La voie a été mise au jour en 2000 par Kevin Kozak, Scott Rowlinson et Lawrence Marnett, à l'université Vanderbilt, qui ont montré que la cyclooxygénase 2 oxygène le 2-arachidonoylglycérol aussi efficacement que l'acide arachidonique et engendre une famille de prostaglandines glycéryliques. La pharmacologie propre de la PGE2-G a été décrite en 2004 par Chaitanya Nirodi et ses collègues, dans le même laboratoire. Ses effets nociceptifs chez le rat ont été rapportés en 2008 par Shu-Jung Hu et son équipe, et le récepteur P2Y6 a été proposé comme cible en 2017 par Antje Brüser, Torsten Schöneberg et leurs collaborateurs.

The molecule is not formed in the Cannabis sativa plant: it appears in animal cells when cyclooxygenase 2 oxygenates 2-arachidonoylglycerol, the most abundant endocannabinoid in the body. It has been detected in activated macrophages and in the rat hind paw, at concentrations of the order of a hundred femtomoles per paw, far below those of 2-AG itself.

Pharmacology

PGE2-G illustrates the meeting point between the endocannabinoid system and the prostaglandin pathway. When cyclooxygenase 2 is induced, it diverts part of the 2-arachidonoylglycerol towards prostanoid chemistry: the cannabinoid signal is extinguished and a mediator with a distinct pharmacological profile appears. The molecule activates neither CB1 nor CB2, its affinity for the prostanoid EP receptors is far below that of PGE2, and it remains inactive at DP, FP, IP and TP. In macrophages it mobilises calcium and activates protein kinase C by a pathway that rimonabant does not block; the nucleotide receptor P2Y6 was proposed as a target in 2017, with potency of the order of the picomolar, but this attribution still rests on a limited number of studies. In rodents, local administration triggers allodynia and hyperalgesia. Part of the measured effects stems from hydrolysis of the ester into PGE2 by carboxylesterase 1, so that the contribution of the esterified form itself remains debated. Human data are lacking: there is neither a clinical trial nor descriptive pharmacology in humans, and no therapeutic benefit is established.

Biosynthetic pathway (in vivo)

2-arachidonoylglycerol, released from membrane phospholipids by diacylglycerol lipase, serves as a substrate for cyclooxygenase 2, which oxygenates it into the glyceryl endoperoxide PGH2-G. Prostaglandin E synthases then convert this intermediate into PGE2-G. Cyclooxygenase 1 does not accept this substrate, which makes the pathway dependent on inflammatory induction of COX-2. Non-steroidal anti-inflammatory drugs that inhibit COX-2, such as ibuprofen or nimesulide, cause tissue levels of PGE2-G to fall in rodents.

Le squelette cyclopentanone de la PGE2 est conservé, mais la fonction acide carboxylique libre est remplacée par une liaison ester avec le glycérol. La forme 2-glycéryle issue du 2-arachidonoylglycérol s'isomérise rapidement et de façon irréversible en forme 1(3)-glycéryle, ce qui complique l'interprétation des études pharmacologiques. Structurellement, il ne s'agit pas d'un cannabinoïde de type terpénophénolique.

Pharmacokinetics

Aucune donnée humaine d'administration n'existe, ni par voie orale, ni par voie inhalée. La molécule est intrinsèquement instable : la forme 2-glycéryle issue du 2-AG s'isomérise rapidement et de façon irréversible en forme 1(3)-glycéryle, si bien que les préparations utilisées en laboratoire sont des mélanges. Elle reste stable dans le plasma humain acellulaire, mais elle est dégradée dans le sang total avec une demi-vie voisine de sept minutes. Les concentrations endogènes sont très basses et restent sous le seuil de détection dans le cerveau et la moelle épinière du rat, sauf à regrouper les tissus de plusieurs animaux.

Metabolism

L'hydrolyse de la liaison ester libère de la PGE2 et du glycérol. Chez l'humain, la carboxylestérase 1 est la principale hydrolase responsable dans les monocytes et les macrophages, les carboxylestérases 1 et 2 y contribuant dans les lignées monocytaires THP-1. La monoacylglycérol lipase, qui dégrade efficacement le 2-AG, est un mauvais catalyseur pour la PGE2-G. L'inactivation de la carboxylestérase 1 atténue les effets pro-inflammatoires attribués à la molécule, ce qui indique qu'une part de ceux-ci passe par la PGE2 libérée plutôt que par l'ester lui-même.

Toxicology and risks

Les données humaines manquent totalement : aucun essai clinique, aucune exposition intentionnelle documentée, aucun cas rapporté. Chez le rongeur, l'injection intradermique provoque une allodynie mécanique et une hyperalgésie thermique à des doses de l'ordre du microgramme, ce qui en fait un médiateur pro-nociceptif expérimental et non une substance recherchée pour un effet agréable. Sur les cellules RAW264.7, l'action sur la voie NF-κB suit une courbe en cloche, activatrice aux faibles concentrations et inhibitrice au-delà. Sur les neutrophiles humains isolés, la molécule freine plusieurs fonctions effectrices, effet qui disparaît lorsque l'hydrolyse en PGE2 est empêchée. Il ne s'agit en aucun cas d'un produit de consommation : la molécule circule uniquement comme étalon analytique et outil de recherche biomédicale.

Detection and analysis

Le dosage repose sur la chromatographie liquide couplée à la spectrométrie de masse en tandem, en mode d'ionisation positive, l'ester glycérylique étant détecté sous forme d'adduit ammonium et distingué de la PGE2 libre par sa transition de fragmentation propre. Des méthodes validées quantifient simultanément les prostaglandines et leurs esters glycéryliques dans le plasma, la moelle épinière, les ganglions rachidiens et les articulations du rat. Aucun test toxicologique de routine ne recherche cette molécule : elle n'est ni un stupéfiant, ni un marqueur de consommation, et sa présence dans un échantillon biologique reflète simplement le métabolisme normal de l'organisme.
