un cyclopropane portant quatre méthyles ; l'azote indolique porte une chaîne pentyle. Ce groupe tétraméthylcyclopropane est l'élément qui définit la sous-famille et qui explique sa fragilité thermique, le cycle à trois carbones s'ouvrant à la chaleur pour donner un dérivé à double liaison.
Pharmacokinetics
Aucune donnée systématique d'absorption, de distribution, de métabolisme et d'élimination n'existe chez l'être humain, comme le rappelle l'examen critique de l'Organisation mondiale de la santé de 2017. Les seules informations disponibles concernent la biotransformation. Chez la souris traitée, la molécule est largement métabolisée et excrétée dans l'urine principalement sous forme de conjugués glucuronides. Les concentrations sanguines relevées chez l'être humain dans les dossiers cliniques et médicolégaux sont basses, de l'ordre de quelques nanogrammes par millilitre : 6 ng/mL chez un patient présentant une atteinte rénale aiguë, 0,24 ng/mL dans le sérum d'un adolescent admis aux urgences. Aucune demi-vie, aucune biodisponibilité et aucun volume de distribution ne sont établis.
Metabolism
La phase I est dominée par la mono-hydroxylation, les dérivés monohydroxylés étant les métabolites les plus abondants tant sur modèle cellulaire que chez l'animal. L'étude sur microsomes hépatiques humains couvrant neuf isoformes du cytochrome P450 montre que la biotransformation est portée très majoritairement par le CYP3A4, qui attaque l'unité tétraméthylcyclopropane, avec une contribution mineure du CYP1A2. La phase II consiste en une glucuronoconjugaison poussée, à tel point que les composés retrouvés dans l'urine sont presque entièrement conjugués. Un travail conduit sur le champignon Cunninghamella elegans, utilisé comme modèle du métabolisme des mammifères, a mis en évidence une gamme plus large de transformations : hydroxylations multiples, formation d'aldéhyde et de cétone, oxydation en acide carboxylique et perte de la chaîne portée par l'azote. Une difficulté d'interprétation domine la toxicologie de cette molécule : plusieurs métabolites sont communs avec ceux de son analogue fluoré, ce qui complique l'attribution d'une exposition à l'une ou l'autre substance.
Toxicology and risks
Aucune étude toxicologique réglementaire n'a été conduite. Les données humaines se limitent à des séries de cas. La plus documentée porte sur trente-neuf dossiers polonais entre 2012 et 2015, dont vingt-six où la molécule était la seule substance détectée : les signes les plus constants sont un discours ralenti et une mydriase, associés à une incoordination motrice, une démarche instable, une difficulté à tenir debout et une réaction pupillaire anormale. Un homme de vingt-six ans ayant fumé pendant environ un an un produit contenant 61 mg par gramme de cette molécule et 69 mg par gramme de son analogue fluoré a présenté douleurs abdominales, nausées, vomissements et une atteinte rénale aiguë sans étiologie identifiée, résolutive en vingt-trois jours ; le rôle respectif des deux substances n'a pas pu être établi. Un décès a été rapporté chez un homme de trente-six ans effondré après avoir fumé, avec crises convulsives, et dont le sang fémoral contenait cinq agonistes de synthèse dont celui-ci à 6 ng/mL, ainsi que de l'amphétamine. Un produit contenant cette molécule et quatre autres agonistes a été associé à plus de deux cents hospitalisations et à un décès, sans qu'elle en soit la cause unique. Sur cellules, elle induit une toxicité des cardiomyoblastes mettant en jeu le calcium cytoplasmique. Le tableau général des agonistes de synthèse ajoute agitation, tachycardie, hypokaliémie, crises convulsives, psychose, tolérance, dépendance et sevrage. Le risque principal en usage réel tient à l'impossibilité de connaître la dose reçue, aggravée ici par la conversion thermique en un dégradant plus puissant.
Detection and analysis
La substance échappe aux tests immunologiques destinés au cannabis. Un travail de 2025 portant sur cinquante-trois analogues de cannabinoïdes montre toutefois qu'une partie des métabolites acides sont reconnus par les immunoessais dirigés contre les métabolites carboxyliques d'agonistes de synthèse, ce qui donne un dépistage d'orientation mais jamais une identification. La confirmation repose sur la chromatographie liquide couplée à la spectrométrie de masse en tandem, en urine sur les métabolites hydroxylés après déconjugaison, en sang sur la molécule mère à des concentrations de l'ordre du nanogramme par millilitre. L'analyse capillaire et l'analyse de la salive sont possibles et documentées. Deux pièges méritent attention : le dégradant thermique doit être recherché à côté de la molécule mère, faute de quoi une exposition réelle peut être manquée, et le partage de métabolites avec l'analogue fluoré interdit de conclure à l'une plutôt qu'à l'autre sur la seule base des marqueurs urinaires communs.
