- La molécule a été décrite en 2015 par Bogna Ignatowska-Jankowska, Aron Lichtman, Ruth Ross et leurs collaborateurs, dans un travail conduit entre l'université Virginia Commonwealth, l'université de Toronto, l'université de Virginie-Occidentale, le Research Triangle Institute et l'université d'Aberdeen, publié dans Neuropsychopharmacology. Le projet répondait à une impasse thérapeutique précise : le récepteur CB1 est une cible prometteuse pour la douleur, mais les agonistes orthostériques comme le tétrahydrocannabinol s'accompagnent d'effets psychotropes qui limitent leur emploi. Les modulateurs allostériques positifs offrent une autre voie, celle d'amplifier la signalisation existante plutôt que de l'imposer.

In plain terms
ZCZ-011 is not an ordinary cannabinoid: instead of activating the CB1 receptor itself, it sits alongside it and amplifies the signal of the molecules the body already produces. In mice, it relieves neuropathic pain without causing the psychotropic effects of THC. It is a research tool, never used in humans, and it is not classified as a narcotic in France.

Pharmacology
ZCZ-011 illustrates a pharmacological strategy different from that of every other compound in this encyclopaedia. The CB1 receptor is a recognised target for pain, but the agonists that occupy its orthosteric pocket, from tetrahydrocannabinol to synthetic agonists, produce psychotropic effects that limit their therapeutic use. A positive allosteric modulator proceeds otherwise: it binds at a distinct site and makes the receptor more sensitive to the ligands the organism already makes, without itself triggering signalling. The work published in 2015 in Neuropsychopharmacology showed that this molecule potentiates the binding of tritiated CP 55,940, amplifies anandamide-stimulated GTPγS binding in mouse brain membranes, and increases beta-arrestin recruitment and ERK phosphorylation at the human CB1 receptor. In the whole animal, it is brain-penetrant and increases the potency of orthosteric agonists in tests of antinociception, hypothermia, catalepsy and drug discrimination, but has no effect of its own in those same tests and produces neither place preference nor place aversion. The decisive finding is that, administered alone, it relieves pain in the chronic nerve constriction model and in the carrageenan inflammatory model, through a CB1 receptor-dependent mechanism and with no cannabimimetic effect: the authors see in this the first proof of principle that an allosteric modulator of CB1 could treat neuropathic and inflammatory pain without the adverse effects associated with this target. None of this has been verified in humans, no clinical study exists, and the molecule remains a laboratory reagent.

No known biosynthetic pathway: the molecule is produced by no plant and no living organism. It comes from organic synthesis, by addition of a methylated 2-phenylindole to a nitroalkene bearing a thiophene. Its manufacture is described here by chemical class only.
Legal framework

ZCZ-011 is not classified as a narcotic in France. It appears by name in no annex of the decree of 22 February 1990, and it falls within none of the generic clauses that annex IV devotes to synthetic cannabinoids: those clauses describe indol-3-yl methanones, indole or indazole carboxamides and carboxylates, cyclohexylphenols and neighbouring families, definitions that all presuppose a carbonyl bridge absent from this molecule. Nor does the generic clause on tetrahydrocannabinols, their esters and their ethers apply, for lack of a benzo[c]chromene skeleton. The compound therefore falls under the regime governing laboratory chemicals, which of course authorises no human use: no dose has been characterised in humans.
European Union
No control measure targets ZCZ-011 at European Union level, and the molecule appears in no schedule of the 1961 Convention on narcotic drugs or of the 1971 Convention on psychotropic substances. It has been the subject of neither a notification to the early warning system nor a risk assessment, which reflects its actual status: it is a pharmacology tool, with no consumer market. Allosteric modulators of the CB1 receptor remain to this day a field of preclinical research, none having obtained a marketing authorisation in the Union.

La molécule n'est pas un cannabinoïde au sens structural : c'est un 2-phénylindole portant un méthyle en position 6 et, en position 3, une chaîne nitroéthyle substituée par un thiophène. Il n'y a ni squelette terpénophénolique, ni pont méthanone ou carboxamide reliant l'indole à un système aromatique, si bien qu'aucune des familles structurales définies pour les agonistes de synthèse ne la décrit. Cette absence de ressemblance avec les ligands orthostériques est cohérente avec son mode d'action : la molécule se fixe sur un site allostérique du récepteur CB1, distinct de la poche que le tétrahydrocannabinol occupe.
Pharmacokinetics
Aucune donnée humaine. Chez la souris, le travail de 2015 établit que la molécule est cérébro-pénétrante, condition nécessaire à ses effets centraux, et les épreuves comportementales ont été conduites par voie intrapéritonéale. Aucune biodisponibilité orale, aucune demi-vie plasmatique et aucun volume de distribution ne sont publiés, chez l'animal comme chez l'être humain. La molécule n'ayant aucun usage de consommation, aucune donnée d'exposition par inhalation ou par voie orale n'existe.
Metabolism
Le métabolisme de la molécule n'a pas été caractérisé et aucun schéma métabolique n'est publié, ni sur hépatocytes humains, ni chez l'animal. Cette absence est cohérente avec son statut d'outil préclinique : les études de devenir métabolique sont conduites pour les candidats médicaments et pour les substances rencontrées en toxicologie médico-légale, deux catégories auxquelles ce composé n'appartient pas. Le groupement nitro et le noyau thiophène sont deux motifs auxquels la toxicologie prête habituellement attention en développement pharmaceutique, mais aucune donnée expérimentale ne documente leur devenir dans le cas présent.
Toxicology and risks
Aucune étude de toxicité systématique n'est publiée et aucune dose humaine n'est caractérisée. Le profil comportemental chez la souris est en revanche instructif : administrée seule, la molécule ne produit aucun effet dans les épreuves classiques du cannabinoïde et n'induit ni préférence ni aversion de place, ce qui suggère une absence de propriétés renforçantes propres dans ce modèle. Cette absence d'effet propre est précisément l'argument thérapeutique du concept de modulation allostérique, et non un certificat d'innocuité : elle ne dit rien de la tolérance, de la toxicité chronique ou du comportement de la molécule chez l'être humain, sujets sur lesquels rien n'a été étudié. Un modulateur qui amplifie la signalisation endocannabinoïde existante voit par ailleurs son effet dépendre du tonus endogène, variable d'un individu et d'un état physiologique à l'autre.
Detection and analysis
Aucune méthode de détection en matrice biologique n'a été développée pour cette molécule, et aucune n'était nécessaire : le composé ne circule pas comme produit de consommation et n'a jamais été signalé dans une saisie ni dans un échantillon toxicologique. Son identification en laboratoire relève des techniques ordinaires de chimie analytique, chromatographie couplée à la spectrométrie de masse et résonance magnétique nucléaire, par comparaison à un étalon de référence. Les immunoessais destinés au cannabis ne réagissent pas à cette structure, qui ne partage aucun motif avec le tétrahydrocannabinol.
