- La molécule est issue du laboratoire d'Alexandros Makriyannis, dans la lignée des aminoalkylindoles dérivés du pravadoline. Sa notoriété vient de l'usage qu'en a fait l'équipe de T. Philip Malan à l'université d'Arizona : le travail publié en 2001 dans la revue Pain, signé Malan, Mohab Ibrahim, Hongfeng Deng et leurs collègues, s'en est servi pour démontrer qu'une analgésie pouvait être obtenue par activation du récepteur CB2 seul, sans passer par CB1 ni par les effets centraux du cannabis. Cette démonstration a ouvert un champ entier de recherche sur les agonistes sélectifs de CB2 comme antalgiques dépourvus d'effet psychotrope. Le mécanisme a été précisé en 2005 dans les comptes rendus de l'Académie américaine des sciences, puis la pharmacologie de la molécule elle-même a été révisée en 2007 par une équipe de Wyeth, dont les résultats ont considérablement compliqué la lecture des travaux antérieurs.

In plain terms
AM-1241 is not a product intended for consumption: it is a laboratory tool. It served to show that a pain-relieving effect can be obtained by activating the CB2 receptor alone, without touching the receptor responsible for the effects of cannabis on the mind. More recent work has shown that its behaviour changes depending on the species studied, which requires results obtained in rodents to be reread with caution.

Pharmacology
AM-1241 is the CB2-selective agonist through which the idea was established, in 2001, that analgesia was possible without going through the CB1 receptor or the central effects of cannabis. The mechanism turned out to be indirect: activation of CB2 in the periphery triggers the release of endogenous opioids by skin keratinocytes, so that the analgesic effect is lifted by naloxone, a result published in 2005 in the Proceedings of the National Academy of Sciences. This molecule nevertheless remains an often-cited example of the fragility of pharmacological tools. The characterisation carried out in 2007 on human, rat and mouse CB2 receptors showed that the racemic mixture, used in almost the whole literature, acts as an agonist at the human receptor but as an inverse agonist at both rodent receptors, even though animal pain models rest on those species. To this is added a dissociation between affinity and efficacy: the R enantiomer binds more than forty times better than the S enantiomer, yet it is the S enantiomer, the only functional agonist at all three species, that produces the better analgesic effect in vivo. These two findings require the conclusions drawn from the racemate before 2007 to be reread carefully, and they illustrate the more general phenomenon of species- and assay-system-dependent agonism at the CB2 receptor. No human data exist: the molecule has never been the subject of a clinical trial and no human dose is characterised.

Origin (research tool)
No known biosynthetic pathway: the molecule is not formed in any living organism. It comes from organic synthesis in the aminoalkylindole family, by acylation at position 3 of an indole whose nitrogen bears a methylene chain linked to an N-methylated piperidine ring, the ketone bridge being attached to a benzene nucleus bearing an iodine atom and a nitro group. Its manufacture is described here only by its chemical class.

The molecule is not listed by name on any schedule, but its structure falls under the generic clause covering indol-3-yl methanones in annex IV of the decree of 22 February 1990. That text covers methanones borne at position 3 of an indole nucleus whose nitrogen is substituted in particular by a 1-(N-methylpiperidin-2-yl)methyl group, and whose methanone bridge carbon bears a naphthyl, benzyl, phenyl, cyclopropyl or adamantyl group, substituted or not. This molecule meets both conditions, with a methylene chain linked to an N-methylated piperidine on the indole nitrogen and a substituted phenyl on the ketone bridge. The closest analogue, AM-2233, which differs only in the group borne by the benzene nucleus, is moreover listed by name on the same schedule. Classification by generic clause is less robust before a court than listing by name, but it carries the same consequences: possession and use for research purposes fall under the authorisation regime applicable to narcotics.
European Union
No control measure at European Union level targets this molecule, which has been the subject neither of a notification to the early warning system on new psychoactive substances, nor of a risk assessment, nor of a Council implementing decision. It appears in no schedule of the 1961 Convention on narcotic drugs or of the 1971 Convention on psychotropic substances. Its status therefore depends on national generic clauses relating to aminoalkylindoles, whose scope varies considerably from one Member State to another: where the structural definition is broad, as in France, a laboratory reagent with no recreational use is captured without ever having been targeted by the legislator.

Aminoalkylindole de la famille des indol-3-yl méthanones, apparenté au pravadoline et proche parent de l'AM-2233. Un noyau indole porte sur son azote une chaîne méthylène reliée à un cycle pipéridine N-méthylé, ce qui crée un centre stéréogène et explique l'existence des deux énantiomères R et S dont la pharmacologie diffère. La position 3 porte un pont cétone relié à un noyau benzénique substitué par un iode en position 2 et un groupe nitro en position 5, combinaison responsable de la sélectivité pour CB2. La classe chimique est celle des cannabinoïdes de synthèse de première génération, ce qui explique la capture par les clauses génériques.
Pharmacokinetics
Aucune donnée chez l'être humain, la molécule n'ayant jamais fait l'objet d'un développement clinique. Chez l'animal, elle est administrée par voie intrapéritonéale, sous-cutanée ou locale selon le modèle, à des doses choisies pour l'expérience et non caractérisées par des études d'exposition dédiées. Aucune biodisponibilité orale, aucune demi-vie plasmatique et aucun volume de distribution n'ont été publiés pour cette molécule. La question du passage de la barrière hémato-encéphalique importe particulièrement ici, puisque l'intérêt de la molécule tient à une action périphérique sur CB2, mais elle n'a pas été résolue par des mesures d'exposition cérébrale.
Metabolism
Le devenir métabolique n'a pas été caractérisé. Aucun métabolite n'a été identifié et aucune enzyme n'a été mise en cause expérimentalement pour cette molécule. Les transformations attendues par analogie avec les aminoalkylindoles seraient la N-déméthylation du cycle pipéridine, son hydroxylation, la réduction du groupe nitro en amine et la coupure de la chaîne portée par l'azote indolique, mais aucune publication ne le documente et il ne s'agit donc que d'une extrapolation structurale. Une question reste entière : l'interconversion ou l'élimination différentielle des deux énantiomères in vivo n'a pas été étudiée, alors qu'ils n'ont ni la même affinité ni la même efficacité.
Toxicology and risks
Aucune étude de toxicité réglementaire, aucun essai clinique, aucun cas d'intoxication humaine. La molécule n'est pas commercialisée pour la consommation et n'apparaît pas dans les signalements des systèmes d'alerte précoce. Les travaux animaux publiés l'emploient comme outil pharmacologique et non comme objet d'étude toxicologique : ils ne rapportent pas de mortalité aux doses utilisées, mais cette absence de signalement ne vaut pas évaluation de sécurité. Aucune dose humaine n'est caractérisée. Le risque propre à cette molécule est d'ordre interprétatif : employée en racémique chez le rongeur, elle peut y produire un effet d'agoniste inverse là où l'expérimentateur attend un effet agoniste, ce qui a pu fausser des conclusions publiées avant 2007.
Detection and analysis
Aucune recherche de routine ne vise cette molécule en toxicologie médicolégale ou en contrôle antidopage, faute d'usage récréatif documenté. Son dosage relève des laboratoires de pharmacologie, par chromatographie liquide couplée à la spectrométrie de masse en tandem, l'atome d'iode et le groupe nitro conférant une masse élevée et une signature de fragmentation caractéristique. La séparation des deux énantiomères exige en outre une colonne chirale, étape rarement pratiquée alors qu'elle serait nécessaire pour interpréter une expérience conduite avec le racémique. La molécule est susceptible d'apparaître dans les criblages larges d'aminoalkylindoles, avec la même conséquence que pour les autres outils de cette classe : une détection en France renvoie à une clause générique sur les stupéfiants alors que le contexte d'usage est un laboratoire.
